Papular and papulovesicular acrolocated eruptions and viral infections.

Twenty-six children with a symmetric papular or papulovesicular acrolocated eruption were studied to evaluate a possible link with viral infection. A history of a recent immunization was present in four patients, two of whom had a polio vaccine virus isolated from their stool samples. There was serologic evidence of a recent Epstein-Barr virus infection in one patient, adenovirus infection in another, a concurrent Epstein-Barr virus and adenovirus infection in a third, and cytomegalovirus infection in a fourth. Viral cultures were positive in two patients for adenovirus and in two others for rotavirus. Although some of the virologic findings could have been coincidental, this study confirms that multiple viral infections can be related to most papular and papulovesicular acrolocated eruptions of childhood. Virologic investigations in the earlier phases of the eruption are recommended to identify the etiologic factors of these lesions.

[Iso and Kikuchi syndrome. Description of a case].

A 2 1/2-year old Italian boy affected by Iso and Kikuchi syndrome is reported. The clinical examination showed hemi-onychogryphosis of both index fingers. Roentgenograms of the affected fingers showed bifurcation of the distal phalanges in the lateral view.

[The Lupus Band Test and serology of lupus erythematosus. Our experience].

The Authors studied, from 1986 to 1988, 84 patients (24 men and 60 women, in an age group that went from 13 to 75 years, with an average of 42 years) in whom DLE was diagnosed. In these patients, a direct immunofluorescent assay was carried out on the affected skin, on the non-affected photoexposed skin, and on the non-affected non-photoexposed skin: furthermore, the serum of these patients was tested for the presence of antinuclear antibodies (ANA), antibodies to extractable nuclear antigens (anti-ENA), and antibodies to double-stranded DNA (anti-DNA). In the study, a significant correlation between the presence of ANA and entity of cutaneous involvement at the Lupus Band Test was found. Furthermore, it was noticed that anti-ENA and/or anti-nDNA tend to be present in association to a positive Lupus Band Test in all 3 biopsied tissues.

[Lichen striatus: a clinical and histological study of 8 cases].

The authors describe eight patients with Lichen Striatus: history, clinical description and pathologic findings are discussed. Particularly, the histologic picture of Lichen Striatus is not uniform and changes, as it seems, with the "oldness" of the lesion.

Porokeratosis palmaris et plantaris disseminata: an unusual clinical presentation.

A 54-year-old man and his son had porokeratoses of the trunk and extremities, predominantly on the palms and soles. Numerous small, slightly depressed, opalescent rings with hyperemic borders were present on the oral mucosa. The spectrum of porokeratosis palmaris et plantaris disseminata and the differential diagnosis of other types of porokeratosis are discussed.

Immunological parameters in Darier's disease.

Patients with Darier's disease have frequently been observed to develop severe bacterial and viral infections. Previous studies have indicated some derangement in the immune system, even though no consistent or specific abnormality has yet been demonstrated. We performed the results of immunologic studies in 10 patients with Darier's disease. In each patient humoral immunity and cell-mediated immunity (in vivo and in vitro) were evaluated and leukocyte chemotactic function was assayed. Humoral immunity was normal; as regards clusters of differentiation (CD), a slight increase in CD4 lymphocyte subpopulations was observed in 7 patients and of CD4/CD8 ratio in 1 case. The lymphocyte responses to phytohemagglutinin and concanavalin A were normal, whereas the response to pokeweed mitogen was decreased slightly, but not significantly. Normal random and chemotactic mobility was found in all the patients except one. The data obtained do not show important immunological alterations. Our patients did not have a high propensity to develop severe infections and this may explain the absence of significant immunological derangement. Our data confirm that no immunological derangement is associated with Darier's disease.

[The multitest in evaluating cellular immunity].

It has been recently shown that the skin has a really active role in the immune system. The evaluation of delayed cutaneous hypersensitivity is an important step for the screening of the cell-mediated immune response. The multitest system contains seven antigens and 1 control put together in the same instrument. The results obtained with multitest are more reliable than those with monoantigenic systems. The Authors studied 138 patients affected by diseases with presumable cell-mediated immune deficiency. The comparison between clinical suspicion of immune deficiency, laboratory tests, and multitest showed statistically the usefulness and reliability of this test as a part of the screening for cellular immune deficiency.

[Congenital transient mechanobullous dermatosis (Bart's syndrome). A clinical case and review of the literature].

Bart's syndrome or congenital transient mechano-bullous dermatosis is one of the lesser known presentations of epidermolysis bullosa. It is characterized by congenital skin defects and by a tendency for blistering of the skin and sometimes of mucous membranes and sometimes associated with nail deformities, all of which remitted within in a few years. A neonate is described and compared with similar patterns from the literature.

[Comparison of the use of classic diazoanalgesia in surgery and an applicative variant with buprenorphine].

The authors have compared two different techniques of anaesthesia: the classical anaesthesia with benzodiazepine-analgesic (diazepam and fentanyl) and one of its variant obtained by using flunitrazepam and buprenorphine. The 45 patients submitted to gynaecological surgery were causally divided in two groups in connection with the two different anaesthesiological techniques. The clinical and statistical analysis of the collected data demonstrated the real validity of this anaesthesiological variant: either for premedication by perfect anxiolysis and sedation, or for the maintenance by low incidence on cardiocirculation system and perfect analgesia that extended its effects for many hours after surgical operation.
