[Analysis of indications for a repeat Caesarean section].

The aim of the study is to establish the indications for a Resectio Caesarean (RS) and to compare them with those of the first Caesarean section (CS). The Study is retrospective and covers the period from 1.1.2000 to 31.XII.2000. For this period were accomplished 3646 deliveries, 894 (24.52%) of them with S. C. The RS were 182 (20.04%) of all CS. The number and the rate of each indication is calculate on the base of the sum of the first and the second indication from all CS. The first two indications reflect well the real reason for each CS. The results show that the average age of women with RS is grater 2 years and 7 months than those with first CS. The RS are elective in 80.7% of cases and is done at 38.4 +/- 1.4 w.g. The average weight of the newborns from RS is non significantly less with 31 g than this from first CS. The indication "status post CS" is not well defined as it is used in 98.3% of RS. The indication is accepted mostly as a relative one because is added in cases with enough reasonable other obstetric indications. The analysis of the indications of RS shows That 60% of them are justifiable 40% are undertaken on the bases of unconvincing complex indication, but which don't differ from those at first CS.

[Problems and outcome in extremely low birth weight newborns, depending on the mode of delivery].

THE AIM: Of this study is to determine the impact of the mode of delivery on survival, morbidity and prognosis of ELBW and extremely low gestational age (ELGA) newborns. METHODS: The retrospective review includes ELBW and ELGA infants born at the State maternity hospital "Maichin dom" Sofia from 1997 to 1999. These are 95 newborns divided into two groups: 31 born by Cesarean section (C. s.), and 64--per vias naturals (p. v. n.) with subgroups: 42--vertex, 22--breech delivery. Resuscitation is similar in all groups, according the routine practice. They are compared by the following indexes: rate of survival, Apgar scores at the 1-st and at the 5-th min, pH from the umbilical artery (u. a.), rate of the intraventricular hemorrhages (IVH) and periventricular leukomalacia (PVL), and early nevrodevelopmental outcome. RESULTS: The birth weight of all babies includes ranges from 500 to 999 g and the gestational age (g. a.) is > 24 weeks of gestation. There is no statistically significant difference (p < 0.05) according birth weight, pathology and treatment of the mother, between the two groups. In the C. s. group 17 babies survived (55%) while in p. v. n. 30 survived (47%), the worst was survival between the babies, delivered in breech presentation--7 (32%), p < 0.05. There is no significant difference between pH and BE from u. a., but Apgar scores are worse in the breech group, p < 0.05. The incidence of all grades IVH and PVL is almost the same, but there is a significant difference in the incidence of severe, grade 3 and 4 IVH and PVL (19% in C. s. group versus 48% p. v. n. group). The survivors with severe IVH are 57% among the breech delivered (p < 0.05) compared with respectively 33% and 26% among c. s. and vertex delivered groups. A CONCLUSION: Is made that C. s. provides better chances for a safe survival in ELBW and ELGA newborns and better prognosis for neurodevelopmental outcome. With the worst prognosis are the breech delivered ELBW babies.

[Hysteroscopic ablation of the endometrium in cases of dysfunctional uterine bleeding--advantage of preparations including zoladex].

Hysteroscopic endometrial ablation (HEA) is a new alternative for patients with dysfunctional uterine bleeding (DUB), resistant to medical treatment. The relatively thin endometrium is a big advantage at the time of operation. In this article the results of an initial series of hysteroscopic operations (HEA) are given--as a whole and depending on preoperative treatment with GnRH-agonist. Seventeen patients with DUB underwent HEA. Six of them were pretreated with Goserelin acetate (Zoladex 3.6 mg, Astra Zeneca) two subcutaneous application at 28 days interval. The other 11 women were operated in the early postmenstrual period without medical pretreatment. Comparison was made between the two groups regarding preoperative endometrial thickness, operative time, operative complications, duration of hospital stay, change of the menstrual pattern after 6 and 12 months. Results showed 41.2% achievement of persistent amenorrhoea in patients as a whole (62.7% in the Zoladex group and 27.2% in the untreated group). Better results in the patients with Zoladex pretreatment (shorter operative time, higher incidence of amenorrhoea, patient's higher evaluation of the operation) can be explained with the reduced endometrial thickness at the time procedure. The authors consider the hysteroscopic roller-ball endometrial ablation as an upto-date cost-effective method for treatment of DUB. The method is quick, with very low incidence of complications, easy toleration, immediately recovery of the patient and the only possibility for women with high anaesthesiologic and operative risk. Two depot-doses of Zoladex before hysteroscopy lead to better intra- and postoperative results.

[Acute renal insufficiency during pregnancy with acute pancreatitis].

According to current data the incidence of acute pancreatitis is from 1:1000 to 1:5000. Symptoms vary a lot, one of rare but most severe complications being acute renal failure. The case is a 24-year-old pregnant patients--30-th gestational week with symptoms of acute pancreatitis based most probably on hereditary hypertriglyceridemia and hypercholesterolemia. Parallel to acute inflammation of pancrease a hypercoagulation syndrome developed. It is possible that acute renal failure was caused by active thrombus formation. Because of danger for the life of mother and baby, an urgency preterm Cesarean Section was performed. Resussitation post-surgery care and drug therapy (lowmolecular anticoagulants, antibiotics, spasmolytics and analgetics, protease--inhibitors, inhibitors of protome pump, regulators and inhibitors of pancreas secretion normalize renal and pancreatic function if based on special dietary regimr. Coagulation status also normalizes.

[Use of the pulmonary artery catheter (Swan-Ganz) in severe eclampsia].

The use of a balloon-tipped, flow-directed pulmonary artery catheter in critically ill and surgical patients was introduced in the mid-1970s. The purpose of this paper is to review experience with the pulmonary artery catheter in subsets of women with severe preeclampsia. Most reports have been observational in nature, elucidating hemodynamic profiles and effects on these profiles of therapeutic treatment.

[Supplemental multivitamin and multimineral preparations during normal pregnancy--effect on a fetus and delivery].

UNLABELLED: The aim of this prospective study is to assess the effect of non indicated supplementation with Materna during normal pregnancy on some parameters of the neonate and the process of delivery. The material includes 98 women who had been taking Materna during the pregnancy and 27 cases without any medications. The criteria for inclusion are: normally progressing clinically and paraclinically pregnancy and delivery after completing 37th w.g. (95% confident interval 39.0-40.5). RESULT: Most of the pregnant women take Materna for 3 to 5 months (95% CI) during pregnancy. The supplementation with Materna lead to increase of the BIP from the control group of 94.4 mm to 96.5; the occipitofrontal circumference from 33.9 to 35.1 and the high of the neonate from 50.2 to 51.1 respectively. The impression of the obstetricians engage in the study is that the scull bones are more solid and the molding of the head after the delivery is very small or even missing. CONCLUSION: Unjustified supplementation of normal pregnancy with multivitamins and multiminerals lead to increase in the occipitofrontal circumference and to the decrease of the molding capacity of the fetal head wherefore the labor prolongs and the rate of CS and the operative vaginal deliveries increase.

[Lipid metabolism and its changes in pregnancy].

A survey is made on physiology of lipid metabolism and its variations during pregnancy. The mechanisms leading to an increase in triglycerides and cholesterol during pregnancy are explained. These changes induce increased aterogenic risk, higher risk for developing pathologic 1 changes in blood vessels on microcirculation level on the basis of endothelial disfunction. A multiorgan pathology may possibly be provoked--kidneys, pancreas or occurrence of praeeclampsia.

Entropically driven ordering in a binary colloidal suspension near a planar wall.

The local ordering of a binary hard-sphere mixture with a size ratio 1:10 near a planar wall is investigated by means of integral equation theory. We find that when the bulk volume fraction of the smaller particles is greater than 15%, the larger particles (at a bulk volume fraction of 1% and higher) become highly localized on the wall surface, forming a quasi-two-dimensional surface-localized monolayer. Our results are discussed and compared against computer simulation data with an effective one-component Hamiltonian that is based on sphere-sphere and sphere-wall depletion potentials.

[Transplantation of hematopoietic stem cells from umbilical cord blood].

Umbilical cord blood transplantation has many potential advantages over Bone Marrow Transplantation using other donor sources to patients in need of allogeneic hematopoietic stem cell transplantation. Collecting of Umbilical cord blood is a technically simple procedure that poses no foreseeable health risks to the mother or the fetus. The results achieved during the last 10 years of the European Transplantation Centers define this new technology as vanguard and perspective, alternative treatment of the patients obtained oncological disorders.

[Influence of maternal position during delivery of fetal oxygen saturation].

The aim of the study is to investigate the alterations in fetal oxygen saturation during right lateral, supine and left lateral maternal position. Thirty six women at term are included--during the first stage of normally progressing labour. Simultaneous monitoring is carried out of fetal heart rate, uterine contractions, fetal oxygen saturation and maternal arterial blood pressure for a certain period of time in different maternal positions. It was established, that fetal oxygen saturation values are lowest in the supine position, still remaining in physiological ranges--47.1 +/- 10.2. If supine syndrome develops, fetal oxygen saturation values decrease significantly: 23%, 25%.

[Prophylaxis with tercef of infection-related complications after cesarean section].

The purpose of this prospective and case controlled study is to determine the effectiveness and safety of antibiotic prophylaxis with Tercef (ceftriaxone) in women undergoing cesarean section and to compare the results with those of 24 hours regiment of Cefazolin and also with a group without prophylaxis. The study includes 122 cases of elective and emergency CS: 41 with a single intravenous dose of 1.0 g Tercef after clamping of the umbilical cord; 41 cases of antibiotic prophylaxis with Cefazolin three times 2.0 g for 24 hours and 40 low infectious risk CS without antibiotic prophylaxis. We take in account the existing before the CS risk factors for postoperative infectious complications as: hours of PROM; length of labor, number of vaginal examinations before CS, previous sections, duration of the operation, anemia, bacteriuria and diabetes. For post CS infection-related complications we take: febrile morbidity, endometritis, wound infections, infection of urinary tract. The results show infection complication in the three groups as follow: 14.6% for tercef, 17.1% for cefazolin and 20.0% for the group without antibiotic. There is not statistically significant difference. According our study in cases of CS with increased risk of post-operative infectious complications the antibiotic prophylaxis reduce the rate of infection-related complications even below that of CS with low infectious risk. The single dose of 1 g tercef i.v. is effective and suitable in comparison with 24 hours regiment of cefazolin.

[Possibilities for an individualized approach to the treatment of climacteric symptoms with phytoestrogens].

UNLABELLED: In climacteric women with contraindications and fear or lack of compliance for taking hormonal replacement therapy as well as in those cases where risks exceed benefits, the use of phytoestrogens is appropriate having in mind their especially good effect on neurovegetative symptoms. AIM: The present randomized prospective study is aimed at estimating the possibility for ameliorating climacteric symptoms with Melbrosia and at assessment of its effect on different symptoms in order to individualize the therapeutical approach. PATIENTS AND METHODS: Two groups of women with climacteric complaints were followed-up--control group (n = 32) and treatment group (n = 34). Climateric symptoms were assessed in the beginning and at the end of the study using Kupperman menopausal index. The levels of FSH, LH, oestradiol as well as some parameters of lipid metabolism were determined. RESULTS: A significant decrease of Kupperman index was observed in Melbrosia treatment group in contrast to the control group. No significant changes in gonadotropins, oestradiol and lipid parameters were found in both groups. CONCLUSION: The preparation Melbrosia, consisting of phytoestrogens and amino acids exerts a good therapeutic effect on climacteric symptoms mainly on the nervousness, anxiety, irritability, headache, and hot flashes.
