Hyaline cast formation in patients treated with cephaloridine.

The administration of 6 g. cephaloridine daily produced large numbers of hyaline casts during treatment, occasionally accompanied by red and white cells and albumin. These cleared when treatment ended and are of uncertain and doubtful significance.

Doppler-derived aortic flow measurements during and after heavy isometric exercise in healthy men versus men with myocardial infarction.

Doppler echocardiography is a useful noninvasive determination of left ventricular function during dynamic exercise. Scarce data are available for the use of this technique during heavy isometric exercise. Therefore, Doppler-derived aortic flow indexes were assessed during and after 50% maximal upper-body isometric exercise in 25 healthy men (aged 47 +/- 6 years) and compared with those of 22 men (aged 48 +/- 9 years) who had suffered myocardial infarction. The heart rate increased (p = 0.01) in each of the groups from a mean of 68 +/- 12 at rest to 84 +/- 11 during isometric exercise. At rest, systolic blood pressure was higher (p = 0.05) in the patients with coronary artery disease. During exercise, the patients with cardiac disease, compared with the healthy volunteers, demonstrated a lesser reduction in flow velocity integral, stroke volume, and cardiac indexes (p = 0.001). Immediately on recovery, the patients with cardiac disease, compared with the healthy group, showed significantly greater (p = 0.001) increase in stroke volume and cardiac indexes. At 3 minute's recovery, the stroke volume index continued to increase in the patients with cardiac disease, while the healthy group showed a decrease to below its resting value. Although 50% of maximal upper-body isometric exercise caused similar heart rate and systolic blood pressure responses in healthy patients and patients with cardiac disease, there were significant group differences in Doppler-derived left ventricular systolic function indexes, which were greatest on immediate and 3 minute's recovery. The results suggest that this novel isometric test may be useful in clinical testing.

Updated review of the coronary artery bypass grafting option in octogenarians: good tidings.

This review of the literature is presented in an attempt to keep pace with the constantly changing statistics in postsurgical outcomes, which reflect the considerable strides in technology and technical skills that have been made during the past decade in cardiovascular surgery. Although there is still much to be done in terms of improving surgical techniques and managing emergency cases, the reported results of coronary artery bypass grafting in octogenarians are similar to or even better than those obtained with medical or angioplastic approaches, and the life expectancy and quality of life are similar to or better than those of age-matched controls.

Psychological distress around menopause.

The authors sought to identify a subgroup of women who are likely to experience psychological distress in the period around menopause. A sample of 189 women (mean age=49.49) was selected from the general population and rated for menopausal status, menopausal symptoms, depression, anxiety, perceived control, body image, and sex role. Menopausal symptoms were not found to discriminate between pre-, peri-, and postmenopausal women. Factor analysis of the symptoms yielded a specific somatic factor that correlated with menopausal status but not with psychological variables and a nonspecific psychosomatic factor that correlated with psychological variables but not with menopausal status. This suggests that psychological distress during the menopausal transition may indicate a personal psychological or physiological vulnerability rather than a specific reaction to the menopausal events.

Motion sickness-like syndrome among tank simulator drivers.

A military tank driving simulator has recently been introduced as a training aid for tank drivers in the Israel Defense Forces. Reports of nausea and vomiting among the first users of the simulator launched our investigation of the possible existence of a motion sickness-like syndrome among simulator drivers. Although the 59 subjects drove the simulator without any report of vomiting, other motion sickness-like symptoms were frequently reported. A comparison of symptoms reported after simulator and real tank driving show that dizziness, nausea, disorientation and hypersalivation were more frequently reported by simulator drivers and were of greater intensity. However, sweating and drowsiness were more prevalent among real tank drivers. The objective effect of driving the simulator was evaluated by instability and performance tests that were conducted before, during and after driving the simulator. A greater decrement in test results was observed among subjects reporting higher frequency of motion sickness-like symptoms.

A pilot occupational health service for small enterprises: first year experience--the Givat Shaul project.

An experimental Occupational Health and Safety Center was established as a pilot project for Israel in the Givat Shaul industrial area of Jerusalem. The center provides occupational health services to those working in its vicinity and is particularly oriented to employees of small enterprises (i.e., fewer than 20 employees). In addition to the routine occupational health services supplied by each occupational health clinic in Israel, the Center's staff also performs safety and hygiene inspections at workplaces, and provides social care services for the working population of the area. This paper summarizes the first year of the project's activities, particularly those of the clinical staff and the safety technician. Projected future activities are also outlined. The effectiveness of this project is currently under study and will be finally evaluated by an independent Hebrew University research team. If their evaluation shows the project to have been successful, establishment of similar centers in other parts of the country will be considered.
