[Heart rupture as a complication of myocardial infarct in the acute stage. Case contributions and clinical course of survivors].

Heart rupture was found in 25 (4.4%) of 560 patients with myocardial infarction admitted to the Coronary Care Unit of the Department of Cardiology, Padua Medical School. Ten (40%) of them had a breaking of the free wall, 10 the interventricular septum, 4 (16%) a papillary muscle and 1 (4%) the ventricular wall with formation of a pseudoaneurysm. Thirteen patients died suddenly, the other 12 (48%) had cardiac catheterisation and later surgery. Four patients with interventricular defect died immediately after surgery, the other patients were discharged. Mortality was 68%; 30% on the patients with VSD as well as those with papillary muscle rupture and pseudo aneurysm survived.

[Right atrial-right ventricular anastomosis for correction of tricuspid atresia].

Primary correction of type 1 b tricuspid atresia with a right atrial-right ventricular anastomosis has been performed on a 23-month-old baby with excellent clinical and hemodynamic results. This type of operation, which has been described for the first time by Björk et al. in 3 patients aged 8, 9 and 12 years respectively, does not require any conduit or prosthesis since it employs the right atrial appendage and a patch of autologous pericardium. The authors review the surgical anatomy of tricuspid atresia, the various operative techniques so far employed for its correction, as well as other congenital heart diseases which could benefit from this surgical procedure. Moreover they underline the advantage of the latter technique which avoids the employment of any foreign material, can be performed in early infancy, and can be considered definitive since it obviates the need for initial palliative procedures.

[Ischemic alterations of the regional mechanical performance of the left ventricle. Study of parietal deformation as a vectorial time-dependent parameter and of pressure-length loops (author's transl)].

A study was carried out on 59 patients with significant stenosis (greater than or equal to 75%) in at least one coronary vessel. For each parietal region of the left ventricle the pattern of contraction was defined through a study of the regional deformation as vectorial time-dependent parameter and of the construction of the pressure-length loops. The different models obtained in basal conditions and through intervention (nitroglycerin and post-extrasystolic potentiation) were correlated with experimental data on ischemic alterations of the regional mechanical performance.

[Correlations between the patterns--dynamic--of regional contraction and the ECG-enzymatic-clinical signs of myocardial infarction (author's transl)].

60 patients with clinical signs of coronary artery disease who had a hemodynamic examination were studied. The study of the regional dynamics using the deformation as a vectorial time-dependent parameter and the pressure-length loops, was carried out on a total of 420 parietal regions of the left ventricle: the basal ventriculography, the post-extrasystolic potentiation and the ventriculography after nitroglycerin were evaluated. The dynamic regional pattern was correlated with clinical-enzimatic signs of necrosis in the electrocardiogram. The possibility of correlating signs of necrosis with particular patterns of hypokinesia, akinesia, dyskinesia which differ from corresponding "static" forms of asynergy in active pressure development and deformation (even in a brief systolic interval) seems to confirm the physiopathologic interpretation of the regional phenomenon based on the ratio between viable not viable myocardium -- as primary cause.
