Clinical presentation of inhalational anthrax following bioterrorism exposure: report of 2 surviving patients.

The use of anthrax as a weapon of biological terrorism has moved from theory to reality in recent weeks. Following processing of a letter containing anthrax spores that had been mailed to a US senator, 5 cases of inhalational anthrax have occurred among postal workers employed at a major postal facility in Washington, DC. This report details the clinical presentation, diagnostic workup, and initial therapy of 2 of these patients. The clinical course is in some ways different from what has been described as the classic pattern for inhalational anthrax. One patient developed low-grade fever, chills, cough, and malaise 3 days prior to admission, and then progressive dyspnea and cough productive of blood-tinged sputum on the day of admission. The other patient developed progressively worsening headache of 3 days' duration, along with nausea, chills, and night sweats, but no respiratory symptoms, on the day of admission. Both patients had abnormal findings on chest radiographs. Non-contrast-enhanced computed tomography of the chest showing mediastinal adenopathy led to a presumptive diagnosis of inhalational anthrax in both cases. The diagnoses were confirmed by blood cultures and polymerase chain reaction testing. Treatment with antibiotics, including intravenous ciprofloxacin, rifampin, and clindamycin, and supportive therapy appears to have slowed the progression of inhalational anthrax and has resulted to date in survival.

Determinants of dental esthetics: a rational for smile analysis and treatment.

An attractive smile has always been the focal point of a person's attention to improving esthetic appearance and thus self-esteem. It is the contrasts of shape, color, line, and texture that enable us to differentiate one tooth from another, the teeth from the gums, and the smile from the face To understand why a smile with dentition in harmony is considered beautiful, clinical examples are presented to illustrate how to apply visual perception to the dental environment. These simple principles of visualization and treatment can be successfully applied to enhance the smile of any patient.

Thermal response to multiple use of a twist drill.

The objective of the present investigation was to determine whether injurious thermal changes occur in the pulp chamber if a twist drill is used 10 times, and to find whether there is a correlation between temperature rise and distance between a drilled channel and the pulp chamber. Twenty caries-free, human, premolar teeth were randomly distributed to two dentists. With a new 2 mm twist drill, each dentist prepared 10 pinholes (one hole per tooth). Intrapulpal temperature change during the drilling procedure was recorded. The data were statistically analyzed by the Spearman rank order correlation coefficient. The results showed that twist drills should not be used for more than five pinholes (p less than 0.001). The distance between the pin channel and the pulp chamber does not influence heat generation in the pulp chamber.

Fracture durability of restored functional cusps on maxillary nonvital premolar teeth.

Palatal cusps of nonvital maxillary premolar teeth were restored with a coronal-radicular restoration of posterior composite resin (Occlusin), amalgam (Tytin), or cermet (Chelon-Silver). Mean fracture strength values were: 370 lb for intact premolar teeth, 266 lb for amalgam, 215 lb for posterior composite resin, and 132 lb for cermet. From this in vitro study it can be suggested that for selected endodontically treated maxillary premolar teeth, the dentist can use acid-etch-retained posterior composite resin or pin-retained high copper amalgam material for a definitive coronal-radicular restoration. The use of cermet for this purpose is contraindicated.

Effect of internal microblasting on retention of cemented cast crowns.

Microblasting of cast mental crowns is considered a standard procedure in most laboratories and an effective alternative to acid pickling. During the process of devesting and finishing, the surfaces of cast restorations are microblasted with an abrasive agent. This investigation measured the retention of type III cast gold crowns after cleansing with pickling solution (group I) or microblasting with 50-micron aluminum oxide (group II) or 50-micron glass beads (group III). Microblasting the internal surface of cast crowns with 50-micron aluminum oxide significantly improved the retention of castings luted with zinc phosphate cement.

Profile of alcoholic liver disease in an Indian hospital. A prospective analysis.

To study the profile of alcoholic liver disease in India, 144 consecutive, histologically diagnosed patients were prospectively studied. The patients were divided into those having alcoholic fatty liver (AFL) (Gr. I, n = 32), alcoholic hepatitis (AH) (Gr. II, n = 42) and alcoholic cirrhosis (AC) (Gr. III, n = 70) on the basis of their histological findings. All the patients were males, those with fatty liver being younger than those with cirrhosis. The amount of alcohol consumed by patients with AFL, AH and AC was not significantly different. Similarly, the duration of alcohol consumption was not significantly different between the three groups of patients. The clinical features of the patients were quite similar to those reported from the West, except that AH was relatively milder in our patients. Intake of poor quality country liquor was quite common (60%), but its use was not found to be associated with more severe liver injury as compared with the use of good quality foreign varieties of liquor.

Fabrication of removable stone dies using cemented dowel pins.

Proper dowel pin placement in the working cast is important in the fabrication of a cast restoration. A technique is suggested whereby the dowel pin is orientated and cemented after the working cast has been separated from the impression. This procedure produces a working die that has a correctly positioned dowel pin, is stable, and can be repeatedly returned to its original relationship. Comparison of some properties of polyether and polysulfide materials.

Based on the present measurements on working time, setting time, and compression set of polyether and polysulfide materials, the following observations were made: 1. The manufacturer recommends a working time for polyether that is approximately 40 seconds more than that indicated by the clinical or ADA test. 2. Clinically, if more working time is desired, polysulfide impression material should be used. 3. Polysulfide material is more sensitive to temperature change than the polyether material. 4. If the material is allowed to set longer, the percentage change in compression set decreases. 5. For polyether, an increase in setting time of 2 minutes will significantly reduce the distortion of the impression upon removal.

Fabrication of a single posterior intermediate restoration.

Intermediate or temporary crowns are important in cast restoration procedures. A techniques is suggested whereby a metal shell crown is relined with autopolymerizing acrylic resin. It produces a restoration that (1) has good adaptation, (2) maintains gingival tissue health, (3) provides proximal and occlusal contacts, and (4) maintains tooth position.

Fabrication of a single anterior intermediate restoration.

A technique of modifying a polycarbonate shell crown with an acrylic resin reline has been presented. The advantage in using this procedure is that it results in a satisfactory treatment restoration that is functionally and esthetically acceptable to the patient.
