[Clinical effect of recombinant human G-CSF on neutropenia induced by chemotherapy for non-Hodgkin's lymphoma].

A clinical trial of recombinant human granulocyte colony-stimulating factor (rG-CSF), produced by Chinese hamster ovary cells, was conducted in 66 patients receiving intensive chemotherapy for non-Hodgkin's lymphoma. Each patient received 2 cycles of CHOP therapy, and one cycle of them was performed with rG-CSF treatment and another one without rG-CSF treatment, in a cross-over fashion. rG-CSF (0.4, 2, 5, 10 micrograms/kg/day) was given intravenously or subcutaneously to each patient for 14 days from 2 days after initiation of the chemotherapy. rG-CSF increased the absolute neutrophil counts (ANC) at nadir, and reduced the period of neutropenia with ANC less than 1,000/mm3 and also the period for restoration to ANC greater than or equal to 2,000/mm3 after initiation of chemotherapy. These effects were remarkable at doses of more than 5 micrograms/kg/day intravenously and 2 micrograms/kg/day subcutaneously. Fourteen infective episodes were observed during the cycles of chemotherapy without rG-CSF treatment, while 7 infective episodes were observed during the cycles with rG-CSF treatment. rG-CSF was well tolerated. These results demonstrated that rG-CSF was effective in neutropenia induced by cancer chemotherapy at a intravenous dose of 5 micrograms/kg/day and a subcutaneous does of 2 micrograms/kg/day.

Changes in clinico-pathological features of Hodgkin's disease and follicular lymphoma in Nagoya, Japan, over a 21-year period (1965-85).

To study changes in the clinico-pathological features of malignant lymphomas in Japan, the histopathological and clinical features of 71 cases of Hodgkin's disease and 49 cases of nodal follicular lymphoma occurring in Nagoya were compared in three different periods (1965-71, 1972-78 and 1979-85). The proportional frequency of Hodgkin's disease among total lymphomas (10%) and that of follicular lymphoma among non-Hodgkin's nodal lymphomas (18%) did not vary over the study period. As to the histological patterns of Hodgkin's disease, the relative frequency of the nodular sclerosis (NS) type, which is relatively common in young women, increased, but that of other types decreased in the more recent periods. For follicular lymphoma, the relative frequency of the completely follicular type, which is more common in the United States than in Japan, increased. The five-year survival rate in patients with follicular lymphoma improved more recently even after adjusting for the effects of other prognostic factors. This recent increase in survival rates may be due partly to the recent improvement in cancer treatment, but is also related to the recent increase in patients with better prognostic features, i.e. the NS type of Hodgkin's disease. These variations over time suggest that the patterns of malignant lymphomas in Japan might be gradually changing, probably because of westernization, as seen in cancers of other organs.

[Smoldering leukemia with pyoderma gangrenosum].

A 76-year-old male admitted to Surugadai Nihon University hospital complaining of general fatigue, slight fever and anorexia. The laboratory examination revealed anemia and an appearance of a few myeloblasts and 7% of monocytes in the peripheral blood. The nucleated cell count was 2 x 10(4)/microliters with 43% myeloblasts in the bone marrow aspirate. He was diagnosed as acute myelomonocytic leukemia. He did not receive any chemotherapy for leukemia because of his old age and smoldering disease. Pyoderma gangrenosum developed in the left submandibular and axillary regions about 6 months later. Three more month later, significant increase of myeloblast was recognized in the peripheral blood and the bone marrow. It has been reported that pyoderma gangrenosum precedes a remarkable increase of leukemic cells in the patients with acute leukemia in complete remission and with myelodysplastic syndrome. In our case, to, the same process was strongly suggested.

[Characteristics of B-cell lymphoma in Japan].

Consecutive 431 patients with B-cell lymphoma seen in Aichi Cancer Center Hospital from 1964 to 1988 were analysed. Median age was 56 yr (range 8-86). There were 76 patients (17.6%) with follicular lymphoma and 355 with diffuse lymphoma. Among the 30 patients with follicular lymphoma, 10 (33%) were found to have a bcl-2 rearrangement. The incidence was lower than those reported in the United States. This might contribute to the lower incidence of follicular lymphoma cases in Japan. There were 168 patients (39.0%) with nodal lymphoma and 263 with extranodal lymphoma. Eight-four cases arose from the stomach, 84 from Waldeyer's ring, 21 small intestine and 13 thyroid. The distributions of extranodal B-cell lymphoma differed from those of T-cell lymphoma. Patients with gastric lymphomas in stage I were treated with resection alone. In the same manner, patients with Waldeyer's ring lymphoma in stage I were treated with radiotherapy alone. More than 90% of these patients were expected to survive without relapse. On the other hand, only 55% of patients with nodal lymphoma in stage I were expected to survive for more than 10 years. These findings suggest the site of localized lymphoma is an important determinant of outcome of clinical behavior.

[Chemotherapy of stage III-IV Hodgkin's disease. A retrospective analysis of the 53 cases collected from 9 institutions in Japan].

The clinical data of 53 patients with Stage III-IV Hodgkin's disease collected from 9 institutions in Japan were analyzed for the efficacy of chemotherapy. CR rate (78%) and five-year relapse-free survival (RFS) rate (61%) were higher in the patients treated with VEPA/CHOP regimen than those in patients with VEMP/BONP or MOPP/C-MOPP regimen, although the difference was not statistically significant because of the small number of the patients. As to 14 patients treated with CHOP regimen, CR rate was 87% and RFS curve trend toward plateau at 67% after 2 years and 3 months from the initiation of chemotherapy. Salvage therapy with adriamycin-based combination chemotherapy achieved CRs in 9 of 14 (64%) patients who had been treated with VEMP, BONP, modified MOPP or C-MOPP regimen. The CHOP regimen was effective in the treatment of III-IV Hodgkin's disease but an alternative multidrug chemotherapy with ADM, CPM, VCR, BLM, etoposide and procarbazine is recommended for achieving a higher CR rate and a better RFS. Prospective study is needed to establish the standard chemotherapy for Hodgkin's disease in Japan.

Hematological response to the new synthetic muramyl dipeptide derivative muroctasin after chemotherapy in patients with malignant lymphoma. A randomized crossover trial.

A randomized crossover trial was performed in 20 patients receiving chemotherapy for malignant lymphoma. N2-[(N-acetylmuramoyl)-L-alanyl-D-isoglutaminyl]-N6-stearoyl-L-lysine (MDP-Lys(L18), muroctasin) at a dose of 200 micrograms was subcutaneously administered after one of two cycles of the same protocol. The administration was started on day 4 after the start of chemotherapy and continued for 10 days. The mean length from the start of chemotherapy to nadir of white blood cell (WBC) was not significantly different between the control and muroctasin cycles. The mean WBC and neutrophil counts at nadir of the control cycle were significantly lower than those at the same point of muroctasin cycle, respectively. A positive effect of muroctasin cycle, defined as WBC count at nadir being increased by 1,000/mm3, WBC count being increased by 1,000/mm3 on two points examined after nadir, or faster reach to nadir and faster recovery to normal range of WBC than in the control cycle, was observed in 7 (35%) of 20 patients. On the other hand, only one (5%) patient showed a superiority of the control cycle over the muroctasin cycle. Toxic effects of muroctasin were observed in 34.8% of patients, but were tolerable for most patients. These results show that muroctasin has a clinical efficacy in the restoration of leukopenia after chemotherapy.

[Prognostic factors of malignant lymphomas].

As prognostic factors of malignant lymphomas, many factors such as histopathology, clinical stage, surface phenotype, primary site, and serum LDH are listed. But when we analyse the data according to the period accumulated for a long time, big differences in survivals are always observed. This fact means that the therapeutic factors become a very important prognostic factors, and the importance of other prognostic factors are influenced by the therapeutic factors. The main subjects in the treatment of lymphomas are the improvement of therapeutic results and the reduction of the intensity of treatment of the patients in whom the cure are expected in high probability. In these situation, statistical analysis of the prognostic factors include therapeutic factor will contribute strongly to the improvement of treatment results.

B-cell lymphoma of probable thymic origin: case report.

A case of B-cell lymphoma of probable thymic origin is reported. A 34-year-old woman was found to have an anterior mediastinal tumor in November 1986. The surface lymph nodes were not palpable. A total resection of the tumor mass was performed. The tumor invaded the right pleura, the right lung and the pericardium. Histologically, normal thymus was found at the margin of the tumor tissue. The neoplasm was predominantly composed of large lymphoid cells, separated by rather thick fibrous bands of nodular fashion in some areas. Immunohistochemical staining demonstrated monoclonal cytoplasmic IgG and kappa chains in a small portion of the neoplastic cells in fixed tissue. The cells showed positive staining with cluster of differentiation (CD) 20 (B1) but negative staining with antibodies reactive with T-cells in unfixed tissue. "Malignant lymphoma, diffuse, large cell type (B)" was the diagnosis. The arrangements of immunoglobulin (Ig) and T cell receptor (TCR) beta genes were studied. Clonal rearrangement bands of IgH and Ig kappa genes were observed in the same sizes in both the tumor and the peripheral blood before chemotherapy. The patient received chemotherapy until September 1987, and is in complete remission at present (January 1988). The peripheral blood showed germ line patterns of IgH and Ig kappa genes in complete remission. No rearrangement bands of TCR beta genes were detected throughout. The B-cell lineage was proved both from gene arrangement analysis and with immunohistochemistry.

[Phase II study of carboplatin in malignant lymphoma].

A phase II study of carboplatin for malignant lymphoma was conducted. Thirty-nine patients with malignant lymphoma were entered into this study, and 6 of them were excluded from evaluation due to violation of selection criteria. The remaining 33 cases were studied to assess the effectiveness and safety of this drug. Two CR and 11 PR were obtained with this medication (effective rate 39%). The significant affecting factors were previous treatment, disease stage and P.S. score. Many cases suffered from gastrointestinal symptoms and reduced hematopoiesis. The former complication was self-limited without any fatal episodes. However, the latter included severe cases, especially thrombocytopenia, which is suggested to be a dose limiting factor (DLF) of this drug.

[Chemotherapy of malignant lymphomas].

According to recent advances in chemotherapy containing adriamycin, now complete remission can be obtained in more than 80 percent of patients with stage 3 and 4 diffuse large cell lymphomas, and the cure rate could also be expected in more than 60 percent of patients. The easiest way to obtain better results would be the introduction of chemotherapy in stage 1 and 2. The kinds of chemotherapy chosen in planning the design of the treatment should be such as to eradicate microdisseminated disease, and only such chemotherapy will contribute to improvement in survival rates.

[Pathological features and treatment of T lymphoma].

T lymphomas are classified into two types from surface phenotype, thymic T (T1) and Peripheral T (T2) lymphoma, and T2 lymphomas, are furthermore subdivided to inducer/helper (Ti/h or T4) and cytotoxic/suppressor (Tc/s or T8) type. But even in ATLL (adult T cell leukemia/lymphoma) believed to be a model of Ti/h type, the heterogeneities of surface phenotype are reported frequently. All of the pleomorphic and lymphoblastic and a part of small cell, medium-sized cell, mixed and large cell types have a T-cell phenotype. In addition to these histologies, AILD/IBL/IBL-like T lymphoma, T-zone lymphoma and So-called Lennert's lymphoma also hold T-cell nature. The heterogenecity and the existence of the borderline area between neoplasm and reactive lymphadenitis or hyperplasia often confused the clinicians and hematopathologists. From our experience, forty nine percent 5 year survival of T lymphoma other than lymphoblastic and pleomorphic type suggests the propriety of classifying T lymphoma into high grade and intermediate grade malignancies. Treatment of T lymphomas are basically distinguishable to two categories, for T1 and T2 lymphomas. The treatment of T1 lymphoma should be an aggressive one including CNS prophylaxis. At present, the recommendable treatment of T2 lymphoma is to apply the best regimens obtained from B cell lymphoma study. As aggressive treatment for ATL does not always yield favorable results, the concept of therapy will become clear near future. An intensive supportive therapy including T-S prophylaxis should be combined actively in the treatment program.
