Selenium status in relation to clinical variables and corticosteroid treatment in rheumatoid arthritis.

Plasma selenium levels, erythrocyte selenium levels and activity of the selenoenzyme glutathione peroxidase in erythrocytes were determined in patients with rheumatoid arthritis (RA) and acute inflammatory arthritis. Results were compared with those from age and sex matched controls. These variables were not statistically different from controls in patients with inflammatory arthritis and in patients with RA not treated with corticosteroids. No correlation was found in RA between plasma selenium biological variables of inflammation and most clinical indices of disease severity. Therefore, acute or chronic inflammation was not the main factor that accounted for low plasma selenium levels in RA. Corticosteroid treatment, particularly at high doses (20-60 mg prednisolone/day), was significantly related to the depressed plasma selenium levels of some patients with RA. The mechanisms underlying this modification remain poorly understood.

About five cases of acute rheumatic fever in the adult.

We describe five cases of acute rheumatic fever in adults (applying Jones's criteria and exclusion of other common causes of acute polyarthritis in adults). The polyarthritis was migratory in three patients and additive in two. Severe tenosynovitis was present in three patients. Only one patient had carditis and received corticoids. The others improved with aspirin. The disease subsided after a time period varying from one week to two months and did not relapse.

Ochronosis: a case report with severe ochronotic arthropathy.

Alkaptonuria is a rare inborn metabolic disorder in which ochronotic pigment is deposited in connective tissue and cartilage. Ochronotic arthropathy is the consequence of longstanding alkaptonuria and leads to progressive joint disability. We report a case of a 67-year old man with severe ochronotic arthropathy involving the spine, the knees, the shoulders and the hips.

Hypouricemia and intravenous infusions.

The clinical significance of hypouricemia in hospitalized patients was studied retrospectively and prospectively. Two thousand two hundred patients routinely screened for serum uric acid were reviewed: 88 (4%) had serum uric acid levels below 2.5 mg/dl and 23 (1%) below 2 mg/dl. We found that 32 (36.4%) patients with serum uric acid below 2.5 mg/dl were patients treated with forced diuresis. Thereafter, to confirm the importance of perfusions in the evaluation of serum uric acid levels, 20 suicide-attempt patients and 7 patients with renal colic all treated with forced diuresis, were prospectively studied. Our results show that perfusions significantly increased the fractional excretion of uric acid that was responsible for the observed decrease of serum uric acid level.

Acute pulmonary edema following carbon monoxide poisoning.

This report describes a patient who presented with coma and acute pulmonary edema after severe carbon monoxide poisoning. Hemodynamic evaluation revealed elevated systemic and pulmonary arterial, pulmonary wedge and right atrial pressures, together with an increased cardiac output. These findings are compatible with the hypothesis that a neurogenic mechanism plays a role in the pulmonary edema of carbon monoxide poisoning.
