[Collageno-vascular diseases and interstitial pulmonary fibrosis. An analysis of diagnostic tests].

Collagenovascular diseases, particularly rheumatoid arthritis, LES and scleroderma frequently involve the lung. A group of 15 patients with collagen vasculopathies was analysed with a view to identifying the most suitable invasive and noninvasive tests for the diagnosis of the interstitial fibrosis of the lung that often accompanies such disorders. Among the noninvasive tests only the respiratory function tests (especially DLCO) are adequately sensitive, while chest X-rays, gallium scintigraphy of the lung and ACE are relatively unreliable. Bronchoalveolar lavage is often altered (though the intensity varies considerably) and transbronchial biopsy appears to be the ideal diagnostic examination.

[Scintigraphic findings with a positive marker in generalized lymphadenopathy syndrome (L.A.S.)].

The behaviour of gallium scintigraphy in a group of 15 drug addicts suffering from LAS is examined. The examination was positive in 11 patients with no pleuro-pulmonary lesions that could be demonstrated by clinical or radiological examination. This is in line with similar observations in AIDS cases and may reflect the preactivation of monocytes and macrophages documented in HIV infection.

[Use of miocamycin in the acute exacerbation of chronic obstructive bronchopneumopathy. Case material contribution].

18 patients with chronic obstructive bronchitis were treated with 900 mg myomycin a day. The clinical parameters examined were the quantity and organoleptic characteristics of the expectoration, coughing, and body temperature. These parameters were studied immediately before and 7.14 and 21 days after the start of treatment. Myomycin was found to be satisfactorily effective with minimal toxicity and well tolerated.

[Radioscintigraphic correlation in interstitial pneumopathy. Case presentations].

Radiological and scintigraphic findings in 82 patients (58 male, 24 female) were compared. The patients were hospitalised in 1981-86 with sarcoidosis (43 cases), silicosis (26), asbestosis (9) extrinsic allergic alveolitis (3), lung disease caused by hard metals (1). Gallium scintigraphy was positive in 61 patients (74.39%). More precisely, the response was positive in 57 cases in both analogic and quantitative terms and in 4 only quantitatively. The radiological (radiography and stratigraphy) finding was significant in 42 of the 61 gallium-positive cases and negative in the remaining 19. In the 21 gallium-negative cases (25.61%) the radiological finding was significant in 9, negative in 12. The study confirms the real value of gallium scintigraphy in the diagnosis and follow-up of interstitial lung disease in line with views presented in the recent literature.

[Usefulness of the determination of CEA and Ca 19-9 in the diagnosis and staging of bronchial carcinoma].

The value of CEA and Ca 19-9 assays in the diagnosis and staging of lung cancer is examined. The particular sensitivity of CEA especially in extensive or metastasised tumours is demonstrated. The minimal sensitivity of Ca 19-9 is pointed out but also its absolute specificity in high risk control groups (smokers and BPCO). It is therefore concluded that Ca 19-9 might be used together with CEA in the diagnosis and staging of lung cancer.

[Clinico-pathological aspects of farmer's lung. Review of the literature and personal cases].

Data are presented on 9 subjects with Farmer's lung encountered in 1981-85. In all cases dyspnea arose within several hours after exposure to the allergen and in 4 cases only a few minutes after exposure. The radiological picture was altered in 7 cases (6 miliary pictures and 1 case of medial and basal nodulation). In functional terms 6 restrictive and 3 mixed forms were identified. The specific bronchial provocation test was positive wherever it was performed. Bronchoalveolar lavage revealed predominantly lymphocytic alveolitis in the most recent cases. Transbronchial biopsy also revealed the presence of pulmonary granulomatosis in various stages of development. Finally skin tests were positive to various antigens in several cases while specific precipitin were found in all cases. These data are compared with reports in the literature.

[CA-50: evaluation of a new tumor marker in the diagnosis of neoplastic pathologies of the gastrointestinal tract].

In order to assess the diagnostic value of CA-50, a new tumour marker, a series of 226 patients with gastroenteric tumours, 152 healthy controls and 175 patients with benign gastrointestinal pathologies, was examined. CEA, CA 19-9, Alphafetoprotein and CA-50 were assayed in all subjects and the results subjected to statistical analysis in order to discover the sensitivity, specificity and diagnostic value of the new marker. Studies conducted to date indicate that CA-50 assays may be of some value in the diagnosis of pancreatic and liver tumours but are no improvement on existing markers in clinical use, for the diagnosis of gastroenteric tumours. Indeed CA-50 gives a very large number of false positive especially in cirrhosis and to a lesser extent in chronic pancreatitis.

[Diagnostic contribution of carcinoembryonic antigen (CEA) in the pleural fluid and serum of patients with metastatic pleural effusion caused by neoplasm of the breast].

The problem of the diagnosis of pleural exudates in breast cancer is examined. It is pointed out that cytology is very little help and it is suggested that carcinoembryonic antigen could be used for assays of the serum and pleural fluid given its high sensitivity even when the cytological examination is negative.

[Pathological and clinical aspects of bronchial carcinoid tumor. The authors' own case reports].

Data (radiological, endoscopic, histological, clinical and surgical) are presented on 21 subjects with pulmonary carcinoid tumour encountered in the Turin University Chest Surgery Centre in 1980-1985. All patients were subjected to fibrobronchoscopy which revealed the tumour in 18 cases (14 were given multiple biopsies and in 4 cases samples could not be taken due to the patient's intolerance of the endoscopic manoeuvre). No appreciable bleeding occurred after any biopsy. Surgery was performed on all 21 patients (2 pneumonectomies, 7 bilobectomies, 12 lobectomies). There was 85.71% correspondence between pre and post-operative histological diagnosis.
