Studies on the prevalence of renal disease and hypertension in relation to schistosomiasis. III. Proteinuria, haematuria, pyuria and bacteriuria in the rural community of Nigeria.

Midstream specimen of urine was collected from the total population of two rural communities in the immediate environs of Ibadan. One has a higher and the other a low endemicity of urinary schistosomiasis. Proteinuria, haematuria, pyuria and bacteriuria were found to be significantly more frequent in the area of high than low endemicity.

Studies on the prevalence of renal disease and hypertension in relation to schistosomiasis. V. Radiological findings: plain X-ray abdomen and intravenous pyelogram.

After studying two rural communities with varing endemicity of urinary schistosomiasis by total cross-sectional population survey, a stratified sample was obtained using defined criteria. Plain X-ray abdomen and intravenous pyelogram were carried out on the sample. Findings on plain X-ray of the abdomen, using the usual parameters of psoas and kidney shadows in the Nigerian, indicate that the two communities studied are similar but urinary calculi and urinary tract distortion are significantly more prominent in the community with the higher endemicity of urinary schistosomiasis.

Surveillance of communicable diseases in tropical Africa.

In tropical Africa, parasitic and infective diseases remain the predominant causes of mortality and morbidity, and health and epidemiological services are poorly developed. Experience elsewhere has shown the value of surveillance in the control of communicable diseases and this paper contains a brief review of the use of surveillance in tropical African countries and considers the priorities, techniques and constraints of the major programmes.

Mycobacterium ulcerans (Buruli) skin ulceration in Nigeria.

Twenty-two of 24 cases of Mycobacterium ulcerans infection in Ibadan are reviewed. The patients included Africans, Caucasians, and Indians. There were no differences between races in the manifestations of the disease. The average age of the patients was higher than that in other reports in the literature. The fact that most cases were originally wrongly diagnosed reemphasizes the need to search for M. ulcerans in cases of "tropical" ulcer that fail to respond to adequate therapy. Early recognition and surgery are the mainstay of treatment. Comexazole seems more effective than clofazimine in the treatment of these ulcers.

Renographic studies in vesical schistosomiasis in children.

Renal function in children infected with Schistosoma haematobium and in uninfected controls has been studied with the I131 hippuran renogram. The results suggest that renographic abnormalities which persist after a water-load are more common in infected children who have heavy egg-output in their urine, and abnormal pyelograms. Specific chemotherapy restored the renogram to normal within 3 weeks in several cases. It is suggested that the renogram is useful for the assessment and followup of patients with vesical schistosomiasis, and further work should be done to more clearly define the renal function of such patients.
