A double blind study of single dose azithromycin and doxycycline in the treatment of chlamydial urethritis in males.

OBJECTIVE: To compare the efficacy and safety of azithromycin and doxycycline in the treatment of males with uncomplicated urethritis caused by chlamydia trachomatis. DESIGN: A multicentre, double-blind, randomised treatment study. SUBJECTS: 130 male outpatients with clinical signs and symptoms of urethritis. SETTING: STD clinics at four Norwegian University Hospitals. METHODS: Patients were randomly allocated to 1000 mg azithromycin as single dose or doxycycline 100 mg twice daily for 7 days. Clinical, bacteriological and safety assessments were made at entry and after 1 and 2 weeks. Safety data were also repeated after 4 weeks. RESULTS: Demographic data were similar in both groups. At the week 1 assessment bacteriological eradication was achieved in 44 of 44 evaluable azithromycintreated patients and in 42 of 42 in the doxycycline group. At the week 2 assessment the corresponding figures were 35 of 35 and 34 of 34 respectively. CONCLUSION: Azithromycin 1000 mg single dose was as effective as doxycycline 100 mg twice daily for 7 days in male patients with chlamydial urethritis.

Serum levels of vitamin D metabolites in isotretinoin-treated acne patients.

Serum levels of vitamin D metabolites were determined in 11 patients treated for cystic acne with a four-month course of isotretinoin (Roaccutane). The levels were measured before treatment and after two months of medication. We found a significant fall in the level of 1,25-dihydroxyvitamin D (p less than 0.01) and a significant increase in the molar ratio of 24, 25-dihydroxyvitamin D to 25-hydroxyvitamin D (p less than 0.05). No significant changes were found for the vitamin D metabolites 25-hydroxyvitamin D or 24,25-dihydroxy-vitamin D, for serum calcium, phosphorus, alkaline phosphatase or parathyroid hormone. Our data indicate early changes in the metabolism of vitamin D in patients on retinoid treatment.

Intraphagosomal pH in alveolar macrophages after phagocytosis in vivo and in vitro of fluorescein-labeled yeast particles.

The pH in phagolysosomes of rabbit alveolar macrophages was studied using yeast particles labeled with fluorescein isothiocyanate (FITC). The yeast particles were added to the macrophages in vitro a few hours or 1 day after they had been lavaged from the lung and in vivo 3 h or 1 day before the lungs were lavaged. Intracellular pH was estimated from the ratio between the fluorescence intensity at wavelength 519 nm with excitation at wavelengths of 495 and 450 nm. In both the in vitro and in vivo experiments pH decreased significantly during the first hours after lavage, but after a few hours reached almost the day-2 levels, i.e., 4.9-5.4. The decrease in pH was related to time after lavage and not to time after phagocytosis of the particles. It is suggested that intracellular measurements of pH in alveolar macrophages should be combined with determinations of lung clearance of metal particles.

Genital herpes.

Due to increasing frequency genital herpes has received much attention in medical as well as non-medical literature throughout the Western world. This article concentrates on reviewing the various clinical manifestations of the disease. Different diagnostic methods, prophylactic and therapeutic measures (with the exception of acyclovir) are discussed, as well as transmission and spread of the virus.

Fish tank granuloma.

A case of fish tank granuloma is presented. A lesional biopsy sample yielded growth of Mycobacterium marinum on Löwenstein's medium. Treatment with cotrimoxazole was successful.

Acquired angioedema and hypocompleentemia in a patient with myelofibrosis. Effect of danazol treatment.

This paper reports the findings of an "acquired" hereditary angioedema-like syndrome in a patient with myelofibrosis. No previous personal or family history of angioedema was present. The serum complement pattern showed a marked reduction of Cl esterase inhibitor, Clq and C4. All family members had a normal complement profile. Because of frequent attacks of laryngeal angioedema, prophylactic treatment with danazol was started. A striking clinical response was observed as well as a normalizing effect on the underlying biochemical abnormality.
