Factors causing hyperkalemia in premature infants.

In order to clarify the causative factors of hyperkalemia in premature infants, 48 newborn infants less than 2000 gm birthweight were examined during the first 48 hours of life. They were divided into two groups: group A (n = 19), which had more than 7 mEq/liter of serum potassium level, was defined as a hyperkalemic group; group B (n = 29) had less than 6.9 mEq/liter of serum potassium level throughout this study period. In group A hyperkalemia occurred on the first day of life and continued throughout the study period. Five of group A infants had arrhythmia, one of whom died from cardiac arrest on the second day of life. Laboratory findings (serum creatinine, creatine phosphokinase, electrolytes, blood gas analysis, calorie balance, and renal function) were compared between the two groups. On the first day of life, serum creatinine and creatine phosphokinase concentrations were significantly elevated and calorie intake was decreased in group A when compared with group B. On the second day of life, calorie intake, urine volume, creatinine clearance, and base excess were significantly decreased in group A. A significant decrease was observed in the serum calcium level in group A on the second day of life when compared with admission data. These data suggest that neonatal hyperkalemia may occur in the sick newborn infants with renal dysfunction when combined with metabolic acidosis and catabolic state.

N-isopropyl-p-[I-123]iodoamphetamine lung imaging in a patient with chronic pulmonary thromboembolism.

A patient with a brain infarct was found to have chronic pulmonary embolism associated with perfusion defects on blood flow imaging with N-isopropyl-p-[I-123]iodoamphetamine (I-123 IMP). However, gradually increasing uptake of I-123 IMP was seen in later images, in areas distal to the embolus. The increased uptake to the embolized lung seemed to be through the systemic collateral circulation.

Relationship between plasma atrial natriuretic peptide concentrations and cardio-thoracic ratio during the early neonatal period.

Twenty-seven neonates were examined at 2.6 +/- 1.5 hours of age (stage 1) and 21 +/- 4.9 hours of age (stage 2) to clarify the relationship between cardio-thoracic ratio (CTR) on chest roentgenograms and plasma atrial natriuretic peptide (ANP) concentration. Among 22 infants who showed elevated plasma ANP, 14 had gained body weight (group A), while 8 other infants had lost weight (group B) at stage 2. The remaining 5 infants had decreased plasma ANP concentrations combined with weight loss at stage 2 (group C). All infants of group B had a patent ductus arteriosus. The plasma ANP concentration and CTR of groups A and B increased during the study period, while those of group C decreased. A linear relation was found between plasma ANP concentration and CTR in all cases (p less than 0.02). This study indicates that increased plasma levels of ANP are associated with an increased CTR.

Maturational changes and origin of urinary human epidermal growth factor in the neonatal period.

In order to clarify the characteristics of urinary human epidermal growth factor (hEGF) excretion in the newborn period, we examined hEGF in the 41 newborn infants; 12 healthy preterm infants (group A), 10 healthy full-term infants (group B), 12 full-term infants with neonatal asphyxia (group C), 7 full-term infants treated with tobramycin (group D) during the first week of life. Renal function tests, i.e. creatinine clearance (Ccr), and N-acetyl-beta-D-glucosaminidase (NAG) as a parameter of renal tubular damage, were examined in 29 full-term infants. Urinary hEGF excretion showed a linear increase with gestational age. During the first week of life, urinary hEGF excretion in group B increased with age. However, urinary hEGF excretion in group A remained at the constant level through the study period. Ccr in group C was significantly decreased through the study period when compared with that in group B. NAG indices in group C during the first week of life and those in group D on days 5-7 of life were more elevated than those in group B. Urinary hEGF excretion in group C on days 4-7 of life and that in group D on day 7 of life were significantly decreased when compared with that in group B. These results suggest that urinary hEGF excretion is related to the maturation and that the source of urinary hEGF may be renal tubular cells.

[Malignant lymphoma displaying rearrangements of both immunoglobulin and T-cell receptor genes].

A 65-year-old male was admitted to our hospital due to recurred malignant lymphoma of left tonsil origin. Studies using flow cytometry on mononuclear cells in peripheral blood revealed the appearance of intermediate B cells, and examinations on gastrointestinal tract showed diffuse infiltration of medium-sized lymphoid cells into stomach and colon, that had the same phenotype as tumor cells in peripheral blood. They suggested leukemic change and gastrointestinal tract infiltration of malignant lymphoma. Southern blot analysis revealed T-cell receptor beta-chain gene rearrangement as well as immunoglobulin gene rearrangement. Analysis by histo in situ hybridization on infiltrated gastric mucosa specimen showed diffuse expression of immunoglobulin mRNA in tumor cells, but no message of T-cell receptor beta-chain gene. Northern blot analysis showed the same result. It suggests that in this case, the rearrangement of T-cell receptor beta-chain gene is ineffective rearrangement without transcription to mRNA.

Synthesis and properties of oligodeoxyribonucleotides containing N6-methoxyadenine.

Oligodeoxyribonuclotides containing N6-methoxyadenine (mo6A), a promutagenic base, were synthesized. The Tm values and thermodynamic parameters for double strand formation of d(CCTGGTAmo6ACAGGTCC) have been measured. The order of the base-pair stability was A-T greater than mo6A-C approximately equal to mo6A-T greater than A-C.

[CD33-positive acute lymphoblastic leukemia with breast tumor and cavernous sinus tumor].

A 38-year-old woman was diagnosed as acute lymphoblastic leukemia (L2) in Oct. 1985. After VP and AdVEMP therapy, complete remission was obtained. In Oct. 1987, she noticed bilateral breast tumors and leukemic cell infiltrations were shown in a biopsy specimen of the breast tumor. Bone marrow was occupied with 94 percent blasts. The second complete remission was achieved by the AdVP therapy. In Nov. 1988, she developed double vision and photophobia. The examinations of CT and MRI showed cavernous sinus tumor, and 20 percent blasts were recognized in a bone marrow aspirate. The leukemic cells were negative for peroxidase, but were positive for both lymphoid and myeloid cell surface markers (CD2, CD5, CD7, CD33). The two color flowcytometry showed that CD5 and CD33 were simultaneously expressed on the leukemic cells.

Synthesis of decadeoxyribonucleotides containing 5-modified uracils and their interactions with restriction endonucleases Bgl II, Sau 3AI and Mbo I (nucleosides and nucleotides 82).

Decadeoxyribonucleotides containing uracil, 5-bromouracil, 5-cyanouracil and 5-ethyluracil in recognition sequences of restriction endonucleases Bgl II, Sau 3AI, Mbo I were synthesized. Decanucleotides containing 5-bromouracil in place of thymine had essentially the same susceptibility to all the restriction endonucleases. Uracil-containing decanucleotides were however very resistant to attack. Decanucleotides containing 5-cyanouracil in the recognition sequence were strongly resistant to hydrolysis by Sau 3AI, but were hydrolysed by Bgl II and Mbo I as well as the parent decanucleotide. Decanucleotides containing 5-ethyluracil were strongly resistant to hydrolysis by Sau 3AI, but were partially resistant to hydrolysis by Bgl II and Mbo I.

On the mechanism of DNA-adenine methylase.

Experiments were performed to determine whether EcoRI methylase catalyzes the transfer of the methyl group of S-adenosylmethionine (a) directly to the N6 of adenine in DNA or (b) initially to N1 to give N1-methyladenine followed by isomerization of the N1-methylamino and 6-NH2 to give N6-methyladenine (Dimroth rearrangement). A facile synthesis of highly enriched [6-15N]deoxyadenosine and a dodecamer substrate of EcoRI methylase with [6-15N]adenine in the methylation site are reported. In the product of EcoRI enzymatic methylation, all of the isotope remains at the N6 position of the N6-methyladenine product. It is concluded that, contrary to existing chemical precedent, the methylation occurs by direct transfer from S-adenosylmethionine to the N6 of adenine in DNA.

Transcatheter arterial chemo-embolization for humoral hypercalcemia of hepatocellular carcinoma.

A 50-year-old male with unresectable hepatocellular carcinoma (HCC) had a hypercalcemic crisis with a serum calcium concentration of 7.8 mEq/zeta, without any evidence for bone metastases or parathyroid lesions. The hypercalcemia was thought to be due to increased renal reabsorption of calcium and increased bone resorption, which was probably caused by humoral factors derived from the HCC, some being parathyroid hormone-like factors. Since conservative therapy for hypercalcemia was not sufficiently effective and was accompanied by progressive exacerbation of ascites and leg edema, transcatheter arterial chemo-embolization (TACE) was performed. On the following day, serum calcium concentration decreased from 6.3 mEq/zeta to the normal range, although serum alpha-fetoprotein levels decreased only slightly. Thereafter hypercalcemia did not develop for about 4 weeks. The results demonstrated that TACE can be effective for humoral hypercalcemia of HCC.

Minor-groove-modified oligonucleotides: synthesis of decadeoxynucleotides containing hypoxanthine, N2-methylguanine and 3-deazaadenine, and their interactions with restriction endonucleases Bgl II, Sau, 3AI, and Mbo I (Nucleosides and Nucleotides Part 75).

Decadeoxynucleotides containing hypoxanthine, N2-methylguanine, 3-deazaadenine in the recognition sequences of restriction endonucleases Bgl II, Sau 3AI, and Mbo I were synthesized. These decanucleotides modified in the base moieties facing in to the minor groove were strongly resistant to hydrolysis by Bgl II and partially resistant to that of Sau 3AI and Mbo I. The decadeoxynucleotide containing 3-deazaadenine in place of adenine was bound to Bgl II strongly, whereas the nucleotides containing hypoxanthine and N2-methylguanine were bound less tightly.
