Antibiotic strategy in orofacial/head and neck infections in severe neutropenia.

A strategy for empiric antibiotic therapy for orofacial/head and neck bacterial infections that appear as clinical swellings in patients with severe neutropenia is assessed. Daily examinations were made in the hospital. Only those with peripheral blood neutrophil counts < 100/mm3 that persisted for at least 5 days after the commencement of resolution of the swelling were included in this article. The strategy consisted of sequential additions of a beta-lactam/aminoglycoside combination, metronidazole to intensify anaerobe cover and a beta-lactamase stable agent (such as vancomycin and floxacillin) as dictated by clinical signs. Progressive and complete resolution of infection occurred in 26 of 27 patients treated.

Zoster sine herpete of the trigeminal nerve.

Zoster sine herpete infection that involves the trigeminal nerve is rarely reported. The present case details a case of varicella zoster virus infection of the mandibular division of the left trigeminal nerve without evidence of a vesicular eruption. The diagnosis was established on clinical findings and was supported by the demonstration of an IgG antibody titer of > 1:640 during the acute phase of the disease.

Perineural spread of squamous cell carcinoma involving trigeminal and facial nerves.

Perineural spread of squamous cell carcinomas in the orofacial region predominantly involves the trigeminal and facial nerves. Central spread, particularly along the trigeminal nerve, produces sensory impairment and involvement of other divisions through invasion of the gasserian ganglion, as illustrated in these case reports. Early diagnosis and medical referral is important because central invasion has a poor prognosis.

Trigeminal neuralgia.

Severe trigeminal neuralgia (TN) is probably the most excruciating pain experienced in the orofacial region. Apart from the primary idiopathic form (ITN), this lancinating pain may be secondary to a range of underlying organic diseases (STN). Diagnosis requires differentiation of these and elimination of other causes of orofacial pain.

Oral complications in type 1B glycogen storage disease.

A case of type 1b glycogen storage disease (GSD) in which neutropenia and neutrophil dysfunction were prominent features is described. Oral complications included rapidly progressive periodontal disease and recurrent mucosal ulceration.

Herpes zoster virus infection: a clinicopathologic review and case reports.

Herpes zoster virus (HZV) infection, particularly of the trigeminal nerve, can be a disabling and disfiguring condition with variable clinical presentations. Acyclovir is a highly effective treatment modality during the acute clinical phase; however, pain control may be very difficult particularly with protracted and severe post herpetic neuralgia (PHN). The clinicopathologic features are reviewed and two cases in immunosuppressed patients with HZV infection of different divisions of the trigeminal nerve are presented.

The teaching of dento-maxillo-facial radiology in Australian dental schools.

The skills necessary to successfully perform radiological procedures in general dental practice are described and should be used as criteria when developing an effective undergraduate curriculum and as determinants of the contents of continuing education programmes. The present status of undergraduate curricula. continuing education, and postgraduate programmes as well as the recognition of the specialized nature of dento-maxillo-facial radiology, are discussed and recommendations for future developments are made.

Facial cellulitis responsive to flucloxacillin in acute leukemia.

A severely neutropenic patient, undergoing bone marrow transplantation for acute leukemia, experienced bilateral facial cellulitis accompanied by high fever and a facial folliculitis. All resolved with the addition of flucloxacillin to existing antibiotics suggesting a causal role for staphylococci.

Metronidazole in the management of anaerobic neck infection in acute leukemia.

A case involving a rapidly progressive neck infection in a severely neutropenic patient with acute leukemia is reported. Resolution followed the addition of metronidazole to an existing standard piperacillin/gentamicin combination, which was used primarily to cover potential gram-negative bacillary pathogens. This indicated the importance of considering extension of anaerobic cover in such infections.

Chronic indolent orofacial herpes simplex virus infection in chronic leukemia: a report of three cases.

Orofacial mucocutaneous infections caused by herpes simplex virus (HSV) may exhibit a distinct chronic indolent pattern of behavior in some immunosuppressed patients as opposed to the more familiar aggressive patterns. Three patients with chronic leukemia who illustrate this chronic indolent pattern are presented. These cases should alert the clinician to the variable clinical appearance that HSV may adopt in the immunosuppressed patient.
