[Pyogenic liver abscess. Analysis and follow-up examination of a personal patient sample 1981-1992].

From January 1, 1981 to January 1, 1992 a total of 30 liver abscesses were treated in the University Clinic for Surgery in Cologne. 23 patients underwent an primary operative treatment and seven patients received a controlled guided percutaneous drainage. During the observation period four patients (13.3%) died by the effect of the liver abscess. 20 patients (66.7%) were subjected to clinical, laboratorical and computertomographical post examinations. The primary rate of operation success amounted to 47.8%, that of controlled guided percutaneous drainage of 42.8% (NS). The secondary success rate (that means after successful operative reintervention) amounted to 86.9% by the operative and to 85.7% by the percutaneous drainaged cases. During the post examination no relapse was determinated. Larger or chambered liver abscesses as well as extrahepatic spreading required operative drainage and, in cases of multifocal spreading, a resection with concomitant antibiotics. In case of solitary abscesses the controlled guided percutaneous drainage is the least harmful and low-prices method.

[Spieghel hernia--a rare cause of acute abdomen].

Spieghelian hernia is a rare rupture of the abdominal wall along the linea semilunaris. Due to the fact that the rupture is overlain by the aponeurosis of M. externus abdominis, it is often not recognized in its early stage. In most cases the rupture neck is mostly very narrow and in 24.1% of all cases an incarceration of intestinal parts has ensued which may even lead to the development of an acute abdomen. The first choice therapy will consist in an operative support including the reposition of the hernial content and a possible resection of ischaemic organs as well as removal and sealing of the hernia. Because of the rarity of this sickness we report on a patient in our clinic who was operated as an emergency.

[The compartment syndrome with special reference to vascular surgery aspects. A patient sample of the Cologne University Clinic 1981 to 1991].

The compartment syndrome, a well known complication in traumatized patients, is not taken enough notice of in the literature of vascular surgery despite it's obviously important pathophysiological context. Considering arterial occlusive disease the ischemia as well as the reperfusion can increase the compartmental volume respectively the tissue pressure. The subfascial pressure measurement should not lead to a false certainty, since no critical pressure values exist that can serve as a general criterion for diagnosis and treatment. The "thought about it" may be the first step to a clinical diagnosis and subsequent therapy. Decompression of all four compartments at the lower leg by the technique of Matsen represents the therapy of choice for the compartment syndrome of the lower leg.

[Treatment of advanced non-small cell bronchial cancer--a review of the literature of the last 5 years].

In the course of a metaanalysis concerning adjuvant therapy and the therapy of inoperable non-small-cell carcinoma of the lung, undertaken from 1.1. 1985 to 1.5. 1990, 376 publications were evaluated on a point system based on a decision tree. Out of a maximum of 71 obtainable points only 5.4% of the publications on adjuvant therapy and only 7.3% of publications on palliative therapy obtained more than 45 points. Regarding inoperable cases X-ray therapy with a median survival of 11 months and satisfactory life quality demonstrated the best results. Significant increases in survival are also possible with chemotherapy (7.4 months) in standard-dose pair combinations which, however, are associated with negative side effects on the treated patients. Immune and radio-chemotherapy cannot be recommended on account of treatment toxicity without a gain in life quality. Adjuvant therapy shows no life-prolonging effect. Consequently surgical treatment remains the first choice of therapy.

The origin of postembryonic neuroblasts in the ventral nerve cord of Drosophila melanogaster.

Embryonic and postembryonic neuroblasts in the thoracic ventral nerve cord of Drosophila melanogaster have the same origin. We have traced the development of threefold-labelled single precursor cells from the early gastrula stage to late larval stages. The technique allows in the same individual monitoring of progeny cells at embryonic stages (in vivo) and differentially staining embryonic and postembryonic progeny within the resulting neural clone at late postembryonic stages. The analysis reveals that postembryonic cells always appear together with embryonic cells in one clone. Furthermore, BrdU labelling suggests that the embryonic neuroblast itself rather than one of its progeny resumes proliferation as a postembryonic neuroblast. A second type of clone consists of embryonic progeny only.

[Treatment of respiratory tract infections with pefloxacin].

Pefloxacin, a 4-quinolone derivative, was administered in the dose of 800-1200 mg for the mean of 12 days to 24 patients with respiratory tract infection complicating chronic bronchial asthma or chronic obstructive lung disease. Patients with positive sputum culture and bacteria sensitive to pefloxacin were included in the study. Total eradication of the offending microorganisms was achieved in 54% of patients, and partial--in the next 20%. A poor efficacy of pefloxacin against Streptococcus species has been confirmed. In a few cases we have observed the development of resistance of isolated bacteria to pefloxacin during the course of treatment.

[Sexual dimorphism of the corpus callosum?].

Morphometric studies carried out during the past years have led to the assumption that the splenium of the corpus callosum in the female brain is significantly larger in size and more bulbous than in the case of men. This result, obtained by the examination of a relatively small number of samples, was to be checked by investigations with a larger number of samples. Our own planimetric studies were based on a total of 70 dissection cases and 55 nucleal magnetic tomograms (NMR), both evenly distributed among men and woman. When applying the identical measuring parameters as those used by the above cited authors, a sexual dimorphism could not be confirmed. This result did also not change when additional modified measuring parameters were taken into consideration.

Bioreactor for mammalian cell culture.

Purpose of this article is to review the current status of bioreactor design for mammalian cell culture. Morphological and biochemical features of two major mammalian cell groups, anchorage-dependent and independent cells are proposed as a basis for different behavior at their cultivation. Different bioreactor configurations are systematically discussed through enumerating elementary physical phenomena and through stressing their physiological significance. Special considerations are given to those areas which are inherent to mammalian cell bioreactor.

[Assessment of causality in fatal incidents in relation to coronary angiography].

Among 2,353 coronary angiographies (from 1.10.1984 until 30.9.1987) four lethal cases were seen. In two cases there was evidence for a causal relationship between coronary angiography and the lethal outcome (0.9%). Possible mechanisms of death are discussed. The clinical course and the time sequence between coronary angiography and the death are considered to be important criteria of a causal relationship.

An integration of cultivation and purification in membrane bioreactors: production of monoclonal antibodies and lymphokines by microencapsulated hybridomas.

Engineering and physiologic aspects of growth and production processes associated with encapsulated cells, mostly of anchorage-independent type, are reviewed. They include membrane diffusion characterization, mechanical properties of microcapsules, process limiting phenomena, physiological state of cells, process modeling and product purity and concentration. Finally, application areas are spelled out with an outlook discussion.

Oxidation of methanol, formaldehyde and formic acid by methanol-utilizing yeast.

Methanol-utilizing yeast, Candida boidini 11 Bh, characterized by high tolerance to methanol during growth, displays even higher tolerance when the oxidation rate by intact cells is tested. Low respiration activity is found even at 22% v/v of methanol. The half-saturation constant was 17-18 mM. The half-saturation constants for the two oxidation intermediates, formaldehyde and formic acid were 3.5-4.0 an .d 30-33 mM, respectively. When applied together with standard concentration of methanol, very low concentrations of both intermediates stimulated the oxidation rate. These results are discussed in connection with the relationship between growth and oxidation, the tolerance to high concentrations of inhibitory products and the mechanism of inhibition.

Application of mathematical optimization methods in microbiology.

Mathematical methods of experiment design have so far found little use in the optimization of microbiological processes. The conventional optimization procedure is still the transformation of functional relationship of n variables into n unidimensional optimizations; furthermore, the Box-Wilson gradient method is often used. This paper presents a review of methods used in other fields, and their application in microbiological practice. The methods are classified according to whether they require, besides the simple determination of the objective function (direct search methods), also the finding of its first (gradient methods) or second derivative (Newton-Raphson method). A modified Rosenbrock's method of direct optimum search and the gradient Box-Wilson method were used in parallel for the optimization of yeast growth on methanol. Their comparison showed that Rosenbrock's method is more suitbale for multiparameter systems.

Bioengineering problems connected with the use of conventional and unconventional raw materials in fermentation. A review.

Some bioengineering problems connected with the use of conventional and unconventional raw materials in fermentation research and industry are reviewed. They include the effect of the physical state of different substrates (solid, liquid, gaseous) and considerations of physico-chemical processes, especially the identification of limiting steps. A new concept of classification of fermentors with respect to the macromixing properties is suggested and its applicability for different substrates is considered.
