The different fatty acids composition of lecithins in the sarcoplasmic reticulum vesicles and the membranes of adrenal chromaffin granules.

Calcium transporting membranes of sarcoplasmic reticulum (SR) and amine transporting membranes of chromaffin granules (AG) isolated from different animal species (ox, rabbit and rat) show similar values for their phospholipid phases (0.5 mumole phospholipids/mg protein). The AG-membranes, however, contain higher lysolecithin (LPC) (10 11%) than the SR-membranes (2-6%). In both types of membrane a characteristic distribution of fatty acids was found in the phosphatidylcholine (PC) fraction. In SR-membranes 18-carbon fatty acids are present in increasing order of unsaturation (18:0 less than 18:1 less than 18:2) and in AG-membranes in decreasing order (18:0 greater than 18:1 greater than 18:2). The fatty acid composition of PCs specific of each membrane seems to be dependent on the presence of different endogenous lysolecithins and suggest a fatty acid turnover in both positions of the PC by the de- and re-acylation cycle. The importance in membranes function with regard to the Ca2+ transport of SR and the amine transport of AG membranes as related to the typical lipid phase is discussed.

The acyltransferase activity of adrenal medulla and sacroplasmic reticulum of skeletal muscles.

The presence of acyl-CoA-lysolecithin-acyltransferase was studied in ox adrenal medulla fractionated by sucrose gradient centrifugation. The highest activity of this enzyme is localized in the microsomes and not in the membranes of chromaffin granules. Estimation of acyltransferase activity in fragments of sarcoplasmic reticulum (SR) from rabbit skeletal muscles before and after sucrose gradient centrifugation shows that in these membranes acyltransferase is a membrane bound enzyme. Both enzyme preparations transfer 18-carbon fatty acids in increasing order of unsaturation to 1-acyl-sn-glycer-3-phosphorylcholine (1-acyl-GPC) (18:0 less than 18:1 less than 18:2) and in decreasing order to 2-acyl-sn-glycero-3-phosphorylcholine (2-acyl-GPC) (18:0 greater than 18:1 greater than 18:2). For the acyltransferase present in ox adrenal medulla and fragments of sarcoplasmic reticulum, 1-acyl-sn-glycero-3-phosphorylethanolamine (1-acyl-GPE) is not a good acyl acceptor. The importance of acyltransferase in the de- and re-acylation cycle for the neogenesis of membranes-specific lecithin is discussed.

A follow-up of vasectomy clients in rural Bangladesh.

A one-year follow-up of 585 vasectomy clients sterilized at vasectomy camps in Shibpur and Shalna in rural Bangladesh showed that almost half of the clients were dissatisfied with their vasectomies. The majority of dissatisfied clients stated that their primary reasons for dissatisfaction were: (a) their ability to work had decreased and (b) they had not received all of the incentives they had been promised. However, 58% of the matched nonvasectomized controls also felt that their ability to work had decreased in the last year. Only 2%--7% of the dissatisfied clients cited decreased sexual performance as their primary reason for dissatisfaction. Satisfied vasectomy clients most frequently cited: (a) the permanence of this method of birth control and (b) the incentives they received as their reasons for satisfaction.

A follow-up of tubectomy clients in Bangladesh.

Five hundred twenty-one tubectomy clients from two health centers and one sterilization camp were interviewed 1-4 years after their tubectomies to determine: (a) demographic characteristics, (b) reasons for selecting tubectomy and (c) long-term satisfaction. Essentially no differences in demographic characteristics or long-term satisfaction were observed between urban and rural clients or by the type of center where the tubectomies were performed. Means ages of the clients were 31-32 years; mean parity was 7. Fifty-four percent of the rural and 85% of the urban patients had used contraceptives before tubectomy. Ninety-five percent or more of the patients stated that they were satisfied with the tubectomy.

Dizziness associated with discontinuation of oral contraceptives in Bangladesh.

Data from four studies in Bangladesh indicate that dizziness is a frequent complaint of women taking oral contraceptives and the most commonly cited reason for discontinuation. Results of a fifth study showed that multiple vitamin or glucose capsules were effective in treating dizziness in about half of the women studied. Although the cause of dizziness is not clear, it may be partly due to the small stature and poor nutritional status of many Bangladeshi women.

Synthesis of 5-chloro-1-(2,3-dideoxy-3-fluoro-beta-D-glycero-hex-2- enopyranose-4-ulosyl)uracil as potential anticancer/antiviral agent.

Peracetylated alpha-D-glucose was coupled with silylated 5-chlorouracil. The product (2) was deacetylated and 4',6'-hydroxyls were then protected with 4',6'-O-isopropylidene group. Fluorine was introduced at the 3'-position, followed by acetylation, deprotection, tritylation, oxidation and deritylation of subsequent compounds gave the target compound (10).

Socioeconomic and cultural influence in the causation of burns in the urban children of Bangladesh.

This prospective case-control study was conducted on 105 consecutive cases admitted to the burn units of Dhaka Shishu Hospital and Dhaka Medical College Hospital. The same number of controls were selected randomly from the community and matched with respect to age group, sex, and location of residence within the defined metropolitan area during the period January to September 1997. Both the controls and cases were divided into three age groups. Group A (19 controls and 19 cases) included neonates and infants, group B included children from 1 to 4 years, and group C included children from 4 to 12 years. The mean age of the controls in group A was 8.42+/-3.86 months and in group B and C was 5.72+/-4.05 years. The mean age of cases in group A was 4.79+/-4.05 months and in group B and C was 5.08+/-2.87 years. A scoring system was used to analyze 14 socioeconomic and cultural factors, which indicated highly significant differences between controls and cases (P < 0.001), and also between male (P < 0.001) and female (P < 0.001) controls and cases. There were significant differences between controls and cases in group A (P < 0.001), group B (P < 0.01), and group C (P < 0.001). Differences were not significant between males and females within groups A, B, and C for cases or controls (P > 0.05). Regression analysis of the total scores for cases and controls indicated a negative correlation between burns and the socioeconomic variables included in the study. There were highly significant associations between burns and lack of alertness to burns among parents (P < 0.001), clothing of manmade fabrics (P < 0.001), and cooking equipment in the kitchen within reach of children (P < 0.001). There was a significant association between burns and illiteracy of the mother (P < 0.01), housing located in slums and congested areas (P < 0.01), illiteracy of the father (P < 0.02), presence of preexisting impairment in children (P < 0.05), presence of a history of burns among siblings (P < 0.05), and low economic status of the parents (P < 0.05). No significant associations were indicated between controls and cases with respect to number of children in the family (P > 0.1), family pattern (P > 0.1), conjugal bonding between parents (P > 0.5), and sibling's death and disability from burns (P > 0.5). It is evident that an increase in alertness to burns, easy availability of clothing of natural fabrics, keeping cooking equipment beyond the reach of the children, increased literacy among parents and children, and improvement in family income might decrease the incidence of burns in children.

Community participation in national family planning programs: some organizational issues.

Increasing the involvement of communities in implementing national family planning programs has been advocated as a means of increasing the acceptability, availability, and sustainability of services provided. This article reviews the nature and extent of community participation in the national programs of Bangladesh, China, the Republic of Korea, the Philippines, and Thailand by analyzing the structures and processes through which participation is organized. Across all five countries a similar pattern of participation has emerged in which a community-based delivery system is supported by the involvement of community leaders in activities that promote family planning. Active participation in planning and management functions is, however, virtually nonexistent. This limited form of participation is attributed to the bureaucratic organization of national family planning programs that seek to implement policies with explicit demographic goals. Given these goals and organizational structures, however, the pattern of organizing participation observed is probably the most appropriate. Consequently, policymakers should be wary of expecting greater involvement by community members in program implementation activities or in contributing resources unless consideration is given to reorienting the goals and organizing programs that meet needs that are directly relevant to families and communities.

Maternal mortality in rural Bangladesh: the Jamalpur District.

During the 12-month period from September 1982 to August 1983, 9,317 live births and 58 maternal deaths were recorded in Melanda and Islampur upazilas in the Jamalpur district of rural Bangladesh, giving a maternal mortality rate of 62.3 per 10,000 live births. Maternal mortality was positively related to maternal age and parity, with the mortality risk rising very sharply beyond age 35 years, and beyond parity four among women aged 25-34 years in particular. The most common causes of maternal death were eclampsia (20.7 percent), septic abortion (20.7 percent), postpartum sepsis (10.3 percent), obstructed labor (10.3 percent), and antepartum and postpartum hemorrhage (10.3 percent). These findings indicate that family planning, by decreasing the likelihood of pregnancy after age 35 and parity four, can help reduce the proportion of women at risk of maternal mortality.
