Leucocyte function in children with kwashiorkor.

A study of leucocyte response to infection, polymorphonuclear leucocyte chemotaxis and bactericidal activity, and nitroblue tetrazolium (NBT) reduction in children with kwashiorkor was undertaken and compared with a control group. The results show that total leucocyte counts were depressed in children with kwashiorkor, and lymphopenia was not infrequent. NBT reduction was normal. Abnormal polymorphonuclear leucocyte chemotaxis and bactericidal activity, though frequently found in children with kwashiorkor, was shown to be dependent on infection and not on protein depletion per se. Therefore, apart from some impairment of leucocyte mobilization in the presence of infection, the qualtiy of polymorphonuclear function, as determined by the above techniques, appears to be normal in kwashiorkor.

Immunologic studies in patients with recurrent bronchopulmonary infections.

In view of the known association of several different immune deficiency disorders with infections of the respiratory tract, a number of immunologic parameters were investigated in 15 patients with recurrent brochopulmonary infections. The patients' neutrophils all demonstrated normal intracellular killing of Staphylococcus aureus, and responded normally to chemotactic stimuli. Although assays of complement 3c were normal in all patients' sera, defective total hemolytic complement activity was consistently shown in 1 patinets manifested elevated serum immunoglobulin concentrations. Severe alpha1 -antitrypsin deficiency (Pi ZZ phenotype) was discovered in 1 patient. Three patients displayed cutaneous anergy to a battery of skin-test antigens, but lymphocytes from all patients studied responded normally to in vitro phytohemagglutinin stimulation. A "blocking" factor found in the serum of 3 patients suppressed the in bitro transformation of normal Candida-responsive lymphocytes by Candida albicans antigen. "Cold" lymphocytotoxins were detected in the sera of 10 patients, but in only 3 of 19 control sera. Although the significance of these findings is unknown, they may possibly have pathogenetic relevance in a small number of the patients studied.

Production of leucocyte inhibitory factor (LIF) and macrophage inhibitory factor (MIF) by PHA-stimulated lymphocytes.

Supernatants from human mononuclear cells cultured with PHA inhibited the migration of both human polymorphonuclear leucocytes and guinea-pig peritoneal exudate cells, but not human mononuclear cells. Using ultrafiltration it was shown that these supernatants contained two inhibiting factors, the one with a molecular weight of 15,000-50,000 inhibited only guinea-pig peritoneal exudate cells (MIF), whereas the fraction containing molecules of a size between 50,000 and 75,000 specifically inhibited the migration of polymorphonuclear leucocytes (LIF). The polymorphonuclear leucocyte inhibiting activity was heat labile. It is suggested that the leucocyte migration inhibition test is dependent upon the production of a lymphokine (LIF) which acts specifically on polymorphonuclear leucocytes causing their inhibition of migration.
