Endometrial hormonal receptors as marker of benign breast disease (B.B.D.).

The problem of hormonal treatment of benign disease and mastodynia is still dividing Clinicians and Researchers. The Authors, in order to select the patients suitable to be treated, propose the assay of Estradiol and Progesterone Receptors in the most accessible hormone-dependent tissue: the Endometrium. Author's data point out a quite different behaviour of Estradiol and Progesterone cytoplasmic receptors in the Endometrium of women with B.B.D. and controls. The knowledge of this information in the single patients will release from subjectivity when deciding about B.B.D. therapy or when trying to correct unsatisfactory treatment.

Surgery as diagnostic and therapeutic moment in the management of cervical cancer.

Therapeutical program in cervical cancer usually include surgical or radiotherapeutical interventions or a combination of the two, variously integrated with techniques and sequences of application. Treatment planning and evaluation of results are normally based on the classification into clinical stages codified by the FIGO. But this classification, though providing useful prognostic standards of judgement, has proved to be inadequate to identify specific therapeutical guidance. Within a single clinical stage different risk factors have been identified which influence prognosis thus determining the therapeutical treatment more than staging itself. Furthermore clinical staging has failed to predict the surgical stage correctly in a significant number of cases. The index of correction after surgical staging shows discrepancy in about 1/3 of cases. In these cases a therapeutical program based on clinical examination exclusively may prove to be inadequate. The results reported by Authors warrant a rational resort to operative staging in cervical cancer in order to establish the real topography of the original focus more correctly. This indication is further strengthened by the need to identify lymphatic involvement, notably of para-aortic nodes, in locally advanced cases since such an occurrence bears a significant influence on the patient's prognosis and possibility of treatment. On the bases of these considerations the Authors suggest surgery as primary treatment in the management of cervical cancer.

Hormonal receptors in vulvar tissues.

Since 1982 we have been associating the study of hormonal receptors with the histological examination of the vulvar lesions in an attempt to interpret the action mechanism of steroids on the vulva, to find possible correlations between histological pictures and receptorial order and to find a possible predictive "marker" toward therapeutic and prognostic ends. In our Laboratory assays for androgen, estrogen and progesterone receptors were performed with the Dextran-Charcoal technique in 41 patients with vulvar dystrophy; 20 of whom had atrophic dystrophy and 21 with hypertrophic dystrophy. Another 24 patients with vulvar carcinoma were studied; 23 with epidermoid carcinoma of various differentiation grades and only one case of Paget's Disease. None of the three receptors assayed could be considered a predictive or prognostic marker in the approach to neoplastic forms. The levels of the progesterone receptor appears increased in the case of dystrophies. This justifies the hypothesis of a hormonal involvement in the pathogenesis of the lesion, as has already been documented by the beneficial effects of therapy with topical steroids.

Steroids receptors in physiological and pathological features of the cervix.

The presence of ER and PgR in human cervical tissues was investigated. The material consisted of receptor measurements in tissue sample obtained from normal and abnormal colposcopical pictures (leukoplakia, mosaic, punctation, atypical transformation), in 38 outpatients. The receptor assays were performed by a dextran-coated-charcoal (DCC) technique.

Repetitive debulking surgery as adjuvant to chemotherapy in advanced epithelial ovarian cancer.

The Authors refer their own experience on a planned prospective study of integrated chemosurgical treatment in advanced epithelial ovarian cancer. Repetitive surgery interspersed with non cross resistant schedules of chemotherapy can achieve more than 60% of complete responses with an expected five years survival rate dependent on the residual disease after primary and especially second operation. In particular residual neoplasia larger than five centimeters after the first laparotomy or larger than two centimeters left behind at second laparotomy constitutes the worst prognostic index.

Progestinic treatment of mastodynia. New suggestion for the follow-up.

The problem of progestin treatment for benign breast disease and mastodynia is still dividing Clinicians and Searchers: the traditional methods of clinical investigation and follow-up do not allow any further step in the debate. In this work we evaluated the role of E2/Progesterone R.I.A. and thermography in monitoring the effect of progestin on BBD and found that only the latter can give useful informations, insufficient, however, to direct the treatment. If we want to be able to select the patients suitable to be treated by progesterone, we must know the tissutal hormone receptivity and be able to reexamine it during the treatment to verify the effectiveness of the administered drug. These informations can be obtained through the assay of Estradiol and Progesterone Receptors in the most accessible hormone-dependent tissue: the endometrium. Our data point out a quite different behaviour of Estradiol and Progesterone cytoplasmic Receptors in the endometrium of women with mastodynia and controls. The knowledge of these informations in the single patient will let us come out from subjectivity when deciding about BBD therapy or when trying to correct unsatisfactory treatments.

Androgen, estrogen and progesterone receptors in vulvar dystrophy.

Assays have been developed for the quantitation of androgen, estrogen and progesterone receptors in the cytosol of 45 vulvar dystrophic tissues. In 27 patients we assayed only cytoplasmatic receptors, while in 18 patients both cytoplasmatic and nuclear receptors. The workers verified the possibility to assay these carriers, finding significant correlations neither with patient's age nor between atrophic and hypertrophic dystrophies. The research concerning the cytoplasmatic depletion and nuclear accumulation allowed us to understand the action mechanism of the androgen in vulvar tissues in patients in menopause.

Two problems in oral contraception: arteriosclerotic cardiopathy and drug interactions.

The Authors examine two aspects of oral contraception: the risk of arteriosclerotic cardiopathy and the interaction with other drugs. The former is rather limited but increases significantly when other arteriosclerotic risk factors, first and foremost smoke, are present. The dose of both the estrogenic and the progestinic components also influences the risk. The Authors examine the most recent information on the interference of some drugs with the contraceptive effectiveness of synthetic, especially low-dose, estroprogestinics, which is mediated by changes of the intestinal absorption or metabolic clearance rate. Finally, they analyse the interference of contraceptive steroids with the kinetics of other drugs and, consequently, with the intensity of therapeutical and side effects.

Introduction to chemotherapy and antineoplastic pharmacology in breast cancer.

Tumor cell biology and growth-kinetics of human breast cancer are reviewed with reference to the different action mechanisms of main antitumor drugs. Particular emphasis is given to pharmacokinetics, dosage, mode of administration, effectiveness and side-effects of drugs most commonly used in breast cancer. The importance of tumor Estrogen and Progesterone Receptor assay for the therapy planning is considered and a short review of estrogens, androgens, progestins, antiestrogens, corticoids and aminogluthetimide is presented. The rationale for combined chemotherapy and/or hormonotherapy in breast cancer is finally discussed.

Epidemiology of nipple discharge.

The Authors report their personal observations of nipple discharge in gynecologic patients admitted to the Obstetric and Gynecologic Department of Padua University or applying to the Senologic Diagnostic- or Endocrinology-Departments of the same. After dividing breast secretions into two groups - the neuroendocrine and the strictly mammary ones - the Authors discuss their etiopathogenesis, incidence, and clinical significance (mainly as to their possible association to breast malignancies), and present their personal results. At the end, a careful examination of all nipple discharges is recommended, which should include, besides cytological evaluation, a complete assessment of the neuroendocrine function and a watchful investigation of local breast conditions.

A planned prospective study of combined treatment in stage III epithelial ovarian cancer.

In a planned prospective study 39 consecutive patients with stage III ovarian cancer have been admitted in two years. All patients have received initial extensive surgery followed on as out patients basis by 5 courses of A-Cy chemotherapy and by a second operation and further non cross resistant chemotherapy with Platinum, Hexamethyl-Melamine, Polymelphalan; 24 have been found pathologically free of disease at second surgery, and 34/39 are clinically free of disease while reporting. These data, exactly collected and evaluated are consistent with our (previously reported) empyrical observation that adjuvant surgery interspersed with courses of aggressive chemotherapy may render free of disease the majority of patients, prolong survival, and possibly cure a portion of this population.
