Short latency and long latency auditory evoked responses in children with attention deficit disorder.

INTRODUCTION: Children diagnosed with attention deficit disorder (ADD) can present with different abnormalities in electrophysiological studies. OBJECTIVE: The purpose of this paper is to compare brainstem auditory (short latency) evoked responses (BSAER) and long latency auditory evoked responses (LLAER) in school children with and without ADD. MATERIALS AND METHODS: A normative study was carried out, 20 normal subjects were studied. All these patients underwent a study protocol including BSAER and LLAER. Eighteen school children diagnosed as ADD were included in the active group. Eighteen school children were selected as controls. All children from both groups underwent BSAER and LLAER. BSAER and LLAER results from both groups of patients were compared. RESULTS: Brainstem transmission was significantly longer in children with ADD. The latency of P300 was significantly longer in children with ADD. Also, mean amplitude of P300 was significantly decreased in children with ADD. CONCLUSION: The results of this study indicates that school children with ADD show significant abnormalities in BSAER and LLAER. These electrophysiological procedures involving the auditory system can be useful for the diagnosis of children with ADD.

[Dementias in the primary care setting].

OBJECTIVE: To study the epidemiological, demographic, social and family, and clinical variables of the population suffering dementia in our health district. DESIGN: A retrospective evaluation of medical records. SETTING: A rural health district. PATIENTS: All those patients with a diagnosis of dementia in 1995. MEASUREMENTS AND RESULTS: The evaluation was based on primary care medical records. The prevalence of dementia in the over-65s ranged, according to the population group, between 1.29 and 5.19 per 100 inhabitants. 90.2% of patients with dementia remained living at home. Their average age was 79.8 (SD, 7.4). 57% had Alzheimer's disease. 40% were in advanced stages of the disease (stage III and IV). Only 26% of the patients were referred to Neurology in 1995. 35% of the patients took no medication for their dementia. 45.6% were included in a home care programme. The main carer was the son or daughter in 47.8% of cases. Expectation of life from when dementia was diagnosed was 5.53 (SD 4.64) years. Patients with dementia died in their own homes in 47% of cases, the main cause of death being infection. CONCLUSIONS: In our rural environment most patients with dementia remained with their families. In most cases the aetiological diagnosis of dementia could be made from primary care.

Relationship between physiological and self-reported stressful response and psychosomatic disorders.

A revised version of the Atonomic Perception Questionnaire (Mandler, Mandler, & Uviller, 1958) was administered to 359 psychology students and the highest and lowest scoring (12.5%) males and females (N = 88) of this group were selected to participate in two experiments. In Exp. 1, skin conductance, muscle tension, respiratory rate, and heart rate were recorded prior to and during presentation of neutral and stressful visual stimuli. In Exp. 2, the same indices were recorded and the same procedure was used. Participants were instructed to rate physiological reactivity to the stimuli. Finally, a medical history questionnaire was mailed to each participant 1 wk. after completing the experiments. The data indicate that this version of the Autonomic Perception Questionnaire was predictive only of responsivity of skin conductance to stressful stimuli.

[Cost analysis and degree of hypercholesterolemia control in type 2 diabetic patients according to different groups and scientific societies].

OBJECTIVES: To analyse the cost and describe the degree of control of the hypercholesterolaemia of type-2 diabetic (DM2) patients according to different groups and scientific societies. DESIGN: Retrospective cross-sectional study. SETTING: Rural health area. PATIENTS: All DM2 patients monitored in the health area (n = 338). MEASUREMENTS AND MAIN RESULTS: Gathering of data on clinical histories, determination of cardiovascular risk, LDL-C concentration and total/HDL cholesterol index. DM2 prevalence was 3.08%; annual incidence, 2.55/1000; 45.2% presented hypercholesterolaemia; 16.9% received lipid-lowering treatment; and 20.1% (n = 68) had DM2 with cardiovascular disease. According to the criteria of the GEDAPS-99, European Consensus-98 (Framingham table), Sheffield Group (objectives of the European Consensus-98) and PAPPS-99 for primary prevention, 59.3%, 58.4%, 24.5% and 17.7%, respectively, of hypercholesterolaemic patients were poorly controlled. The percentages of hypercholesterolaemic patients who in each case should receive treatment were: 64.9%, 66.1%, 36.4% and 29.6%. Expenditure in thousands of pesetas every 28 days to achieve proper control of these patients was 282, 423, 274 and 117. The criteria on secondary prevention of the GEDAPS-99 and European Consensus-98 gave figures of 81% and 72.4%, respectively, of poor control of Hypercholesterolaemia. 82.7% and 77.5% of these patients should receive treatment. Expenditure in thousands of pesetas every 28 days to achieve proper control of these patients was 342 and 210. CONCLUSIONS: The degree of control and the costs of hypercholesterolaemia in both primary and secondary prevention vary enormously, depending on the criterion used by the group or scientific society.

[Simultaneous analysis of myocardial perfusion and ventricular function. Normal values in the Mexican population].

UNLABELLED: Myocardial perfusion imaging is an useful procedure in the evaluation of patients with coronary artery disease, Gated SPECT technique evaluates simultaneously perfusion and ventricular function, left ventricular ejection fraction (LVEF), ventricular volumes and the transient ischemic dilatation of the left ventricle. OBJECTIVE: To evaluate the normal ventricle volumes and the ejection fraction of the LV obtained automatically with the Gated SPECT in Mexican population. METHODS: 100 patients were studied with low likelihood for CAD. All of them were studied with Tc-99m Sestamibi Gated SPECT. We obtained automatically the LVEF, and the end diastolic and systolic left ventricular volumes. RESULTS: Myocardial perfusion, regional motion and systolic thickening were normal in all patients. We obtained the mean values of EF and end diastolic and systolic volumes. These values were lower in female. CONCLUSION: Tc-99m Sestamibi myocardial perfusion SPECT is an useful procedure, with high accuracy for the simultaneous evaluation of myocardial perfusion and ventricular function.

[Simultaneous assessment of myocardial perfusion and ventricular function using thallium-201 at rest/Tc-99m tetrofosmin at effort with single-photon emission computerized tomography, synchronized with electrocardiogram (synchronized SPECT). Initial experience in Latin America].

UNLABELLED: Gated SPECT is a useful technique to evaluate myocardial perfusion, wall motion abnormalities and wall thickening. There is not published experience in Latin-America using dual isotope rest Thallium-201/stress Tc-99m Tetrofosmin Gated SPECT. METHOD: We studied 27 patients with coronary artery disease. Coronary angiography was performed in all the patients. We used an injection at rest of 3 mCi of Thallium-201 followed by stress and the administration of 15 mCi of Tetrofosmin and Gated SPECT acquisition. Five days after, the patients were injected with 20 mCi of Sestamibi at stress, with a second Gated SPECT acquisition. Perfusion data were analyzed using 20 SPECT segment analysis. To study the wall motion, we divided the heart in 29 segments and every segment was scored using a 4 points scale (3 = normal, 2 = mild hipokinesia, 1 = severe hipokinesia, 0 = akinesia). RESULTS: The perfusion and wall motion segmental score agreement between stress Tc-99m tetrofosmin and stress Tc-99m sestamibi were 97% and 84% respectively. CONCLUSIONS: Rest Thailium-201/stress Tc-99m Tetrofosmin Dual isotope Gated SPECT technique is a good protocol to assess simultaneous myocardial perfusion and wall motion. There is a good agreement of the results with the use of rest Thallium-201/stress Tc-99m Sestamibi Dual isotope Gated SPECT study.

Ketamine antagonizes toxic action of anticholinesterase agents.

Ketamine is an anaesthetic interacting with several neurotransmitters. Among others, ketamine exerts some cholinergic actions (ACh). This paper presents the results of studying the interaction of ketamine with ACh in two animal species. Atropine slightly increased the time of immobility produced by ketamine injections in rats. Meanwhile, neostigmine slightly decreased such immobility. Ketamine resulted similar in behavioral actions and shared some electroencephalographic (EEG) actions of scopolamine in cats. The most striking interaction consisted on an antagonism of ketamine on the action of anticholinesterase agents. In both species, ketamine blocked the EEG and the behavioral toxic effects of neostigmine and physostigmine. Notwithstanding, the anticholinesterase agents were unable in reducing the actions of ketamine. This partial cholinergic agonist action of ketamine support certain but limited use of the anesthetic against insecticidal anticholinesterase poisoning.
