Significance of circulatory immune complexes in thromboangiitis obliterans (Buerger's disease).

Circulatory immune complexes were estimated in the sera of 25 patients and 20 age and sex matched controls by precipitation with 8% polyethylene glycol (PEG). Characterization of the immune complexes, thus obtained, was performed by the double immuno-diffusion method. The mean protein content of the PEG precipitates was significantly higher in the patients of thromboangiitis obliterans than in the controls. The immunoglobulins in the immune complexes were predominantly of mixed type in contrast to mainly single class immunoglobulins in the controls. C3 complement distribution in immune complexes of patients and control sera was 64% and 45% respectively. The immune complexes in patients were found to be biologically active. The mechanism of formation of circulatory immune complexes and their role in the pathogenesis of arteritis in relation to smoking of tobacco has been postulated.

Localization of delta 5-3 beta- and 17 beta-hydroxysteroid dehydrogenase activity in the efferent ducts, epididymis and vas deferens of the rabbit, hamster and marmoset monkey.

The presence and distribution of delta 5-3 beta-hydroxysteroid dehydrogenase (delta 5-3 beta-HSD: EC 1.1.1.51) and 17 beta-hydroxysteroid dehydrogenase (17 beta-HSD: EC 1.1.1.51) were studied histochemically in the excurrent ducts of the rabbit, hamster and marmoset monkey. Dehydroepiandrosterone (DHEA) and testosterone were used as substrates for delta 5-3 beta-HSD and 17 beta-HSD respectively, while phenanthroline monohydrate was used to eliminate non-specific staining due to other tissue dehydrogenases. The rabbit possessed least enzyme activity, which was confined to tubules in the middle segment of the epididymis. Enzyme activity was demonstrable throughout the excurrent ducts of the hamster and marmoset, with maximal staining occurring in the middle segment of the epididymis in both species. The region of maximum activity of hydroxysteroid dehydrogenase is where spermatozoa first develop their fertilizing capacity.

Ultrastructure of the clear cell lining the epididymal lumen of the rat.

2 types of clear cells can be distinguished in the rat epididymis. The Type I clear cell occurs in the middle segment and it is distinguished by a centrally located nucleus and the presence of lipid droplets. The Type II clear cell occurs in the terminal segment; it has a basally situated nucleus and does not have lipid droplets. Ultrastructural studies indicate that the clear cells carry out a dual function of absorbing substances from the epididymal lumen by pinocytosis and secreting products into it by merocriny. both these functions are not carried out simultaneously by a particular clear cell but at different times. The Type I clear cell appears to secrete a glycolipoprotein while the Type II clear cell seems to secrete glycoproteins.

Retroperitoneal liposarcoma.

The diagnosis of retroperitoneal fatty tumour is not extremely difficult provided that the possibility of its presence is kept in mind by the surgeon in all cases of distended abdomen in an apparently well-nourished patient. A careful clinical examination and a good excretory pyelogram are very important in the diagnosis. Surgical excision requires a considerable amount of patience and boldness on the part of the surgeon and is always gratifying, even if the tumour cannot be totally removed. Three cases of retroperitoneal tumour treated by us are presented. All three had had previous unsatisfactory treatment, the first two being incorrectly diagnosed and the third being inadequately excised. The pathology, diagnosis and treatment of these tumours are briefly discussed.

Land drainage as a factor in "red tide" development.

Although the exact mechanisms and factors of red tides are not fully understood, red tides are essentially a coastal phenomenon. The role of land drainage in the modification of coastal waters is discussed. The role of other factors (salinity, biologically active substances, etc.) that are altered by the process of land drainage is analyzed and correlated to experimental evidence derived from laboratory-grown dinoflagellate cultures.

Ileocaecal tuberculosis.

A retrospective study of 92 cases of ileocaecal tuberculosis, carried out at the Institute Hospital, New Delhi, is reported. The maximum incidence of the disease appeared to be between the ages of 20 and 40 years. There were 57 females and 35 males in the series. The majority of patients had symptoms for more than a year before they presented for treatment. Varying grades of intestianl obstruction constituted the usual clinical presentation, 44% of patients having subacute and 16% acute intestinal obstruction. Perianal fistula and abscesses were not seen. Most patients had no evidence of pulmonary tuberculosis. Barium X-ray studies were very helpful in the diagnosis. Bowel resection or short-circuiting gave very satisfactory results.

Carcinoma of the stomach.

Seventy-five cases of stomach carcinoma were analyzed. The incidence was highest between 41 and 50 years of age. Male to female ratio was 4:1. The average duration of symptoms was one year. There was no correlation between incidence and the dietary and smoking habits of the patients. Epigastric pain was the commonest complaint followed byanorexis, vomiting and loss of weight. Anemia was the commonest finding. Epigastric mass was present in nearly 50% of cases. Pyloric obstruction was common. Upper gastrointestinal x-ray studies were rewarding. Occult blood was often found in the stools. The majority of patients were about equally distributed between blood groups A and O. Many patients had no or low acid levels but 7.5% had normal or high acid levels. At operation, the growth was commonly found near the pylorus. Diffuse involvement was infrequent. Curative surgery could be done in only 20% of cases. One-third of the patients had palliative gastrojejunostomy. The operative mortality and morbidity were high. The overall five-year survival was only 5%.
