Transplantation and implantation procedures after surgical treatment of ameloblastoma.

Attention is called to the fact that in cases of large ameloblastomas systematic, careful treatment is essential. This implies proper reconstruction of the jaw and restoration of the occlusion after adequate extirpation of the tumor. A case involving a large mandibular ameloblastoma is reported. It was successfully treated, step by step, by tumor extirpation, jaw reconstruction with rib transplants and installation of titanium implants (Biotes) in the rib transplants, anterior vestibuloplasty with skin graft, and, finally, an overdenture. Carefully planned coordination of well-documented surgical and prosthetic procedures has achieved a functionally and esthetically successful result.

Re- and allotransplantation of teeth--an experimental study in monkeys.

In each of six monkeys one of the permanent lateral incisors with uncompleted root development was replanted. At the same time the contralateral incisors in four of the monkeys were allotransplanted by pairs. The remaining two contralateral incisors served as controls. After 4 weeks and 4 months, respectively, three monkeys were decapitated and the upper lateral incisors with adjacent periodontal tissue were examined histologically. The replanted teeth in five cases had a vital pulp. In the sixth case, however, the replanted incisor exhibited pulpitis and partial necrosis of the pulp. The periodontal membrane had a normal histologic appearance. In two cases a small area of arrested root resorption was observed. With the allotransplanted teeth the histologic appearance was altered. After 4 weeks a pronounced inflammatory reaction was observed in both the pulpal tissue and the periodontal membrane. The pulp was in most cases necrotic, and marked root resorption was observed. Most of the allotransplanted teeth were resorbed and replaced by an irregular hard tissue that resembled osteoid, which in many cases was connected with the tooth remnants by ankylosis. These results indicate that allotransplanted teeth, in contrast to replanted teeth, exhibit the characteristic features of an allograft rejection, including an inflammatory reaction and resorption.

Absele in bone hemostasis--a clinical and experimental investigation.

Control of bleeding from bone tissue may sometimes be difficult to maintain. Existing methods of haemorrhage control with standard bone wax can interfere with subsequent healing. This paper presents clinical and experimental results of the use of a new absorbable bone sealant, Absele a paste made from stabilised fibrin and soluble collagen.

The effect of polyphloretin phosphate (PPP) on oral infiltration anaesthesia. An experimental investigation.

The possibilities to discontinue the anaesthetic effect using supplementary substances were studied. This investigation showed that administration of polyphloretin phosphate (PPP), 15 minutes after local anaesthetic injection, decreased the duration of infiltration anaesthesia of human teeth. The same result was also found when PPP was replaced by saline solution. However no effect on the duration of soft tissue anaesthesia was noted. There were no signs of adverse reactions.

The tissue-integrated prosthesis in the treatment of edentulous patients. A follow-up study.

This paper reports the results of clinical series of osseointegrated implants (BiotesTM) at the Karolinska Institute, Stockholm. From 1980 to 1985, 350 implants were installed in 68 upper jaws and 440 implants in 83 lower jaws. After observation periods of 1 to 6 years, 307 (88%) maxillary implants and 434 (99%) mandibular implants were fully functional. Of 128 bridges and 23 overdentures 124 (97%) and 21 (91%) respectively were in function at the end of the observation time. The most frequent tissue complications were fistulae (8 cases) and mucosal hyperplasia adjacent to abutments (11 cases). The most frequent prosthetic complications were phonetic problems in the maxilla only (10 cases), acrylic fractures in bridges (3 cases) and fractures or distortion of the metallic framework (2 cases). The results indicated that treatment with osseointegrated implants (BiotesTM) is a safe method with predictable long-term prognosis for rehabilitation of the edentulous patient.

Glass-ionomer cement as retrograde filling material. An experimental investigation in monkeys.

The tissue reaction following the use of glass-ionomer and amalgam, as retrograde filling materials was investigated in an experimental study using 8 monkeys. After apicectomy of the upper central incisors, amalgam and glass-ionomer cement was used at random as retrograde filling material. 2 animals at a time were sacrificed after 2 weeks, 1, 3 and 6 months. Irrespective of the length of time, the tissue reactions were similar for the 2 materials. 2 weeks after surgery, a vascular granulation tissue containing lymphocytes, plasma cells and polymorpho-nucleated leukocytes was observed close to the retrograde fillings. 1 month after surgery, good healing was found, the granulation tissue being replaced by new osteoid or bone formation. After 3 and 6 months, there was complete healing with no inflammatory reaction and a mature alveolar bone surrounding the apicectomized roots.

Indications for surgical removal of the mandibular third molar. Study of 2,630 cases.

Indications for surgical removal of 2,630 impacted mandibular third molars were studied. The investigation comprises all operations during a period of 12 months and was carried out at 4 departments of oral surgery within the Stockholm area. The average age of the patients was 27 years. Pathological changes were seen in about 60% with pericoronitis as the most common diagnosis. Root resorption of the adjacent molar was seen in 4.7% and cysts in 4.5%. Orthodontic indications were noted in 10.7%. In about 20% of cases, prophylactic indications (i.e. complications were predicted of tooth remained) were given as the reason for extraction. It was stated that the indications were in some cases uncertain. Further studies of relevant factors are therefore well motivated.

Impacted maxillary canines--a study of surgically treated patients over 20 years of age.

The prevalence of impacted maxillary canines is about 1-3 per cent. They comprise therefore, an essential part of the oral surgeon's work. There are very few published surveys of oral surgery case material, particularly in adults, in which preoperative condition, treatment and postoperative sequelae are considered. Therefore the author has reported a material of 100 impacted maxillary canines in patients over 20 years of age, surgically treated during 1980-85 at the Department of Oral Surgery, Karolinska Institute, Stockholm. Records and radiographs have been studied and parameters such as preoperative pathologic findings, position of the impacted teeth, indications for treatment, kind of treatment and postoperative consequences have been investigated.

Postoperative complications after surgical removal of mandibular third molars. Effects of penicillin V and chlorhexidine.

A random material of 112 patients, was investigated after surgical removal of impacted lower third molars. 2 experimental groups and 1 control group were studied. Prophylactic medication with penicillin V combined with preoperative rinsing using 0.2% chlorhexidine gluconate (Hibitane) was found to reduce postoperative symptoms, when compared with preoperative rinsing alone. In both cases, patients were compared with the control group.

Long-acting local anaesthetics in oral surgery. Clinical evaluation of bupivacaine and etidocaine for mandibular nerve block.

2 long-acting local anaesthetics, bupivacaine and etidocaine, have been compared to lidocaine with regard to effect during surgical removal of impacted mandibular third molars and effect upon the immediate postoperative pain. It can be concluded from this study that both bupivacaine and etidocaine as well as lidocaine were highly effective and without significant difference with regard to pain blocking during surgery. Both long-acting solutions were significantly superior to lidocaine in providing an extended duration of postoperative pain control. Comparing the 2 long-acting solutions, patients in the bupivacaine group had a significantly longer pain-free period than patients in the etidocaine group.

Endosseus titanium implants in extraction sockets. An experimental study in monkeys.

In a series of monkeys, titanium implants (Xenodent) were installed in the sockets of mandibular incisors. The animals were sacrificed 7 and 12 weeks after the implantations. The healing was studied by histological and microradiographical techniques. A gradual osseointegration free from adverse interference could be noticed.

Periodontal ligament injection. An experimental study in the monkey.

In an experimental double blind, cross-over study in monkeys, the reactions of the periodontal tissue after periodontal ligament injection of local anaesthetic solution were investigated. Lidocaine 20 mg/ml with adrenaline 12.5 micrograms/ml and mepivacaine 30 mg/ml were used, and the injection times were 5 or 20 s. Histopathological analysis showed cell damage in the tissues near the injection areas immediately after the experimental procedure. In the specimens obtained one week after the periodontal ligament injections, the tissues were found to be normal.

Effect of methylprednisolone on complications after removal of impacted mandibular third molars.

Bilateral impacted mandibular third molars were surgically removed in 20 healthy patients. The effects of methylprednisolone on postoperative healing was investigated in a double-blind, cross-over study. Some decrease in postoperative discomfort (swelling and trismus) was noted in the methylprednisolone group. No statistically significant difference was noted, however in comparison with a placebo group.
