[Membranous obliteration of the inferior vena cava in the hepatic portion--postmortem study of a rapidly deteriorated aged case].

A 67-year-old female of Budd-Chiari syndrome was rapidly deteriorated and died. Autopsy revealed membranous obliteration of the inferior vena cava in the hepatic portion with a small hole. The inferior vena cava and right hepatic vein contained fresh and organized thrombi. The left hepatic vein was patent and opened above the membrane. The onset and the clinical course of this case seem to be atypical, probably because of the patent left hepatic vein and a small hole of the membrane. We call attention such a rare case in managing Budd-Chiari syndrome.

[A case report of abdominal aortic aneurysm complicated by left-sided inferior vena cava].

Left-sided inferior vena cava (IVC) was identified preoperatively in the case of abdominal aortic aneurysm. At operation, the left-sided IVC and bilateral common iliac veins were severely pressed by the huge aneurysm, and the walls of these vessels were thin and fragile at the site adhered to the aneurysm. Careful dissection around the common iliac arteries resulted in a successful operation. Without an awareness of such anomaly, accidental injury of IVC and the common iliac veins may occur. Preoperative diagnosis and accurate information from echography, CT scan, and venography of the vena cava are recommended in such rare cases.

A cell division regulatory mechanism controls the flagellar regulon in Escherichia coli.

The formation of flagella in various thermosensitive (Ts) cell division mutants of Escherichia coli was examined at the nonpermissive temperature. The number of flagella per unit cell length decreased sharply after shifting the culture temperature from 30 degrees to 40 degrees C in the following Ts mutants: ftsC108, ftsD1033, ftsE1181, ftsF1141, ftsG29, ftsZ84, parA110, dnaB42, nrdB, and dnaG. It was found that transcription of genes responsible for the formation and/or function of flagella (hag, fla, mot, che) decreased significantly at 40 degrees C. However, in the ftsI730 mutant at the nonpermissive temperature, or in penicillin G treated wild-type cells, cell division was blocked but formation of flagella continued. Moreover, when the cfcA1 mutation, of a gene involved in coordinating DNA replication and cell division, was introduced into the dnaB42 mutant strain, inhibition of cell division and also of formation of flagella at 40 degrees C was relaxed. These results indicate that the flagellar regulon is under the control of a cell division regulatory mechanism.

A new gene controlling the frequency of cell division per round of DNA replication in Escherichia coli.

A novel mutant of Escherichia coli, named cfcA1, was isolated from a temperature-sensitive dnaB42 strain, and found to have the following characteristics. Division arrest and lethality induced by inhibition of DNA replication was reduced and delayed in the cfcA1 dnaB42 strain, as compared with the parental dnaB42 strain. Two types of inhibition of division induced by the addition of nalidixic acid or hydroxyurea were suppressed by the cfcA1 mutation. Under permissive conditions for DNA replication, the colony forming ability of cfcA1 cells was significantly reduced as compared with that of cfc+ cells; conversely the division rate of cfcA1 cells was higher than that of cfc+ cells. The cfcA1 mutation partially restored division arrest induced in the thermosensitive ftsZ84 mutant at the restrictive temperature and suppressed the UV sensitivity of the lon mutation. The mutation was mapped at 79.2 min on the E. coli chromosome. Taking these properties into account, it is hypothesized that the cfcA gene is involved in determining the frequency of cell division per round of DNA replication by interacting with the FtsZ protein which is essential for cell division.

Construction of an ordered cosmid collection of the Escherichia coli K-12 W3110 chromosome.

A cosmid library of the Escherichia coli K-12 W3110 chromosome was constructed in which clones were assigned to locations on the chromosome map by hybridization and genetic marker complementation tests. Approximately 70% of the genome was represented by this library. The identified clones can be maintained in the homologous system and would facilitate genetic studies of E. coli.

[Evaluation of long survival cases treated with intra-arterial cancer chemotherapy using implantable reservoirs].

We have treated unresectable liver tumor with intraarterial infusion chemotherapy using an implantable reservoir since 1983. Out of the total 44 cases receiving the chemotherapy during the period from 1983 to February 1989, the evaluation of 8 cases (18.2%) surviving over a year is reported. The 8 cases consist of 3 cases of primary hepatic cancer, 4 cases of metastatic hepatic cancer and 1 case of malignant hemangiopericytoma of pelvis. The cases of primary hepatic cancer are 2 cases of hepatoma (413, 420 days) and 1 case of cancer of bile-duct (400 days). The metastatic cases are 1 case of gastric cancer (826 days), 2 cases of colo-rectal cancer (698, 1080 days) and 1 cases of leiomyosarcoma of small intestine (577 days). A case of malignant hemangiopericytoma of pelvis has survived 4 years and 3 months after the infusion chemotherapy via the internal iliac artery. The two cases of colo-rectal cancer were treated with continuous infusion of FUDR via the proper hepatic artery using Infusaid. For the other cases, ADM and CDDP were infused repeatedly with single-shot type Infuse-a port. Intra-arterial infusion chemotherapy is very useful because treatment in the outpatient clinics is possible over the longterm, and it is possible for patients receiving the therapy to maintain quality of life.

[A case of remission induced in diffuse pleural malignant mesothelioma by the treatment with cisplatin and doxorubicin].

A 66-year-old male patient with diffuse pleural malignant mesothelioma was successfully treated with cisplatin and doxorubicin. He was admitted to our hospital because of massive pleural effusion. The diagnosis was made by pleural needle biopsy. He received 100 mg of cisplatin and 50 mg of doxorubicin every 5 weeks. Regression of the tumor and decrease of pleural effusion were observed after the treatment. A partial remission of 10 months duration was achieved. Cisplatin and doxorubicin were thought to be the most active cytotoxic agents in the treatment of malignant mesothelioma.

Genetic mapping of the Escherichia coli gene for the stringent starvation protein and its dispensability for normal cell growth.

To determine its map position, the sSP gene was cloned into plasmid pBR322 and the recombinant plasmid was integrated into the chromosome of a polA mutant at the site of the sSP gene by homologous recombination. The chromosomal location of Ampr was then determined by P1 phage-mediated transduction. Thus, the sSP gene was mapped between gltB and glnF at min 69.5 on the Escherichia coli chromosome. Strains were constructed in which the sSP gene was brought under the control of the lac regulatory system. This indicated that the stringent starvation protein (SSP) is dispensable for growth, at least under normal culture conditions.

[Transarterial embolization in the treatment of hepatoma complicated with cirrhosis, esophageal varices and hypersplenism].

A case of hepatoma with cirrhosis for whom hepatectomy was impossible because of a severe complication is reported. The case has been treated with various treatments, so long survival has been obtained. The patient is a 56-year-old female with hepatoma with cirrhosis. The initial symptom was bleeding from esophageal varices. Her condition was not suitable for hepatectomy because of hypersplenism and remarkable hepatic disorder. Consequently, she was given endoscopic sclerotherapy for esophageal varices, partial splenic embolization for hypersplenism, and transarterial embolization with ADM, Lipiodol and Spongel powder for hepatoma. Although abdominal pain, pleural effusion and bleeding from gastric ulcer appeared after embolization, esophageal varices and hypersplenism were significantly improved; reduction of 75% of hepatoma was observed and AFP decreased from 18.7 ng to 3 ng. At 12 months after the embolization, there is no sign of hepatoma growth, rupture of esophageal varices or hypersplenism.

Response of pancreatic tumor to intraoperative radiotherapy: medical imaging and pathologic system approach.

For analyzing the local reaction of pancreatic carcinoma to electron intraoperative radiotherapy, successive computed tomography scanning for tumor volumetry was employed, together with surgical clip localization for tumor area using orthogonal x-rays. Soon after radiotherapy the tumor volume and clipped tumor region began to decrease. After a certain interval, computed tomography volumetry revealed that the irradiated tumor had started to increase in size again, whereas the clipped area had not. Autopsies demonstrated that the increase related chiefly to cancer regrowth outside the primary radiation field.

Radioprotective effects of thiomethylhydantoin derivatives on Escherichia coli and mice.

Protection of Escherichia coli NIHJ and C57BL mice from the effects of 60Co gamma-rays provided by S-alk(en)yl-L-cysteines and their hydantoin derivatives was examined. E. coli (10(6) cells/ml) suspended in a 20 mM aqueous solution of one of the drugs was irradiated with 60 Gy of gamma-rays. Five week-old male mice were exposed to 5.0-9.5 Gy of gamma-rays after a single intraperitoneal injection of 0.75 mmol/kg body weight of each compound. In both E. coli and mice, S-allyl compounds afforded more effective radioprotection than S-propyl compounds. The replacement of the alpha-hydrogen of S-substituted cysteines by methyl groups decreased the radioprotective effect. Hydantoin derivatives were much more radioprotective than the original sulfur-containing amino acids. Especially, DL-5-allylthiomethyl-5-methylhydantoin had a remarkable radioprotective effect in mice. The gamma-radiolysis mechanism of thiomethylhydantoin derivatives was discussed in connection with the radioprotective effect of the drugs.

Prognostic significance of pancreatic tumor regression after intraoperative radiotherapy.

Tumor volume measurement using tumor-margin clipping radiography was performed for estimating tumor regression of eight pancreatic adenocarcinomas treated with intraoperative electron irradiation. Half of the tumors regressed exponentially from the first day of treatment, but the other half increased slightly within the first week, after which volume reduction occurred. The volume-halving time was calculated from the regression curve. A wide distribution of volume-halving times as well as initial tumor volumes was seen in all patients. No correlation was found between these variables, but the former correlated well with survival time (r2 = 0.80). This well-known technique may offer a chance to foresee the prognosis of the disease by determining the volume-halving time.

[Study on the erroneous diagnosis of fine needle aspiration of breast cancer].

A histological examination of the breast tumors in 852 patients, after they had received fine needle aspiration, was performed. The false positive and false negative cases were then studied; 32 false positive and 15 false negative cases respectively. As causes for misunderstanding, five items were pointed out in the false positive cases and three in the false negative ones. There were 5 false positive cases in which cellatypia was remarkable, and 3 false negative cases whose cellatypia was not significant. Cytologic diagnosis by fine needle aspiration for these 8 patients was difficult.
