[Nontraumatic chronic subdural hematoma due to dural metastases of breast cancer. Case report].

A case of nontraumatic chronic subdural hematoma due to obstruction of dural vessels by tumor cells is presented and 25 reported cases are reviewed. A 39-year-old female was referred for headache, vomiting, disturbance of consciousness and right homonymous hemianopia with macular sparing. She had undergone mammectomy for medullary nodular carcinoma of the left breast five years before. She had been treated with combined hormonal therapy and chemotherapy for the cancer metastases to the liver in preceeding six months. Hematological examination revealed drug-induced thrombocytopenia, increase of FDP in blood (80 micrograms/ml), but no abnormality of prothrombin time and fibrinogen content. Therefore in the present case there was no evidence of disseminated intravascular coagulation (DIC) after Colman's criteria. However, it was suggested that this case had compensated DIC after Cooper's criteria. CT scan showed a biconvex-shaped low and partially iso-density area over the left fronto-temporal convexity, indicative of chronic subdural hematoma, and no abnormal findings in the occipital area. After removal of the hematoma she became alert without headache and vomiting. However, seven days later she complained of headache and vomiting again. Repeated CT scan showed a larger biconvex-shaped low density area over the left hemisphere extending to the parietal region at that time. Second operation was performed, but she expired four days later. Autopsy showed systemic metastases of the medullary nodular carcinoma in the scalp, temporal muscle and dura as well as lungs, adrenal glands, ovaries and bone marrow.(ABSTRACT TRUNCATED AT 250 WORDS)

Synergic effects of kallikrein-kinin and prostaglandins on renin release on infusion of isolated hog kidney with aldosterone.

The effects of infusion of a large amount of aldosterone into the renal artery of isolated perfused hog kidney on the release of renin, prostaglandins (PG) and kinin and the excretion of urinary kallikrein were investigated. Infusion of aldosterone at a rate of 100 ng/min (100 to 800 ng/ml of perfusate) resulted in significant releases of renin, PG (PGE2 , 6-0-PGF1 alpha), and kinin and increase in urinary kallikrein. Infusion of aldosterone and an inhibitor of kallikrein, aprotinin, decreased the releases of renin, PG and kinin and infusion of aldosterone with indomethacin decreased the release of PG but increased that of kinin and urinary kallikrein without significant change in renin releases. These findings suggest that the release of renin by aldosterone may result from synergic effects of renal PG and the kallikrein -kinin system.

Effect of 1-alkyl-2-acetyl-sn-glycero-3-phosphorylcholine inhibitor on the reduction of one-kidney, one clip hypertension after unclipping in the rat.

The role of 1-alkyl-2-acetyl-sn-glycero-3-phosphorylcholine in regulating blood pressure was studied in one-kidney, one clip hypertensive rats using 3-(N-n-octadecylcarbamoyloxy)-2-methoxypropyl-2-thiazolio ethylphosphate, which specifically inhibited the hypotensive activity of exogenously injected 1-alkyl-2-acetyl-sn-glycero-3-phosphorylcholine. The blood pressure of rats with established hypertension produced by clipping one renal artery and contralateral nephrectomy normally decreases rapidly after unclipping the artery, but this rapid decrease was significantly inhibited by intravenous infusion of 3-(N-n-octadecylcarbamoyloxy)-2-methoxypropyl-2-thiazolio ethylphosphate. This shows that endogenous 1-alkyl-2-acetyl-sn-glycero-3-phosphorylcholine participates in the rapid decrease of blood pressure after unclipping the kidney in one-kidney, one clip hypertensive rats.

The role of antihypertensive polar renomedullary lipid in the pathogenesis of hypertension.

One-0-hexadecyl-2-0-acetyl-sn-glycero-3-phosphorylcholine (HAGPC), a component of semisynthetic antihypertensive polar renomedullary lipid (APRL) reported by Muirhead et al. in chemically treated rabbit renomedullary lipid had a strong hypotensive action, a cardiosuppressing action and an apparent anti-norepinephrine action. 3-(N-n-octadecylcarbamoyloxy)-2-methoxypropyl-2-thiazolio ethyl phosphate significantly inhibited the hypotensive action of HAGPC, whereas the hypotensive activities of prostaglandin I2, prostaglandin E2, bradykinin, histamine and acetylcholine were not affected by this drug. The blood pressure of rats with established hypertension produced by clipping one renal artery and contralateral nephrectomy normally decreases rapidly after unclipping the renal artery, but the initial rapid decrease was significantly inhibited by an intravenous infusion of 3-(N-n-octadecylcarbamoyloxy)-2-methoxypropyl-2-thiazolio ethyl phosphate. This shows that endogenous 1-alkyl-2-acetyl-sn-glycero-3-phosphorylcholine participates in the rapid decrease of blood pressure after unclipping the renal artery in one-kidney, one clip hypertensive rats.

[Cerebellar hemangioblastoma developing after nephrectomy for renal cell carcinoma].

Cerebellar hemangioblastomas are known to be occasionally associated with abdominal tumors such as renal cyst and renal cell carcinoma. But most of the abdominal tumors reported are clinically silent and usually diagnosed after the hemangioblastomas have been found in the central nervous system. We report a rare case in which a renal cell carcinoma preceeded in its onset of symptoms 4 years to a cerebellar hemangioblastoma. A 56-year-old female underwent left nephrectomy because of a left renal cell carcinoma at the age of 49. About 4 years after the nephrectomy, she developed signs and symptoms of intracranial hypertension and was operated on for a cerebellar tumor which was histologically diagnosed as hemangioblastoma. Her family history was non-contributory. Four years later, recurrence of the cerebellar tumor was found on the CT scan and abdominal studies were negative except for a suspected parapelvic cyst in the right kidney on abdominal ultrasonography. Subtotal removal of the cerebellar hemangioblastoma was performed followed by irradiation.

Enhancement of lymphatic transport of 1-(2-tetrahydrofuryl)-5-fluorouracil by polyacrylic acid aqueous gel and emulsion type suppositories in rats.

1-(2-Tetrahydrofuryl)-5-fluorouracil (FT-207) concentrations in lymph and plasma were measured after rectal administration of three types of suppositories containing FT-207. The suppositories used in this study were prepared from polyacrylic acid aqueous gel bases, emulsion type bases and a commercial fat base, witepsol w-35. Both aqueous gel and emulsion type suppositories were superior to the suppositories from witepsol with regard to FT-207 concentration in lymph and plasma. The addition of aqueous gel to the polyacrylic acid aqueous gel was found to be effective for the lymph transport from the aqueous gels. Polyacrylic acid aqueous gel containing 1% glycyrrhizin was better than other suppositories. Solidification of the emulsion type bases are also investigated, and it is suggested that these may be better substitutes of the oily suppository.

alpha-Adrenergic blocking action of 1-O-hexadecyl-2-O-acetyl-sn-glycero-3-phosphocholine in rats.

The effect of 1-O-hexadecyl-2-O-acetyl-sn-glycero-3-phosphocholine (HAGPC), a major component of antihypertensive polar renomedullary lipid, on the pressor responses to norepinephrine and angiotensin II was investigated in normal Wistar rats. The pressor activity of norepinephrine (1 microgram/kg of body weight (BW)) and angiotensin II (40 ng/kg BW) were markedly suppressed when these substances were injected immediately after intravenous administration of 80 nmol/kg BW of HAGPC. When HAGPC was infused continuously at a rate of 20 nmol/kg BW/min, pressor responses to bolus injection of graded doses (0.5 to 10 micrograms/kg BW) of norepinephrine were significantly lowered, but pressor responses to 10 to 200 ng/kg BW of angiotensin II were not affected. These observations suggest that HAGPC has alpha-adrenergic blocking activity and does not influence angiotensin II receptors.
