[Temporomandibular joint dysfunction and neurofibromatosis. Apropos of a case].

The authors report one case of Von Recklinghausen's disease localized to the temporomandibular joint capsule and unilaterally associated with other cephalic manifestations. Although no other study, notably a CAT-scan, showed this tumor localization, surgical exploration was imperative and led to exeresis of the lesion which proved to be the treatment of choice.

[Bone involvement in histiocytosis X in adults. 11 cases].

The authors report 11 cases of bone lesions associated with histiocytosis X in adults, with a total of 25 bone sites: 22 were detected on X-rays and 3 were detected on scintigraphy when the corresponding X-rays were normal. They stress the high incidence of cranio-facial involvement, predominantly in the maxillae, with premature localised loosening of the teeth. They recommend X-rays in the staging of histiocytosis X and reserve scintigraphy for the cases with normal or doubtful X-rays. The bone lesions do not affect the prognosis of histiocytosis X, which depends on the associated visceral lesions. It generally has a favourable outcome and requires careful treatment: limited to a localised procedure for the purposes of histological diagnosis or to control the local underlying visceral risk or the functional complications. Bone lesions do not usually participate in the decision to treat histiocytosis X by chemotherapy.

[A new physiological scale for assessing posttraumatic disability in stomatology].

Establishing post-traumatic disability according to the present scale raises problems in relation to patients with 100% disability who cannot, under existing conditions, be compensated for further injuries. A complete reorganization of this system is proposed in which total 100% invalidity corresponds to death of the patient. For patients with some, even limited, capacities, the post-traumatic physiologic disability + remaining capacity would be equal to 100%. Proposed scales are reviewed and classified into 5 groups.

[Value of immediate prosthetic technics in hemiresections of the face].

A brief historical survey is completed by the presentation of a technique for immediate prosthetic rehabilitation during facial hemiresections for malignant tumors. The method presents multiple advantages when compared to typical drainage procedures from the physiological, mechanical and psychological points of view. A detailed description of the introduction of this prosthesis is followed by a brief summary of postoperative sequelae and the advantages the method supplies due to its extreme simplicity in application.

[External fixation devices in maxillofacial surgery].

External fixation devices have relatively little application in stomatology, but they are indispensable in certain situations. Equipment presently available for use by maxillofacial surgeons is relatively reduced in number, and two new fixation devices are proposed : a Jacquet type miniaturized fixation device and one of the Sorem type particularly adapted for the mandible.

[Reconstruction of the pavilion of the ear after burns].

Severe sequelae from facial burns, usually morphologic in type, include lesions affecting the pinna of the ear. Reconstructive surgery encounters very variable difficulties mainly related to the site, extent and effect on adjacent tissues of the lesion. Surgical treatment by partial otopoiesis often gives satisfactory results, but this is rarely the case after total or subtotal otopoiesis. In these cases, in fact, the often long surgical process which is a frequent cause of breaking down of adjacent tissue, when this can be used, rarely gives satisfactory results. Facial plastic prosthesis introduction is therefore of major interest, once the difficult problem of fixation of the prosthesis of the pinna has been resolved. All available therapeutic means should be employed to rehabilitate patients with disfigurement due to burns, while informing them of the difficulties and limits of therapy.

[Surgical treatment of acquired massive loss of substance of the roof of the mouth].

Surgical treatment of post-traumatic major loss of substance of the palate is reviewed, injuries in the series studied resulting from projectiles. The extensive loss of substance excludes the use of a mucous reservoir and skin grafts have to be employed. Free micro-anastomotic flaps represent a marked progress, as rapid results can be obtained, but these are not always totally effective. Pedicular flaps may have to be used, either of regional origin (cheek, neck, forehead, pectoral region) or from other zones (abdomen, internal surface of the arm). This allows effective, durable obturation of the palatine breach, and the application of the mobilized tissue to associated lesions that, because of the etiology, are almost always present.
