Glutamine metabolism of intestine grafts: influence of mucosal injury by prolonged preservation and transplantation.

The demand for glutamine increased only in the preserved intestine in the early postoperative period (3 days after transplantation). Glutamine demand of the preserved grafts returned to control and immediate levels 7 and 14 days after transplantation. Three days after intestinal transplantation, when the intestinal mucosa was actively regenerating, the demand for glutamine was markedly enhanced. The enhanced demand for glutamine was met by increased output of glutamine by the liver and skeletal muscle. Glutamine uptake by the intestinal graft was enhanced by a brief infusion of glutamine. Thus, we believe exogenous glutamine supplementation may be beneficial for the recovery of intestinal grafts with severe mucosal injury.

Maturational sequence of fetal breathing movements.

The inspiratory times of fetal breathing movements were measured in REM and NREM periods, respectively, and their changes with advancing gestational age were investigated. Ten women without complications in their 20th to 40th week of pregnancy were examined. Fetal breathing movements and REM were observed simultaneously using ultrasonography. REM and NREM periods were selected for analyzing breathing movements separate from the whole examination period. The inspiratory times of fetal breathing movements were measured with the hand-held pressure transducer. The changes in inspiratory time with fetal age showed the same pattern both in REM and NREM periods. There were no significant changes in inspiratory time before 28 weeks gestational age. After that, the inspiratory time lengthened, showing the maximum at 32-35 weeks and then shortened at 36-40 weeks gestational age. There were no differences of the inspiratory time between REM and NREM periods before 32 weeks. After 32 weeks, the inspiratory time in the NREM period was longer than in the REM period. These changes in fetal breathing movements are thought to be caused by the morphological maturation of the fetal lung and the functional development of respiratory and sleep center of the fetal central nervous system.

[A case of adenoma of the nipple with breast cancer].

Adenoma of the nipple is an uncommon lesion which can be mistaken clinically for Paget's disease and pathologically be misinterpreted as an adenocarcinoma. We recently experienced a case of very rare adenoma of the left nipple with left breast cancer. The patient was a 42-year-old, married woman with a firm, partially erosive lump measuring 12 X 12mm in her left nipple and with a left breast lump. Biopsy revealed the breast lump invasive ductal carcinoma and lump of the nipple was adenoma. Modified radical mastectomy was carried out. It is important to bear this rare lesion in mind and perform a thorough examination before making a diagnosis of a lump in the nipple.

[A clinicopathological study on carcinoembryonic antigen in breast cancer].

The production of carcinoembryonic antigen (CEA) by human breast cancer tissue has been studied in relation to the prognosis of 60 patients with breast cancer who were classified in Clinical Stage of UICC I, II and III, and who were treated by radical mastectomy. Tissue CEA was studied in primary tumors using an immunoperoxidase (PAP) method. The obtained results were as followings: 1) Patients who had CEA-negative tumors had significantly higher 10-year survival rates. 2) The positive staining rate of CEA rises as Clinical Stage and metastatic status of lymph nodes advanced. 3) There were significant correlations between staining rate of CEA and classification of histological type and differentiation. 4) There were significant correlations between staining rate of CEA and lymphoid infiltration around main tumor and Sinus Histiocytosis in regional lymph nodes. 5) These results suggest that immunohistological assessment of CEA in breast cancer tissue may provide more precise prognostic information.

Lissencephaly with congenital muscular dystrophy and ocular abnormalities: cerebro-oculo-muscular syndrome.

A Japanese male infant with lissencephaly, congenital muscular dystrophy (CMD), and ocular abnormalities is described. This patient represents features of the cerebro-oculo-muscular syndrome. Cranial computerized tomography revealed diffuse agyria, low density of the white matter, and hypoplasia of the cerebellar vermis. Brain histology suggested type II lissencephaly. These findings are correlated with other similar conditions, such as Walker-Warburg syndrome, Fukuyama-type CMD, and muscle, eye and brain disease.

A simple and rapid purification procedure for the preparation of tetramethylrhodamine-labeled antibodies.

A conjugate preparation of antibody (Fab) and tetramethylrhodamine (TMR) generally adsorbs free TMR which is very difficult to remove because of its strong hydrophobic binding. On the basis of criteria such as sodium dodecyl sulfate (SDS) gel electrophoresis and staining of the plasma membrane of live cells, we found that simple extraction with n-butyl alcohol or iso-amyl alcohol could remove the contaminating free dye. The procedure is especially useful when one needs to prepare conjugates with low nonspecific binding for the study of lateral diffusion of cell membrane-associated antigens.

Studies on the salivary secretion induced by substance P in perfused submandibular gland of rat.

The purpose of these experiments was to clarify the effects of substance P (SP) on the salivary secretion in comparison with those of autonomic agents. Salivary secretion from rat submandibular gland (SMG) was induced by intravenous infusion into the femoral vein or partial perfusion of SMG with isotonic solution containing SP or autonomic stimulants. Blood and salivary flow rates, and electrolyte concentrations of saliva were measured. The relationship between electrolyte concentrations and salivary flow rate after intravenous infusion of SP was similar to that obtained using parasympathomimetic agents. Salivary flow rates induced by phenylephrine and isoproterenol were significantly increased by the further addition of SP, while the pilocarpine-induced flow rate was not affected. SP alone caused a significant increase in blood flow rate of perfused rat SMG. SP with atropine or phentolamine somewhat increased the blood flow. However, concomitant perfusion of atropine and phentolamine completely inhibited the SP effect. Salivary secretion induced by SP perfusion was reduced by atropine or phentolamine. Both blood flow elevation and salivary secretion induced by SP were reduced with increasing doses of a SP-antagonist. Also blood and salivary flow responses induced by phenylephrine were severely reduced by the SP-antagonist. On the other hand, the salivary secretion induced by pilocarpine was moderately reduced by the SP-antagonist. These results indicate that the salivation and especially the glandular vasodilation induced by SP were partially modified by the autonomic agents.

[A case of stromal sarcoma of the breast].

A 28-year-old woman with a stromal sarcoma of the breast is reported. By palpation, she had uncovered a tumor in the upper inner quadrant of the right breast. The subsequent physical examination and echographic findings suggested a phyllodes tumor and an excisional biopsy was performed. A pathologic examination revealed the infiltration of spindle cells with a fascicular pattern, a bizarre mitotic configuration and no epithelial component, thus leading to the diagnosis of a stromal sarcoma. The patient then underwent a modified radical mastectomy. A stomal sarcoma of the breast is very rare, and early diagnosis and a curative resection improves the chances of survival.

[A study on the definition of early breast cancer].

The most important factor in improving the survival rates of the patients with breast cancer is early detection. A high cure rate can be expected if the condition is discovered in early stage. There were 195 patients with primary breast cancer in our hospital from August 1978 to November 1986. Among 280 patients with breast cancer who received radical operations from July 1970 to November 1986, there were 50 patients (18%) with recurrence. Mammography and echography were effective for the diagnosis of infiltrating cancer smaller than 10mm in diameter as well as those between 11 and 20mm in diameter. None of the patients with non-infiltrating or infiltrating carcinomas smaller than 15 mm or smaller than 20 mm in diameter with negative nodes had recurrence. Consequently, at present we define early breast cancer as the lesions of pathologically noninfiltrating or infiltrating smaller than 15 mm in diameter, or infiltrating smaller than 20 mm in diameter with negative nodes.

[Necessity of adjuvant therapy after radical mastectomy based on a study of patients with negative nodes, who have recurrent breast cancer].

The status of the regional lymph nodes influences the prognosis of women with breast cancer who have received a radical mastectomy. To investigate the characteristics of patients with negative nodes who have had a recurrent breast cancer, a statistical study has been carried out involving 279 women who had received a radical mastectomy in our hospital from July 1970 to October 1986. From this study we conclude that systemic adjuvant therapy after radical mastectomy is necessary for patients with negative nodes who have had a breast cancer greater than 2.1 cm (T2) in diameter or a solid-tubular carcinoma.
