Endovascular Doppler guide wire in renal arteries. Correlation with angiography in 20 patients.

RATIONALE AND OBJECTIVES: To evaluate the feasibility of endovascular Doppler recording in renal arteries and to compare the reliability of Doppler parameters for detection of renal artery stenosis. METHODS: The authors examined 36 renal arteries in 20 patients with a 0.018" endovascular flow wire. Peak systolic velocity and the renal artery-to-aortic peak were measured in the main renal artery. From intrarenal waveforms, acceleration, acceleration time, and the renal resistive index were obtained. Spectral analysis with consensus scoring of early systolic peak was also performed. RESULTS: Twelve significantly stenosed renal arteries and 26 normal renal arteries were examined with the Doppler guide wire, without complications. Peak systolic velocity was the only parameter significantly different in renal artery stenosis (P = 0.037). Peak systolic velocity also differed significantly between hypertensive and normotensive patients. Tardus parvus was specific for severe renal artery stenosis. CONCLUSIONS: Endovascular Doppler is a safe and accurate method for the determination of velocity measurements and may be useful during percutaneous renal artery revascularization.

[Radio-anatomic study of the angle of origin of dysplastic renal arteries].

PURPOSE: To compare the angulation at the origin of dysplastic renal arteries compared to atherosclerotic renal arteries, in order to improve the technique for percutaneous transluminal angioplasty of FMD. MATERIALS AND METHODS: Retrospective analysis of 40 aortograms in patients who underwent renal angioplasty for dysplastic stenosis, with comparison with 45 control aortograms (with or without atherosclerotic lesions of the renal arteries). The angle of implantation of the renal arteries was measured relative to the aortic axis in the frontal plane, taking into account only the angulation of its proximal segment. We identified three types of kidneys with regard to their position relative to the renal artery ostium. RESULTS: The angle of implantation of dysplastic renal arteries is significantly sharper compared with the control group (63.8 degrees vs 80.9 degrees, p = 0.0001), irrespective of the side. The angulation did not correlate with the position of the kidney or the direction of the renal artery, suggesting a congenital origin. CONCLUSION: The angle of implantation of the dysplastic renal arteries relative to the aortic axis in the frontal plane is sharper than the angle measured in non dysplastic renal arteries.

[Treatment using percutaneous arterial embolization of post-traumatic priapism in children].

High flow rate priapism was diagnosed in a 14-year-old adolescent 5 days after a fall onto the perineum. Highly selective embolization of the 2 internal pudendal arteries was performed with gelatin (SPONGEL). Rapid resolution of the priapism as well as return of normal erection were observed within 48 hours. Doppler ultrasound revealed a high flow rate and identified the side of the traumatized artery. Endovascular embolization allows effective treatment of traumatic arteriocavernous fistula.

Fish oil effects on tissular fatty acids and plasma lipid peroxidation in zinc deficient rats.

Zinc has been reported to play a key role in lipid metabolism as well as in defences against oxidative stress. The aim of this work was to investigate the fatty acid distribution (plasma, heart, kidney, liver) and peroxidation (plasma) in zinc-deficient rats fed with n-6 fatty acids (10% corn oil) or n3 fatty acid fish oil (10% Maxepa). Zinc deficiency led to a decreased tissular and plasma n-6/n-3 ratio both in triglycerides and phospholipids. This effect was more marked in the Maxepa group than in the corn oil group. In plasma, the TBARs/TG + PL ratio was significantly enhanced in zinc-deficient animals, especially in rats receiving Maxepa. With regard to these results, zinc deficiency could appear as an aggravating factor of oxidative risk when associated with a n-3 fatty acid-rich diet. This work draws attention to the harmful oxidative risk associated with patients' intake of fish oil concentrate, without taking into account their antioxidant dietary intakes and status.

[Angioplasty of renal artery stenoses before surgical treatment of subrenal abdominal aortic lesions].

PURPOSE: To evaluate the results of the association of a percutaneous transluminal renal angioplasty before a surgical revascularization of the infra-renal aorta. PATIENTS AND METHODS: Percutaneous transluminal angioplasty of one (n = 12) or two (n = 2) renal arteries have been performed in 14 patients (11 hypertensive and one with azotemia), 2 to 240 days (mean 54) before infra-renal aortic surgery (aneurysms: 8, occlusive disease: 6). RESULTS: 15 of the 16 dilatations were completely technically successful (residual stenosis less than 20%), with one groin hematoma treated medically. Mean duration of the surgical intervention were 137 minutes (occlusive disease) and 147 minutes (aneurysm). In the post-operative course were noted resolutive infections in two patients, and one death because of myocardial infarction. At the end of the follow-up (3 to 60 months, mean 32), all of the 13 patients alive had improved in terms of vascular disease. Hypertension was improved for 2 patients, stable for 10, worse for 1. Creatinine serum level was stable for 12 patients, and the renal insufficient was worsened. CONCLUSION: Combined percutaneous transluminal renal angioplasty and aortic revascularization seems to be reliable and safe; it can be an alternative to a simultaneous aortic and renal revascularization, notably for high-risk patients.

Cure of renovascular hypertension by percutaneous occlusion of a large idiopathic renal arteriovenous fistula. Case report.

Percutaneous balloon occlusion of a large hypertensive idiopathic renal arteriovenous fistula is reported. A lumbar thrill was noted in a 52-year-old hypertensive woman. Intravenous digital substraction angiography identified the lesion. After arteriographic assessment a percutaneous embolisation was performed with a releasable balloon positioned on a coaxial micro-catheter. Immediate post-procedural angiographic control demonstrated complete occlusion with a small segmental area of renal hypoperfusion. The lumbar thrill disappeared. Normalisation of the blood pressure was noted two days later and confirmed eight months later. Persistent occlusion of the fistula was angiographically confirmed, whereas the area of renal hypoperfusion had partially decreased. Releasable balloon percutaneous embolisation appears to be a safe alternative to surgery in the treatment of large renal arteriovenous fistulas.

[Spontaneous remission of renal artery stenosis].

Retrogression of a renal artery stenosis after a six months follow-up is reported in a 43 years old woman who stopped smoking after the first angiography and was treated with antiplatelet, suggesting spontaneous atherosclerosis injury revision.

Survey of management and housing in farrowing quarters among independent and integrated swine farms in Québec.

Forty-eight randomly selected owner-operated swine breeding farms (independent farms) and 38 belonging to 5 integrated organizations specializing in swine production chosen from the largest in the province of Québec (integrated farms) were separately described regarding their general characteristics, sow feeding, management practises, and housing features in farrowing quarters. The parallel description of these 2 groups of farms aids in understanding what is done in the field. It also provides insight into potential differences between independent and integrated farms. Generally speaking, production tended to be more specialized and concentrated in integrated organizations. Specifically, more new practises seemed to have been adopted on the integrated farms, and their stockpersons seemed to have a more proactive style of management in farrowing quarters. Increased size of operations, proximity of information sources, profits yielded by new practises, and ease of implementation are discussed as explanations for this higher rate of adoption of new techniques among the organizations. These differences between the independent farms and the integrated organizations appeared to be all related to basic differences in their respective sizes. Although some differences were observed within, as well as between, each organization, many similarities were found across the majority of farms within each organization, thus supporting the existence of policies specific to each organization. Although these findings have to be confirmed before being generalized, they tend to suggest that independent swine farms and integrated organizations should be considered differently.

Antioxidant vitamins in hospitalized elderly patients: analysed dietary intakes and biochemical status.

DESIGN: Descriptive study. SETTING: Geriatric department of the Grenoble University Hospital. SUBJECTS: 24 hospitalized elderly women: 13 long-stay patients and 11 in rehabilitation after femoral neck fracture. MAIN OUTCOME MEASURES: Retinol, carotene, tocopherol and vitamin C dietary intakes were evaluated by 5-day duplicate portion analysis. Circulating levels of retinol, beta-carotene, alpha-tocopherol and vitamin C were determined in parallel (HPLC). RESULTS: Mean intake of vitamin C (21 mg/d), and vitamin E (3.1 mg alpha-tocopherol equivalents TE/d) were low compared to recommendations, in relation with poor energy intake (5.27 MJ/d) and nutrient densities. More than 85% of the patients exhibited vitamin C and vitamin E intakes below two-thirds the recommendations (60 mg/d and 10 mg TE/d, respectively) and 50% did not meet recommendations for vitamin A (800 micrograms retinol equivalents/d). With the exception of retinol, dietary vitamin intakes were positively correlated to corresponding blood concentrations. No values below cut-off levels were found concerning plasma retinol, plasma tocopherol or ratio of alpha-tocopherol to cholesterol. In contrast, 26% and 32% of the elderly patients had low circulating levels of beta-carotene and vitamin C, respectively. CONCLUSIONS: The present study highlights low antioxidant vitamin intakes, particularly concerning vitamin E and vitamin C, and an important proportion of low blood vitamin C and beta-carotene concentrations in hospitalized elderly women. Further studies are needed to determine the actual requirements of hospitalized elderly patients and to evaluate the potential benefits of providing micronutrient-enriched foods to this population.

[Parietal calcifications of the kidney pelvis in Corynebacterium urealyticum urinary infection].

Linear renal pelvis calcifications in a native kidney due to Corynebacterium urealyticum are described. These micro-organisms have an opportunistic behaviour and can be responsible for nosocomial urinary tract infection. Alkaline incrusted cystitis with linear vesical calcifications are considered as the most typical pattern. Renal pelvis may also be concerned, with parietal calcifications.

Psychodemographic profile of stockpeople working on independent and integrated swine breeding farms in Quebec.

As a part of a larger study, the psychodemographic profile of Quebec pig stockpeople was described based on a sample of 48 persons working on independent farms randomly selected in the Richelieu-Saint-Hyacinthe region, and on a convenience sample of 38 others employed by 5 organizations of integrated swine production chosen among the largest ones in Québec. The 2 groups of stockpeople were described separately because of the differences between the 2 types of production. Demographic data (age, sex, level of education, training and experience in swine production, seniority on the farm) were obtained through a face-to-face interview. The personality profile was assessed using a standard personality test (French translation of the Sixteen Personality Factor Questionnaire). Both the stockpersons on independent farms and those employed by the organizations were more reserved, emotionally stable, serious, conscientious, unsentimental, controlled, introverted, and less anxious than the average person in the general population. However, multivariate analyses revealed some differences between these two groups of stock-persons (P = 0.021) as well as between the employees of the different organizations (P = 0.0038). These differences in personality profiles may reflect differences in working conditions, particularly regarding the human relationships, and in corporate cultures of the organizations.

Vitamin intakes (A, E, C, carotenes) in a French institution: assessment by the duplicate-diet method.

Duplicate meals of those eaten by 14 healthy young males (25-35 years of age), usual hospital diets for breakfast, lunch, and dinner, were collected for 5 consecutive days and freeze-dried. Fat-soluble vitamin (alpha-, beta-, gamma-, and delta-tocopherols, retinol, alpha- and beta-carotenes) and vitamin C contents were analyzed. Duplicate meals presented a wide range in vitamin levels. Large individual day-to-day variations were observed. The median daily intakes (n =70) of retinol equivalent (0.31 mg/day) and tocopherol equivalent (3.48 mg/day) were low. Only one third of the meals could provide sufficient amounts of fat-soluble vitamins, whereas vitamin C intakes were adequate (87.4 mg/day). So, fat-soluble recommended intakes were not supplied for most of the subjects in this study.

[Evolution of serum creatinine level 48 hours after angioplasty for renal artery stenosis in 91 patients].

To determine variations of serum creatinine level immediately after percutaneous angioplasty for renal artery stenosis. After technical success of unilateral or bilateral percutaneous angioplasty for renal artery stenosis in 91 patients (mean age 62.7 years) in whom ioxaglate was always used (90 to 300 ml, mean 195), serum creatinine levels immediately before and 48 hours after dilatation were compared. In the whole population, no significant variation was observed in the overall population. No significant difference was observed between unilateral and bilateral renal angioplasty. The volume of contrast media did not influence creatinine level, nor did iodine injection performed during (n = 4) or more than 3 days before angioplasty. A significant difference was observed with or without diabetes mellitus (n = 15, p = 0.05). In patients without diabetes mellitus technically successful percutaneous angioplasty for renal artery stenosis when using ioxaglate do not significant alter serum creatinine level 48 hours later.

Effects of varying dietary proteins on plasma lipids and rabbit platelet function.

The aim of this paper was to study the effect of dietary proteins on rabbit platelet aggregation induced by ADP, collagen and thrombin. In addition, blood coagulation tests (Stypven, Stypven cephalin and cephalin kaolin plasma clotting times) were performed on these rabbits fed semi-synthetic diets containing animal protein (casein) or vegetable proteins (walnut or soybean). As part of this study, total cholesterol, HDL cholesterol, phospholipid and triglyceride plasma levels were determined and platelet fatty acid composition was analysed. The results show that the addition of walnut meal in the diet decreases platelet aggregability and total plasma cholesterol. The relative effects of casein, soybean and walnut protein on platelet aggregability best correlated with the ratio of lysine to arginine in the protein and the ratio of HDL to LDL. An increase in the ratio C20:5 n-3/C20:4 n-6 in platelet fatty acids was also seen in walnut and soybean protein-fed animals, but the changes were greatest in soybean protein-fed animals while inhibition of platelet aggregation was greatest in walnut protein-fed animals.
