Skin and bone lesions (dermato-osteolathyrism), possible side effects of D-penicillamine treatment, in a boy with cystinuria.

A 2 1/4 year-old boy was treated for cystinuria and urolithiasis with high fluid intake, sodium bicarbonate, and D-penicillamine, over a period of 5 3/4 years, unauthorized interruptions and prescribed pauses included. Therapy was partially sucessful but regrowth of calculi coincided with interruptions of D-penicillamine administration and also with the institution of a low-dose D-penicillamine regime. Flat feet, scoliosis, pectus carinatum, hypermobility of joints, molluscoid pseudotumors and atrophic scars were alarming side effects of D-penicillamine. However, the possibility was not excluded that a forme fruste of an Ehlers-Danlos syndrome preexisted in this boy and was effected by D-penicillamine. Only the molluscoid pseudotumors regressed when D-penicillamine was reduced or omitted temporarily. Osteolathyrism caused by D-penicillamine has hitherto not been reported in man.

[Sudden death caused by massive air embolism during respirator therapy in the newborn].

A premature male infant with hyaline membrane disease succumbed suddenly during continuous positive pressure respirator therapy. Autopsy revealed a very rare complication to be the cause of sudden death: air embolization to pulmonary veins, right ventricle and superior and inferior venae cavae as well as interstitial and subpleural emphysema, pneumothorax, pneumomediastinum and pneumopericardium. The important role of lymphatic vessels as a port of entry for air is stressed.

Nutritive value of N-acetyl-L-tryptophan in man.

The nutritive value of N-acetyl-L-tryptophan was investigated by loading tests with deuterated L-tryptophan and deuterated N-acetyl-L-tryptophan in three healthy adults, and by a 7 day-long feeding experiment with deuterated N-acetyl-L-tryptophan in one healthy adult. After loading with deuterated N-acetyl-L-tryptophan, serum levels of deuterated tryptophan and kynurenine were only about 10%, and the amounts of deuterated 5-hydroxyindole-3-acetic acid excreted in 24 h approximately 30% of those found after deuterated tryptophan loading. During the feeding experiment with deuterated N-acetyl-L-tryptophan, the serum level of deuterated tryptophan remained below 20% of total serum tryptophan, and serum protein fell below the normal range. It is concluded that the nutritive value of N-acetyl-L-tryptophan is inferior to that of L-tryptophan. Contradictory findings of earlier studies are discussed.

Nonketotic hyperglycinemia treated with strychnine, a glycine receptor antagonist.

A 6 1/2-month-old boy suffering from nonketotic hyperglycinemia has been treated for at least 12 months with oral strychnine. The drug caused improvement of muscle tone, motoricity, vigilance and social behavior. Glycine is one of the putative neurotransmitters of postsynaptic inhibition, particularly at the spinal and reticular levels, strychnine its antagonist at the postsynaptic membrane. Strychnine improved the patient's motoricity presumably by blocking the excessive glycine-mediated inhibition of motoneurons. The beneficial effect of strychnine on vigilance and social behavior is more difficult to explain but may have been related to its antagonism of glycine inhibition of brainstem reticular neurons.

The use of a crystalline amino acid mixture for parenteral nutrition in low-birth-weight infants.

Thirty-six preterm, sick, low-birth-weight neonates were given either total or partial parenteral nutrition. The patients were divided into three groups according to their birth weights: group A -less than 1,000 gm, 12 patients: group B--between 1,000 and 1,500 gm, 15 patients: group C--more than 1,500 gm, 9 patients. The solution for total parenteral nutrition contained 20% glucose and 2.6% crystalline amino acids plus appropriate amounts of vitamins and minerals. The volume of infusate given was usually 125 ml/kg/day, but varied depending on the clinical condition of the patient; occasionally it was as high as 150 to 175 ml/kg/day. Infusate of one-half strength was administered initially; its concentrations of glucose and amino acids were increased to three quarters and full strength gradually, if tolerated. The solution for total parenteral nutrition was infused into the superior vena cava via a central venous catheter; that for partial parenteral nutrition was given into a peripheral vein to supplement inadequate oral feedings. The period of parenteral nutrition lasted from 5 to 49 days, with an average of 13.2 days. The intake of 500 mg of nitrogen as crystalline amino acids and 100 kcal as glucose was capable of achieving body weight gain. Positive nitrogen balance of various degrees was also observed. Hyperglycemia of a slight to moderate degree was observed in nine patients; only three required insulin therapy. Two patients had thrombotic occlusion of the central venous catheter. The conclusion was reached that total parenteral nutrition or partial parenteral nutrition, when properly managed, is a safe procedure in small, premature infants. The amino acid solution given as a nitrogen source along with adequate calories was effective in promoting weight gain and nitrogen balance; it was apparently well tolerated by low-birth-weight neonates.

[Thyroid gland ultrasound and urinary iodine excretion in children and adolescents with type I diabetes mellitus].

BACKGROUND: Date, studies on thyroid volume and urinary iodide excretion in patients with diabetes mellitus are not available. Sonographically determined parameters of the thyroid size are correlated to other anthropometrous data and the urinary iodide excretion is correlated to glucosuria, the HbA1c value and the diabetes duration. METHOD: In this prospective study we evaluated sonographically the thyroid volume in 107 patients with type I diabetes mellitus and 112 healthy children. The urinary iodide excretion was measured photometrically by using a modified ceric ion arsenious acid method for spontaneous urinary specimen and if available for the 24 h collected urin. RESULTS: The thyroid volume depended on site and age. A positive correlation of the thyroid volume and age, body weight and height, could be demonstrated. Referring to reference data a goitre prevalence of 30% in juvenile patients with diabetes mellitus type I was detected. Interestingly, juvenile type I diabetics presented with an average urinary iodide excretion of 183.0 micrograms iodide/g creatinine. Even the urinary iodide excretion of 162.5 micrograms iodide confirmed this increased level. The urinary iodide excretion in 24 hours correlated with glucosuria and the HbA1c level. The healthy children presented with an average urinary iodide excretion of 42.6 micrograms iodide/g creatinine. The mean value was clearly below the WHO recommendation of 150-300 micrograms iodide/g creatinine. Only 2.8% of the healthy children examined exceeded the lower limit of this range. CONCLUSION: In addition to the existing distinct under supply of iodide we assume an increased urinary iodide excretion in context with the osmotic diuresis in juvenile diabetics. Contrary to current opinion, that these data are correlated to the daily intake of iodide, which was calculated from urinary excretion rate, this thesis could not be affirmed for juvenile diabetics. Therefore it seems reasonable to frequently control thyroid volume and thyroid function in children and adolescents with diabetes mellitus.

[The epizootiology and pathogenic significance of infections with Isospora suis in ten piglet production operations in Nordrhein-Westfalen].

From July 1993 to June 1994 2036 fecal samples from 196 litters from 10 pig farms were collected. Three or four piglets from each litter and the respective sows were examined 3 times at 2, 3 and 4 weeks after farrowing. I. suis was detected in all 10 farms. The rate of infection of the litters was 62.2%. 71.6% of all litters with diarrheic problems were I. suis positive. The percentage of I. suis in the feces of piglets was 17.4% at the age of 2 weeks, 41.3% at the age of 3 weeks and 39.1% at the age of 4 weeks. It was also shown that I. suis occurs on any farm with any type of management system and at any time of the year. Fecal samples of sows were always Isospora negative. It is inferred that I. suis is a more important causative agent of porcine neonatal diarrhoea than considered so far. Therefore, further studies on epizootiological and therapeutical aspects and sanitation measures are necessary.
