Hysteroscopy in perimenopausal and postmenopausal women with abnormal uterine bleeding.

From February 1983 to January 1985, we performed outpatient microhysteroscopic examinations on 618 women 45 years of age or older with abnormal uterine bleeding (AUB). Three hundred thirty-four (54%) had normal and functional or hypoatrophic endometrium, 78 (12.6%) had low-risk hyperplasia, 8 (1.3%) had high-risk hyperplasia, and 66 (10.6%) had adenocarcinoma. Correlation with histologic findings revealed the considerable diagnostic accuracy of the technique: its reliability approaches 100% when one deals with endometrial neoplasia, 87.5% with high-risk hyperplasia and 65.2% with low-risk hyperplasia. The diagnosis cannot rely on hysteroscopic examination only. A biopsy can be performed during the examination or immediately thereafter. In 54.1% of AUB patients, no endometrial changes could be detected on hysteroscopy and biopsy. Curettage, therefore, would have resulted in overtreatment of these patients. Moreover, the usefulness of dilatation and curettage in about half of AUB patients over 45 should be questioned seriously.

[Epidemiological study on the assay of antibodies to anti-TORCH agents and to HBV in a group of mother/child pairs in Palermo].

The TORCH group: toxoplasma, rubeola, cytomegalovirus (CMV), herpes virus hominis (HSV) and also the B-virus of hepatitis (HBV) determine congenital malformations in the newborns. For this reason, the Authors leaded an epidemiologic study testing with some antibodies (anti-TORCH and anti-HBV) the serum of groups of mother-newborn of the Obstetrical Clinic of Palermo. This study became manageable because now there are available high specific immunoenzymatic methods, for example ELISA, the passive haemoagglutination and the haemoagglutination-inhibition. In our people, high concentration of rubeola, HSV and CMV and very low concentration of toxoplasma and HBV are shown. The frequency of malformation is related with the subclinical infection. The Authors suggest to extend same investigation at an higher number of pregnant women to prevent these congenital malformations.

Hysteroscopic patterns in women receiving replacement estrogen therapy.

There is general agreement that unopposed estrogen therapy can cause cystic hyperplasia of the endometrial mucosa, and it is also possible a progression of this lesion towards borderline forms of hyperplasia and adenocarcinoma in patients where there is a predisposition. The Authors report their experience of a combined, hysteroscopic and histological, study made on a group of patients who had undergone cyclic therapy with conjugated estrogens. In this group the more complex forms of hyperplasia were found in those patients treated with the higher dose of estrogens. The Authors emphasize that cyclic therapy of small doses of estrogens associated with 7-12 days of progestins every month can prevent or correct endometrial cystic hyperplasia. The microhysteroscope is a new instrument very simple to use; because of its accuracy, it makes the gynecologist able to perform an immediate endo-uterine diagnosis, thus avoiding some of the other techniques used in the examination of the uterine cavity; in the Authors experience this very interesting endoscopic method has proved to be extremely useful in the controls of patients receiving replacement hormone treatment for menopausal troubles.

[Methodological problems in the induction of exercise-induced asthma in atopic patients].

The considerable discrepancy in results reported by the literature on the question of exercise induced asthma (EIA) poses some methodological problems as regards the standardisation fo such tests. As the variability of the response depends to a great extent on the duration of the physical effort (and, to a smaller extent, on other quality features of the exercise), the importance of being able to deploy an effective, standardised control instrument, with characteristics of simplicity and easy repeatability, is evident. An exercise test with these features is proposed here and significant results in an early sample of ten patients suffering from atopical asthma are reported.

Endodermal sinus tumor of the ovary.

Endodermal sinus tumor (EST) of the ovary is a germ cell tumor that is now recognized more frequently than in the past. Since problems concerning natural history, prognosis and therapy of these neoplasms are not entirely resolved, it should be underlined the need to report new cases. The authors describe four cases of EST with reference to serial alphafetoprotein determination by radioimmunoassay; they also emphasize the importance of this technique as a very sensitive method in monitoring therapy and in detecting recurrences of disease even before any clinical evidence.

Nomifensine in the diagnosis of hyperprolactinemia.

The Authors try out Nomifensine, a new antidepressant drug which activates dopaminergic neurotransmission by inhibiting the DA reuptake. The patients tested consisted of: 11 normoprolactinemic control patients; 2 hyperprolactinemic patients with sellar alterations; 5 hyperprolactinemic patients with no radiological alterations; 6 patients with micro-adenomas; 1 patient with macroadenoma. We administered 200 mg of the drug orally and then took blood samples for 4 hours. We conclude positively that nomifensine is a useful complement to the diagnostic collage for discriminating between tumorous and non-tumorous hyperprolactinemia, and we look forward to further research which will increase the literature on this subject and which will explain some mechanisms and effects which have not yet been clarified, and we hope that this research will further prove the importance of the test.

[Radiopaque hemithorax, with special reference to scintigraphic methods].

Radioopaque hemithorax is frequently observed. The various situations in which it may occur are examined aetiopathologically, along with the general and specifically radiological methods used for its investigation. Particular attention is given to the employment of radio-isotopes for this purpose and the methods involved. Results obtained with perfusion are noteworthy, and even more so those achieved with positive tracers such as 67Ga citrate and 57Co Blm, especially in the differential diagnosis of pleural effusion and mesothelioma through what is described as the "paradox effect".

True pubertas praecox in a girl; onset at nineteen months.

The authors report a case of true pubertas praecox in a girl with onset at 19 months; they describe the principal histopathologic, clinical and laboratory features and discuss problems of etiopathogenesis and differential diagnosis, as well as the prognosis and treatment of this disorder.

[Gitelman syndrome in a 11 year old boy: incidental or delayed diagnosis?].

Gitelman syndrome (GS) is a heritable renal disorder characterized by hypokalemia, hypomagnesemia, metabolic alkalosis and hypocalciuria. Patients with GS usually have mild symptoms as muscular weakness and cramps, and they often are diagnosed in older age during routine investigation. We report a 11 year old boy with GS.

The use of three different therapeutic protocols according to the immunologic pathology present in three women with recurrent spontaneous abortion and fetal death for immunologic reasons: flucortolone and salicylates; high-dose intravenous gammaglobulins; subcutaneous heparin.

The authors report three cases of immunologic spontaneous abortions and fetal death. In the first patient there was lupus-like anticoagulant activity with a diagnosis of sub-clinical autoimmune disease; the second showed inadequate blocking factor activity, while the third subject presented excessive lymphocytotoxicity. In these cases three different therapeutic protocols were successfully used: flucortolone and salicylates, high-dose intravenous gammaglobulins and subcutaneous heparin.

Ultrasonography and blood flow velocity waveform assessment in human fetuses.

The authors report their experience with the use of ultrasonography and fetal blood flow measurement in the evaluation of fetal conditions recording patients with previous repeated immunologic abortion (R.I.A.) and with pre-eclampsia. In both groups of patients, the two biophysical methods, proved to be extremely valuable indicators of the fetal conditions.

Uteroplacental blood flow in pregnant women with hypertension and recurrent spontaneous abortion.

Uterine velocimetry was performed in 13 pregnant women with pregnancy induced hypertension (PIH) and in 15 pregnant women with recurrent spontaneous abortion syndrome (RSA-S). Measurement were performed on both uterine arteries and averaged. 75% of PIH group had RI 95th percentile from the first measurement (20 weeks gestation). In the RSA-S group, RI was between 75th and 95th percentile, suggesting an almost pathological vascularization.
