Restoration of lymph drainage after extremity replantation.

A clinical analysis of 171 major extremity replantations was carried out. From one month to 2.5 years after surgery, direct lymphographic studies of replanted limbs were done in 17 patients (16 upper limbs and one lower limb). Despite a complete disruption of lymphatic drainage, replanted limbs underwent adaptive and compensatory structural changes of the lymphatic system in the post-replantation period; first, with regeneration of cutaneous vessels and then (after six to eight weeks postoperatively) with regeneration of lymphatic collecting vessels. Such changes led to improvement as early as four to six weeks postoperatively and subsequently to restoration of lymphatic drainage in the replanted limb. The rate and extent of lymphatic recovery depends largely on the surgical techniques used in replantation.

[Changes in the lymphatic vessels of the upper extremity following a radical mastectomy (based on lymphographic data)].

The study group included 171 patients suffering from postmastectomy syndrome which had developed either after combined treatment for breast cancer or after radical mastectomy alone. Lymphographic examination of 146 cases carried out long after treatment for primary treatment revealed frequent roentgenologic features characteristic of all patterns of secondary obstructive lymphatic edema. In 20 patients, lymphographically-detectable changes were followed at early stages (until day 40 after radical mastectomy) before edema became clinically apparent. Radiation therapy and erysipelatous inflammation were shown to have an effect on lymphatic lesions.

[Correction of lymph flow disturbance following radical mastectomy].

Results of operations for the lympho-venous anastomosis in 50 patients were analyzed. The operation was shown to correct the lymph flow in patients with secondary lymphatic edemas of upper extremities. Good results were obtained in 15 patients, satisfactory results in 28 patients. More careful selection of the patients, improvement of the operation technique are the ways to get better results of the operation.

[Computerized tomographic arteriography].

It is not always possible to verify with a sufficient degree of confidence malignant tumors of the abdominal cavity using computerized tomography (CT) and angiography. The paper is concerned with working out and specifying a method of CT angiotomography (CTA), based on ia administration (via the celiac trunk, the renal arteries) of a contrast medium to increase the informative value of CT imaging, followed by scanning. A conclusion has been made after 27 investigations that CTA is capable of providing more complete information on the nature of an abnormal process than CT (even with i.v. administrations) or angiography.

[Computed tomography in the diagnosis of intervertebral disk hernias].

Computed tomography (CT) was conducted in 75 patients with discogenic radiculitis. In 15 patients the study was carried out without intensified contrasting of the spinal cord meningeal spaces (native CT), in 60 with contrasting of the meningeal spaces with air (42 patients) or Omnipaque (18 patients). The CT was analysed at the level of the bony and articular segments of the spinal canal. Deformity of the dural sac of the spinal cord was the most frequent sign of Schmorl's body. It was encountered in 96.2% of verified cases. CT is one of the methods for establishing the diagnosis of Schmorl's body.
