Impaired muscle energy metabolism in uremia as monitored by 31P-NMR.

Uremic patients often complain of fatigue and muscle weakness. In order to elucidate the abnormalities of energy metabolism in the muscles of such patients, we measured the concentrations of phosphocreatine (PCr), inorganic phosphate (Pi) and adenosine triphosphate (ATP) as well as the intracellular pH in skeletal muscles by 31P-NMR at rest, during aerobic and anaerobic exercise and during recovery in 15 uremic patients (7 non-dialyzed patients and 8 dialyzed patients) and 6 control subjects. At rest, there was no difference in intracellular pH between the uremic patients and controls, but the concentrations of PCr and ATP in the skeletal muscle were lower in the uremic patients. However, during aerobic exercise, the uremic patients showed a rapid decrease in intracellular pH and a delay in its recovery. They also revealed an increased PCr utilization during aerobic exercise and its delayed resynthesis during recovery. During anaerobic exercise, the uremic patients, especially non-dialyzed patients, displayed a slower decrease in pH than the controls and a delay in its recovery. An increased PCr utilization during anaerobic exercise and a delayed resynthesis during recovery were also demonstrated. These findings suggest that the aerobic and anaerobic energy productions in uremic patients are impaired and that the energy production of the muscle depends on anaerobic glycolysis during exercise. Hemodialysis apparently facilitates recovery of the inhibited enzyme activities of oxidative phosphorylation and glycolysis in uremic patients.

Electroconvulsive treatment: effects on phospholipase C activity and GTP binding activity in rat brain.

The effect of electroconvulsive treatment (ECT) on activities of phospholipase C hydrolyzing phosphatidylinositol (PI-PLC) and phosphatidylinositol 4,5-bisphosphate (PIP2-PLC) and guanosine-5'-(3-O-thio)triphosphate (GTP gamma S) binding activity were examined in membrane and cytosol fractions from four discrete areas (prefrontal cortex, hippocampus, striatum, and amygdala) of the rat brain. A single ECT resulted in an increase in cytosolic activities of PI-PLC in the prefrontal cortex and of PIP2PLC in all 4 brain regions examined. There were no significant changes in either PI-PLC or PIP2-PLC activity in membrane fractions after a single ECT. Repeated ECT caused regionally specific changes in PLC activities as follows: in the prefrontal cortex, both cytosolic PI-PLC and PIP2-PLC and membranous PI-PLC activities were decreased; in the hippocampus, no changes in any PLC activities were seen; in the striatum, only membranous PI-PLC activities were increased; and, in the amygdala, cytosolic and membranous PI-PLC and cytosolic PIP2-PLC activities were increased. The pattern of changes in GTP gamma S binding activity following repeated ECT resembled those found in PLC activity as follows: in the prefrontal cortex, GTP gamma S binding activities were significantly reduced in both membrane and cytosol; in the hippocampus, the activity was decreased in membrane; in the striatum, no changes in GTP gamma S binding activity were seen in any fraction; and, in the amygdala, the activity was increased in cytosol. These findings suggest that ECT has complex effects on the G protein-phospholipase C system, possibly affecting neuronal signal transduction.

A single gene controls resistance to Japanese encephalitis virus in mice.

The inheritance of resistance to Japanese encephalitis virus (JEV) was investigated using inbred strains of mice to study genetic resistance against JEV infection. C57BL/6 mice immunized intraperitoneally (i.p.) with an infective dose of JEV were resistant to intracerebral (i.c.) challenge with JEV, whereas most C3H/He mice treated in the same manner died. C57BL/6 mice developed this resistance 2 weeks earlier than C3H/He after intraperitoneal (i.p.) immunization. Passive transfer of spleen cells from immunized C57BL/6 protected the recipient mice from i.c. challenge, while transfer from immunized C3H/He was less effective. Since immunized athymic nude mice were not resistant to i.c. challenge with JEV, T lymphocytes were considered to be necessary for protection. When F1, F2 and backcross mice derived from C57BL/6 and C3H/He were challenged i.c. with JEV after i.p. immunizations, the number of resistant and susceptible mice were consistent with Mendelian ratios. Thus it can be concluded that resistance to JEV in mice was controlled by a single, dominant autosomal gene which was not linked to a (non agouti)-locus (chromosome 2).

Photoresponsive peptide and polypeptide systems: 10. Synthesis and reversible photochromism of azo aromatic poly(L-alpha,beta-diaminopropionic acid).

Poly-L-alpha,beta-diaminopropionic acid) having azo aromatic side chain was synthesized by the water-soluble carbodiimide procedure. The photochemical properties of the azo polypeptide poly[N beta-p-(phenylazo)benzoyl-L-alpha,beta-diaminopropionic acid] (PPABLDPA) was investigated by absorption and circular dichroism (c.d.) spectroscopy in hexafluoro-2-propanol (HFIP) and dimethylformamide. The photochromism of the absorption band in the visible and ultraviolet wavelength regions was found to be mostly reversible as a function of irradiation time at different wavelengths due to the photostationary state (88% trans)-cis photoisomerization of the azo aromatic moieties. The c.d. spectra exhibited two and three-stage photochromism on irradiation by light. The reversible photo-induced solubility change was also studied. On irradiation PPABLDPA is soluble under ultraviolet light (cis) and precipitates under visible light (88% trans) in HFIP-water. A discussion was presented that includes our previous results on this azo aromatic polylysine homologue series.

Insolubilizing and adhesive studies of water-soluble synthetic model proteins.

Insolubilizing and adhesive studies of water-soluble synthetic copoly(Tyr1 Lysx) (x = 1-10) were examined using tyrosinase in water and simulated seawater systems. Tyrosinase oxidized tyrosine aromatic nuclei, causing intermolecular crosslinking reactions, which have been assigned by the absorption band at around 360 nm. The viscosities of the model polypeptides were affected by salinity and the kinds of salts in solution systems. As a whole the amino acid compositions, salinity, system pH and beta-structure conformation are considered to play roles in the insolubilizing reaction. The bonding strengths of the model polypeptides exhibited tensile strengths of 16-24 kg/cm2 without enzyme and 29-31 kg/cm2 with tyrosinase on iron, and increased up to 10 kg/cm2 on metals by the addition of tyrosinase as an oxidant.

Transient global amnesia and Raynaud's phenomenon in scleroderma.

Transient global amnesia (TGA) is a well-recognized clinical entity, but its pathophysiology and prognosis have remained arguable. We reported that a 63-year-old woman with scleroderma developed two TGA episodes. The patient sometimes suffered from headaches when Raynaud's phenomenon appeared in her fingers, but she did not experience further cerebrovascular events. This case suggests that the unique clinical presentation of this syndrome may result from an ischemic event, possibly triggered by a vasospastic mechanism like Raynaud's phenomenon.

The ability of granulocytes to generate superoxide anions and hypochlorite during phagocytosis: comparison of neonatal granulocytes with adult granulocytes.

The ability of granulocytes to generate superoxide anions (O2-) and hypochlorite (OCl-) during phagocytosis was investigated using peripheral blood samples from adults and cord blood samples from neonates, using the chemiluminescence probe cypiridina luciferin analog (CLA) for O2- generation and luminol (L) for OCl- generation. OCl- generation by granulocytes was also monitored by taurine chloramine formation. The chemiluminescence probe based upon CLA was highly specific for and sensitive to O2- and could be adopted to determine O2- generation in terms of xanthine oxidase units. The CLA-dependent chemiluminescence by cord blood granulocytes was significantly higher than that by normal adult granulocytes. Taurine chloramine formation was significantly correlated with the L-dependent chemiluminescence (L-CL). Thus, the L-CL is considered to be mainly involved in OCl- generated by phagocytizing granulocytes. L-CLs by cord blood granulocytes and normal adult granulocytes were essentially the same during phagocytosis.

[A case of paratesticular rhabdomyosarcoma].

A case of paratesticular rhabdomyosarcoma in an 18-year-old male is reported. The patient was admitted for swelling of the right scrotum and right inguinal pain. Malignant tumor of the right testis was suspected and a high inguinal orchiectomy was performed. The pathological diagnosis was paratesticular rhabdomyosarcoma. Following retroperitoneal lymph node extirpation, chemotherapy, so-called CYVADIC therapy, consisting of cyclophosphamide, vincristine, adriamycin and dacarbazine was employed in 3 regions. The patient is in good health without recurrence 16 months after the surgery.

Inhibitory action of cyclic AMP on inositol 1,4,5-trisphosphate-induced Ca2+ release in saponin-permeabilized platelets.

Ca2+ release triggered by inositol 1,4,5-trisphosphate (IP3) has been measured in saponin-permeabilized human platelets with quin2 or 45Ca2+. Ca2+ was sequestered by intracellular organelles in the presence of ATP, and IP3 released half of the sequestered Ca2+. The addition of cyclic AMP (cAMP) to permeabilized platelets transiently accelerated Ca2+ sequestration, but did not alter the steady-state level. In contrast, IP3-induced Ca2+ release was greatly inhibited by cAMP. Phorbol myristate acetate, an activator of protein kinase C did not affect IP3-induced Ca2+ release. These results indicate that cAMP may be involved in the regulation of IP3-induced Ca2+ release in human platelets.

Elevated plasma prostaglandin E2 levels in schizophrenia.

Plasma levels of prostaglandin E2 (PGE2) were determined with radioimmunoassay in 40 DSM-III schizophrenics, 15 patients with other mental disorders, and 23 normal controls. The mean value of plasma immunoreactivity of PGE2 was significantly higher in the schizophrenic patients than in the normal controls. Schizophrenic patients with high plasma PGE2 levels had more guilt feelings and hallucinatory behavior on BPRS, relatively successful heterosexual relations, and a higher incidence of birth complications.

Accumulation of diacylgylcerol in platelet phosphoinositide turnover in schizophrenia: a biological marker of good prognosis?

Phosphoinositide (PI) turnover was examined by measuring phosphatidic acid (PA) and diacylglycerol (DG) production following thrombin stimulation in platelet membranes from 20 schizophrenic patients, 10 patients with other mental disorders, and 9 normal controls. In 6 of 13 acute schizophrenic patients, DG was not transformed into phosphatidic acid (PA), but accumulated in the platelets instead. The abnormal findings persisted for at least 2 months, but then reversed over a long period. Three years later, the patients with abnormal PI turnover had a significantly better outcome than other acute schizophrenic patients.

Computerised tomography in schizophrenia. Familial versus non-familial forms of illness.

Findings on CT and demographical or clinical data in 80 patients with DSM-III schizophrenia and 45 medical controls were evaluated. Multiple-discriminant analysis showed that the enlargement of the third ventricle and frontal and parietal atrophy could significantly predict the diagnosis of schizophrenia. Widening of Sylvian fissures and parietal atrophy differentiated familial schizophrenics of both horizontal transmission (who had affected siblings) and vertical transmission (who had affected parents or/and offspring) from non-familial patients. Parietal atrophy and a history of birth complications differentiated horizontal from vertical transmission. The CT findings together with some clinical characteristics could differentiate the three subgroups classified by the hereditary form; thus each of these subgroup may belong to a different disease entity, of a different pathophysiology.

[Partial nephrectomy for renal tumors].

We performed partial nephrectomy in 5 patients with renal tumors. In 1 patient with synchronous bilateral renal cell carcinoma, radical nephrectomy was conducted simultaneously for the contralateral kidney. The second patient had a transitional cell carcinoma of the renal pelvis of the contralateral side, for which the pelvis was resected simultaneously. In the remaining 3 patients the contralateral kidney was normal. In these patients the tumor was less than 3.0 cm in size and localized in either the upper pole or peripheral region, for which partial nephrectomy was performed. The weight of the resected kidney ranged 6 to 35 g with a mean of 18.2 g. The tumor size was 1.8 to 4.0 cm, the mean being 2.4 cm. Tumor grading was renal cell carcinoma grade 1 in 2 patients, grade 2 in 2 patients and oncocytoma in one patient. In all patients the tumor was encapsulated with a pseudocapsule with no involvement of the perinephric fat, venous or regional lymph nodes, and the tumor was diagnosed as pT2V0N0M0, stage I. In none of the patients intraoperative or postoperative complications developed with renal function of the operated kidney normal. The follow up interval after the operation was 5 years in 1 patients and 6 months to 1 year and 2 months in the remaining 4 patients. All patients have survived without metastasis or local recurrence in the operated kidney. We believe that partial nephrectomy is worth performing in selected patients with renal cell carcinoma not only in those having lesion in solitary kidney or bilateral kidneys but also in those whose function of the contralateral kidney is normal.

[Cohort study on cariostatic effect of 100ppmF- fluoride mouthrinsing].

The cariostatic effect of fluoride mouthrinsing with dilute fluoride solution in the school dental health programs was examined. The subjects were 138 children (75 of boys and 63 of girls) who carried out fluoride mouthrinsing with solution of 100 ppmF- for 6 years and 105 children (65 of boys and 40 of girls) who had no fluoride mouthrinsing were analysed by cohort study from 1983 to 1989. The cariostatic effect was evaluated by DMFT index and DMF rate for each tooth type in each erupted school grade. The increment of DMFT index was less than that of the control group. There was a clear cariostatic effect with the advance in the school grade. DMF rate for each tooth type of 1 in 2nd grade and 2 in 3rd grade students showed a statistically significant difference. Both that of upper and lower first molars in 1st and 2nd grade also showed the same result as that of upper incisors. In conclusion, a high cariostatic effect was obtained by introducing the use of fluoride mouthrinsing with a solution of 100ppmF- to the school dental health program, and the observation of DMF rate for each tooth type classified with each erupted school grade was a useful method to evaluate the cariostatic effect of fluoride mouthrinsing.

Radioreceptor assay of clonazepam and diazepam in blood for therapeutic drug monitoring.

Rat cerebral cortex was used to prepare a suspension of benzodiazepine receptors. The suspension was mixed with an extract of specimen and [3H]flunitrazepam. The mixture was filtered through a membrane, and radioactivity on the membrane was measured. Clonazepam concentrations in patient plasma specimens determined by radioreceptor assay and gas-liquid chromatography agreed well. However, diazepam equivalent concentrations determined by radioreceptor assay were close to the sum of diazepam and desmethyldiazepam concentrations determined by high-performance liquid chromatography, as desmethyldiazepam had binding activity for the receptor. This receptor assay method is accurate, simple, requires less than 0.5 ml of plasma, and is therefore suitable for analyzing numerous samples in a short time.
