[Non-operative/conservative treatment of hemoperitoneum in children].

The authors present 34 cases of the traumatic hemoperitoneum with nonoperative/conservative treatment followed up in the past five years. The cases of hemoperitoneum are divided into three categories: hemorrhagic shock; stable clinic status; unstable clinic status. The first category included the cases of operational emergency, the other two categories were treated in the intensive care unit; the most of them needed no operation. The punction and punction-lavage of the peritoneum was systematically carried out and it was a therapeutic step of draining out the blood and appreciating the importance of hemoperitoneum. If laparotomy was requested initially or later on, a conservative surgery was used: spleen-hepatorrhaphy, partial splenectomy, selective arterial ligations, splenic replantation.

[Difference between dental plaque formation by Lactobacillus Sp. and Strep. mutans].

Human teeth were suspended in a basal medium with sucrose 10%; they were incudated for formation of dental plaques "in vitro". The microorganisms used were: Strep. mutans, Lactobacillus salivarius var. salivarius and a mixture of them. The following aspects were stablishad by scanning electron microscopy: 1) The teeth inoculated with Strep. mutans had coccoid forms, lying on an organic film with spheric spaces in some sectors, and irreguar forms in others (Fig. 1). 2) The teeth inoculated with Lactobacillus salivarius had rod forms attached to the adamantine surface (Fig. 2, 3). 3) Teeth incubated with the mixutre had rod forms over a granular film (Fig. 4). 4) The control teath did not have plaque (Fig. 5). This experience shows that the extracellular polisacharide, reticulated or granular (mutano) appeared only in teeth incubated with Strep. mutans alone (Fig. 1) or mixed with Lactobacillus (Fig. 4). On the other hand Lactobacillus appeared attached on the enamel surfaces, perhaps due to cohesive adhesive forces (Fig. 2). Two different interpretation or the formation of dental plaques that take place on the enamel free surfaces of teeth are presented.

[41 hepatic resections].

Clinical and therapeutical aspects are discussed with reference to 41 cases of malignant and benign tumours of the liver for which various types of hepatic resections were performed. The high mortality rate (22%) and short postoperative survival (less than a year) in malignant tumours of the liver plead for limited hepatic exeresis in cancer. Hepatic exeresis in benign tumoural pathology gave better, but nevertheless restricted, immediate and late results. Hepatic scintigraphy with colloidal 198Au and 131I rose bengal proved to be useful both preoperatively for determining the quality and quantity of the intact hepatic parenchyma and for late, postoperative follow up of the hepatic regeneration process. Moderate intraoperative hypothermia was used as an adjuvant method, allowing for the approach of broader hepatic exeresis by scissural access and longer clamping of the hepatic pedicles and inferior vena cava.

[Cholecystectomy in asymptomatic gallstones. Indications, opportunities and arguments in favor of surgical intervention].

Asymptomatic biliary lithiasis (discovered accidentally by paraclinical methods or intraoperatively) is, despite its hidden character, a state of disease. Asymptomatic lithiasis--having a frequency of 1.3%-7.5%--requires the use of several therapeutical measures. In this way some severe complications at the onset or at the turning of disease into a clinically manifest suffering, especially after the age of 60-70 years, or concomitantly with other affections, might be avoided. The conservatory therapeutical means have limited indications, or results dependent on several parameters, and some methods require a special equipment. The author pleads for cholecystectomy, which, if no complication appears, offers a radical solution with minimum morbidity and unsignificant postsurgical mortality indices.
