Genito-urinary tuberculosis--a local experience on its diagnosis and management.

Though the incidence of pulmonary tuberculosis has been on the decline, the incidence of genito-urinary tuberculosis has not decreased proportionately as the majority of patients have no associated pulmonary tuberculosis. Genito-urinary tuberculosis affects male and female equally and is commonest in the 4th decade of life. The most frequent presentation is that of persistent urinary tract infection in 2/3 of patients and gross haematuria in about 1/3 of our patients. A few may be relatively asymptomatic with loin ache. Though urine culture for acid-fast bacilli is important, diagnosis in most patients was established on histopathology and typical radiological features. The main stay of treatment is medical. The role of surgery is mainly reconstructive, to overcome strictures causing obstruction so as to preserve renal function. Close cooperation between physician and surgeon is important in the management of this disease.

Cancer of the pancreas--a clinical study of 22 patients.

A retrospective study of 22 patients with cancer of the pancreas seen in a medical department was carried out. The mean age was 61.7 years, although the youngest was only 32 years. Weight loss, pain and jaundice were the most frequent presenting symptoms. Hepatomegaly and jaundice were the most common physical findings. The gall bladder was palpable in 27.3%. Serum alkaline phosphatase, bilirubin and ESR were raised in most cases. Ultrasound examination was done in 72.7% of cases and was diagnostic in 62.5% with 12.5% falsely negative. Five patients had CT scan of the abdomen, of which 4 were diagnostic. Percutaneous transhepatic cholangiogram was positive in all the 8 patients where it was carried out. The cancer was of the pancreatic head in 72.7%. Gallstones were present in 22.7%. Secondary tumour involvement of the liver was present in 40.9%. Laparotomy was performed in 11 patients of which only 2 had a curative procedure (Whipple's operation). The overall prognosis was very poor: 54.5% died during the same admission.

Renal cell carcinoma - a local experience.

A review of 23 cases of renal cell carcinoma was undertaken. The sex distribution was 3.6 males to 1 female, the male preponderance being higher than in most reported series. The tumour was most common in the 6th and 7th decades of life. Haematuria was the commonest presenting symptom (52.2%); however, all the features of the classical triad of haematuria, loin pain and swelling were not present in any of our patients; 26% of patients presented with non-urological symptoms. At the time of diagnosis, four patients (17.3%) had evidence of metastasis. Another four developed secondaries after nephrectomy. Our approach to renal cell carcinoma has been aggressive and all 23 patients including those with metastasis were treated by nephrectomy.

Traumatic duodenal injuries.

Eleven cases of duodenal injury are reviewed. A correct preoperative diagnosis was made in only one patient. In 10 patients, the duodenum was disrupted in the second part. When duodenal injury was associated with devitalisation of tissue or there was delay in operative intervention, repair was aided by a serosal patch graft. In these high risk cases, an attempt to defunction the injured area was done by suction gastrostomy or gastrojejunostomy with vagotomy, Three patients died and in two of these, there was delay in surgical intervention.

Major salivary gland tumours.

Forty-one major salivary gland tumours and tumour-like glandular enlargements are reviewed. In nine patients the swellings could not be differentiated from a true neoplasm. The diagnostic and therapeutic approaches to these patients are discussed. The clinical features, diagnostic and surgical management of 31 salivary neoplasms involving the parotid and submandibular glands are correlated with their histological features.

The solitary thyroid nodule.

213 clinically solitary nodules were analysed. Their clinical pattern, diagnostic problems, pathological varieties and surgical management have been assessed. 9.9% of the nodules were malignant in this series.

Neurilemmoma of the vagus nerve.

A case of neurilemmoma of the cervical portion of the vagus nerve is reported to illustrate its common presentation as a lateral neck swelling often mistaken for more common neck lesions. Recognition of its vagal origin, followed by careful meticulous surgery is imperative for preservation of ipsilateral vocal cord mobility.

Unusual small intestinal obstruction in adolescent girls: the abdominal cocoon.

Ten cases of small intestinal obstruction seen over a period of 6 years (1971-6) in young girls within the narrow age range of 13-18 years are described. The patients were all within 2 years of menarche. In all these cases the obstruction was due to a membrane encasing the small intestine in the manner of a cocoon. There was no previous history of abdominal operation, peritonitis or prolonged drug intake. The clinical features, operative findings and management of the cases are presented. Possible causes of the condition are discussed. In view of the similar clinical presentation in all these patients, their conditions can be grouped as a clinical entity--'the abdominal cocoon'.

Adenomyoepithelioma of the breast. Report of a case with fine needle aspiration cytology and histologic, immunohistochemical and ultrastructural correlation.

A case of adenomyoepithelioma of the breast was studied by cytology, histology, immunohistochemistry and electron microscopy. Aspiration cytology of the tumor showed sheets and tubular-type clusters of cohesive, regular polygonal cells with abundant, fairly well preserved, pale, granular to finely vacuolated, clear cytoplasma and a low nuclear/cytoplasmic ratio. The nuclei were small, round and monotonous, with dispersed chromatin and small or inconspicuous nucleoli. Acinus formation was striking: an occasional gland showed a suggestion of an inner layer of cells with denser cytoplasm surrounded by an outer mantle of paler cells. The histologic, immunohistochemical and ultrastructural findings confirmed the two cell populations, constituting an inner epithelial and outer myoepithelial component, both participating in the neoplastic process. The differential diagnoses of various breast lesions with somewhat similar cytologic features are discussed.
