[Management of coronary artery bypass grafting surgery by continuous ambulatory peritoneal dialysis in a patient with chronic renal failure under hemodialysis].

A 64-year-old patient with chronic renal failure underwent coronary artery bypass grafting (CABG) because of the frequent severe angina during hemodialysis. Intraoperative hemodialysis and extra-corporeal ultrafiltration method (ECUM) was used during cardiopulmonary bypass. Postoperative management was successfully performed by continuous ambulatory peritoneal dialysis (CAPD), which made hemodynamics stable and prevented postoperative bleeding, providing with adequate fluid removal and satisfactory control of serum chemistry.

[The clinical studies of intermittent hepatic artery occlusion with infusion chemotherapy--19 cases of unresectable hepatocellular carcinoma and 14 cases of metastatic liver cancer].

For unresectable liver tumors, intermittent occlusion of the feeding artery by means of surgical placement of double lumen balloon catheter in the hepatic artery was combined with chemotherapy. Through the catheter, fluorouracil was continuously infused and one-shot injections of mitomycin or adriamycin were given at the time of balloon inflations. Nineteen patients with primary hepatocellular carcinoma (7 were at Stage III and 11 were at Stage IV) and 14 patients with metastatic liver tumors (4 were at H2 and 9 were at H3 stages) received this newly developed anticancer methodology. In terms of Karnofsky's criteria, 17 of 19 cases with hepatocellular carcinoma achieved Group I responses (89.5%). Fourteen were classified to I-B (73.7%), and one year survival rate was 51.6%. Of 14 cases with metastatic tumors, 13 achieved Group I responses (92.9%). Five were classified to I-B (35.7%), and one year survival was 23.1%. Intermittent occlusion method has advantages in minimizing adverse effects, enabling controlled arterial infusion of anticancer agents and stepwise occlusion of the feeding artery. Despite advanced stages and poor conditions of the patients, remarkable responses were achieved without any serious problems. We are at a conclusion that intermittent occlusion of the hepatic artery when combined with infusion-chemotherapy would, in synergism, be a potential modality against unresectable liver tumors.

[Clinical investigation of plasma prostaglandin levels in patients with digestive cancers].

Plasma PGE2 level of 45 patients with digestive cancers, 10 patients with digestive benign diseases and 10 healthy controls were examined. Some patients were also examined plasma levels of TXB2, 6-keto-PGF1 alpha and PGF2 alpha. Plasma PGE2 level of digestive cancer patients (53.5 +/- 69.1 pg/ml, mean +/- SD) were about 10 times higher compared to healthy controls (5.2 +/- 3.8 pg/ml, p less than 0.01). Patients with digestive benign diseases also revealed elevated level of plasma PGE2, but the change of value were smaller than those of digestive cancer patients. Furthermore, in the case of digestive cancer patients, significantly elevated levels of TXB2 (49.7 +/- 60.3 pg/ml, p less than 0.05) and 6-keto-PGF1 alpha (33.9 +/- 31.5 pg/ml, p less than 0.01), and significantly reduced levels of PGF2 alpha (246 +/- 63 pg/ml, p less than 0.01) were noted. The changes in plasma prostaglandin levels of digestive cancer patients may reflect, even though prostaglandins are present in normal human body and are inactivated rapidly, the biological characteristics of tumors and in vivo reactions of the host.

Dopamine-induced relaxation in human pulmonary arteries.

Dose-dependent relaxations were induced by dopamine in human pulmonary arteries that had been contracted with prostaglandin F2 alpha without alpha-adrenergic blocking agents. The dopamine-induced relaxation was inhibited by haloperidol and fluphenazine, but not by domperidone, suggesting that this relaxation was mediated via DA1 receptors.

[Clinical studies of intermittent hepatic arterial occlusion with infusion chemotherapy for unresectable hepatocellular carcinoma associated with arterioportal or arteriovenous shunts].

Intermittent hepatic artery occlusion combined with infusion chemotherapy is a newly devised methodology. A double lumen balloon catheter was surgically inserted into the hepatic artery. Through the catheter, fluorouracil (5-Fu) was continuously infused and mitomycin (MMC) or adriamycin (ADM) was injected in one-shot at the time of blood flow occlusion. This new methodology was performed in 19 patients who had unresectable hepatocellular carcinoma. Although five of 19 cases had arterioportal (A-P) or arteriovenous (A-V) shunts, four of them also responded well and objective anti-tumor effects resulted. In addition, complications associated with A-P or A-V shunts such as bleeding from the digestive tract due to portal hypertension were well managed by this new methodology.

[In vivo and in vitro studies on PGE2 production and function in human hepatocellular carcinoma].

In order to investigate the production of PGE2 and its' function in human hepatocellular carcinoma, the effects of indomethacin and PGE2 on tumor growth were examined using in vivo and in vitro techniques. HH2-6 cells produced PGE2 in the culture media, and the inverse relationship was observed in between the cell proliferation and the culture supernatant PGE2 levels. While in vivo, plasma and tumor tissue PGE2 levels of tumor bearing nude mice were significantly increased for 1 or 2 weeks after tumor inoculation. In the case of which indomethacin was injected daily into the abdominal cavity (4 mg/kg body weight), the elevation of plasma and tissue PGE2 levels was remarkably suppressed, and the latent time of tumor growth was also prolonged. On the other hand, another case of which PGE2 was injected (10 micrograms or 0.1 microgram i.p.) at first 10 days revealed shortened latent time. These results indicate the intimate relation between PGE2 and latent time on tumor growth. Furthermore, histological findings suggest that tumor derived PGE2 might play an important role in tumor angiogenesis.

[Operative results in cancer of the pancreas, especially complicated with large vascular involvement].

Surgical treatment has been performed in 25 cases of cancer of the pancreas. Localized large vascular involvements of portal vein, celiac axis, common hepatic artery and/or superior mesenteric artery by the pancreatic cancer were observed in 11 cases, which necessitated total or subtotal pancreatectomy associated with segmental resection of the vessels with vascular reconstruction in 7 cases but in remaining 4 cases resectional procedure was not performed. Curative pancreaticoduodenectomy was carried out in 5 cases without large vascular involvement and non-curative pancreatectomy in one case. Exploratory laparotomy or only anastomosis for the bile duct or the intestine was done in 8 cases. The operative resectability rate was 52% and the curative one was 44%. The regional pancreatectomy seemed to contribute to improvement of resectability. However, one year survival rate was only 42% (2 year, 0%) in the curatively resected cases with large vascular involvement, while 5 year survival rate was 67% in those without vascular involvement. The quality of life after extended total pancreatectomy was distressful. On the other hand, two extended totally pancreatectomized cases with successful autotransplantation of distal pancreas are living at one year and 3 months after operation respectively, and enjoying the common social life at present.

[A case of two patients with primary lung cancer secreting CA19-9].

We experienced two primary lung cancer patients who showed high levels of CA19-9 (Carbohydrate Antigen 19-9). One case was small cell carcinoma and the other was adenocarcinoma. In contrast to malignancies of digestive organ origin, rare CA19-9 positive cases are reported in lung cancer. To get evidences that these lung cancer tissues were producing CA19-9, PAP stain (Peroxidase anti peroxidase method: Sternberger) was performed, and positive stains were obtained in tumors surgically removed from these two patients. Especially in a patient who showed very high level of CA19-9 (27,369 U/ml), almost every tumor cell was positively stained. From these findings, we suggest the importance of checking CA19-9 in patients with lung cancer.

[A case of unusual total anomalous pulmonary venous drainage (supra cardiac type)].

Here we report a case of total anomalous pulmonary venous drainage (TAPVD) appearing in an unusual way with Ib type of Darling's classification. The patient was a five-month old male baby. We found that the left pulmonary vein traveled down and then horizontally behind the heart and flowed into SVC together with right middle, lower pulmonary and right superior anterior pulmonary veins. On the other hand the right superior posterior pulmonary vein flowed into SVC directly. We performed a radical operation on this patient by making an internal conduit using a Gore-Tex patch, but he died of LOS on the 2nd postoperative day. We have reviewed and discussed the preoperative diagnosis, the type of surgical procedures and the prognosis of TAPVD in this paper in the light of our case.

[A case of abdominal aneurysm accompanied by an aneurysm of left subclavian artery].

The case was a 62-year-old man. Two years ago under a diagnosis of aneurysm of the left subclavian artery he received: Ligation of aneurysm, aneurysmorrhaphy and extra anatomic bypass grafting from right axillary artery to left axillary artery. This time we palpated a pulsatile mass on his abdomen and performed replacement of abdominal aorta by Y shaped prosthesis following a diagnosis of abdominal aneurysm. In recent years population of advanced age have been increasing and arteriosclerotic diseases including aneurysm are also remarkably increasing. In the future we think that cases of multiple aneurysms such as this one will increase. In Japan we collected fifty cases of aneurysm of the subclavian artery including our case. In this paper we reviewed those cases and discussed the diagnosis and treatment of aneurysm of subclavian artery and multiple aneurysms.

[A clinical report of vascularized segmental pancreatic autotransplantation in chronic pancreatitis].

A vascularized segmental autotransplantation of distal 80% of the pancreas to the right iliopelvic space was successfully performed in a 51-year-old male who had alcoholic chronic pancreatitis. Distal 80% of the gland with severe calcified pancreatitic change was resected and prepared for transplantation by injecting the splenic artery with Ringer's lactate solution containing heparin, albumin and gabexate mecilate cooled to 4 degrees C. The spleen was removed from the specimen. An arteriovenous fistula was constructed at the tail. End-to-side anastomosis of spleno-external iliac veins was carried out after small fusiform resection of the iliac venous wall for the purpose of sufficient venous patency. End-to-end anastomosis of spleno-hypogastric arteries was done. The pancreatic stump was anastomosed to the ileum. Post-operatively, the pain completely resolved. Patency of the graft was demonstrated by angiography. Higher response of the C-peptide immunoreactivity on 75 g OGTT was found in the iliac vein on the transplanted side than in another peripheral vein. The exocrine pancreatic function diagnostant test showed 78.8% post-operatively (vs. 63.2% pre-operatively). Vascularized segmental pancreatic autotransplantation offers an excellent method of relieving pain with preservation of endocrine and exocrine functions in selected cases with chronic pancreatitis.

Current status of benign biliary disorders in Japan and accuracy rates of preoperative diagnoses. Collective review of 14,654 patients.

Data on 14,654 patients preoperatively diagnosed for benign lesions who had undergone surgical treatment during a 2 year period (April 1979 to March 1981) were obtained from 145 institutions. A total of 18,891 biliary disorders were confirmed and classified into 18 groups: stones in the gallbladder, 12,272 patients (83.7 percent of all patients); stones in the extrahepatic bile duct, 3,605 patients (24.6 percent); stones in the intrahepatic bile duct, 570 patients (3.9 percent); acalculous cholecystitis, 463 patients (3.2 percent); idiopathic dilatation of bile ducts, 375 patients (2.6 percent); stenosing papillitis, 274 patients (1.9 percent); benign stricture of bile ducts, 133 patients (0.9 percent); and others. Malignant tumors were found in 243 patients, of whom 132 (54.3 percent) were not expected to have gallbladder cancer. Accuracy rates for preoperative diagnoses in the patients with gallstones of the intrahepatic or extrahepatic bile ducts or the gallbladder, idiopathic dilatation of the bile ducts, benign stricture of the bile ducts, or benign stenosis of the papilla of Vater were higher than 90 percent. However, the false-negative rates for these six disorders, except gallstones of the gallbladder, were higher than 16 percent. Therefore, despite advances in diagnostic procedures before surgery, intraoperative examinations remain indispensable for determining the most effective approach to surgical repair.
