[Serum neopterin in dilated cardiomyopathy].

RIA was used to measure the level of neopterine and beta 2-microglobulin in the sera of 43 patients with a verified diagnosis of dilated cardiomyopathy (DCMP). In DCMP patients, the mean level of neopterine was 11.25 +/- 1.05 nM/l and was significantly higher than in donors (5.6 +/- 0.5 nM/l). The concentration of neopterine exceeding 9 nM/l was discovered in 55% of DCMP patients. The rise of neopterine concentration was particularly marked in patients with inflammatory reaction in the myocardium according to endomyocardial biopsy. In that patients' group, the mean level of neopterine was 12.9 +/- 2.02 nM/l while the rise of its concentration over 9 nM/l was recorded in 9 (81%) of 11 patients examined. The correlation was established between the rise of the concentration of neopterine and beta 2-microglobulin. In patients with the concentration of neopterine over 9 nM/l, the mean level of beta 2-microglobulin constituted 3.33 +/- 0.32 mg/l and in patients with the normal concentration of neopterine, it amounted to 1.91 +/- 0.18 mg/l. The growth of the concentration of beta 2-microglobulin in the groups of patients under comparison was seen in 79 and 14% of cases, respectively. It is concluded that neopterine measurements are advisable in the laboratory diagnosis of myocarditis in DCMP patients. Potential mechanisms of the rise of neopterine formation are discussed from the standpoint of hyperinterferonemia.

[The localization of the components of the extracellular matrix and cytoskeleton in myocardial biopsies in alcoholic cardiomyopathy].

Distribution of I, III, IV and V-type collagen, fibronectin, vimentin, filamin, and desmin in myocardial biopsy specimens of 8 patients with alcoholic cardiomyopathy was studied by indirect immunofluorescence. It is found that all types of collagen and fibronectin participate in formation of diffuse cardiosclerosis while small cardiosclerosis is formed by interstitial types of collagen and to a lower extent of fibronectin. Filamin and desmin distribution in cardiomyocytes proves deformation and destruction of Z-lines and intercalated discs.

[Functional status of the hypertrophic left-ventricular myocardium in patients with hypertension].

27 patients with IIB stage of essential hypertension (EH) and left ventricular hyperthropy (LVH), 14 of them with IHD, were studied. In all of them coronaro- and ventriculography was performed. In 14 cases atherosclerotic alterations of coronary arteries were present. Nobody had abnormal pump function or contractility of myocardium. End-systolic, end-diastolic and stroke volume indexes in EH patients with pronounced coronary atherosclerosis were significantly increased. The examined patients demonstrated significant alteration of the left ventricular myocardium relaxation function. The conclusion is drawn that on a certain stage LVH is of compensatory character and plays an important role in supporting adequate myocardial pump function.

[Quantitative contrast angiography of the right ventricle: the normal indices and the problems in analyzing global and regional contractility].

Normal parameters of the right ventricular (RV) volumes, global and regional contractility were obtained in 20 normal subjects (15 men and 7 women, mean age, 47 +/- 8 yrs) by quantitative contrast RV ventriculography. Analysis of different methods of the angiographic RV volumes calculation was made. It was shown, that Ferlinz's method is adequate to get real values for RV. A new method for computer assessment of RV regional contractility was proposed--shrinkage of 5 areas in percent. The most difference (15 and 17%) between "internal" and "external" observer was found in the anterior and anteroapical segments of RV, differences in another segments were not significant (5-8%).

[Volumetric load in the diagnosis of early right-ventricular heart failure in patients with alcoholic heart damage].

Coronaroventriculographic data are presented for 32 patients with alcoholic heart damage (AHD). Volumetric load (400-450 ml polyglucin administration) was used to detect preclinical heart failure. Coronarography demonstrated atherosclerotic coronary arterial lesions in 7 of 32 patients. A comparative assessment of central hemodynamic and right-ventricular myocardial contractility parameters in the patients and control subjects showed that the ejection fraction was reduced in AHD patients because of a significant increment of end diastolic and end systolic volumes as well as significantly reduced right-ventricular stroke index. Repeated ventriculography after volumetric stress showed significantly increased volumes and reduced ejection fraction that were indicative of latent right-ventricular heart failure.

[Comparative angiographic evaluation of the functions of the left and right heart ventricles in dilated cardiomyopathy].

The paper is concerned with comparative analysis of disorder of left and right ventricular function (LV and RV, respectively) in 30 patients with dilatation cardiomyopathy after coronography, left and right ventriculography, myocardial biopsy. Disorder of LV and RV contractility was noted, however in most cases LV dysfunction was more noticeable. Direct correlation was revealed between the gravity of LV dysfunction and heart failure, whereas no significant differences were noted between RV parameters and heart failure, IIA-IIB stages.

[Total and regional myocardial contractility of the right ventricle during a volumetric loading test in patients with an alcoholic heart lesion].

The authors presented the results of right contrast ventriculography with a volumetric exercise test in patients with alcoholic heart affection. An increase in right ventricular end-systolic and diastolic volumes (most of the former), a decrease in the ejection fraction in response to an increase in the right ventricular end-diastolic pressure were noted during the test. A diffuse decrease in right ventricular regional (segmental) contractility in the absence of zones of akinesia or local contractility disorder was also revealed.

[Determination of the right ventricular volume and ejection fraction by echocardiography (a comparison with angiographic data)].

The determination of function of all cardiac chambers is important in clinical practice. The improvement of the method of two-dimensional echocardiography permitting visualization of the right ventricle (RV) in different topographic projections made possible ultrasonic assessment of RV function. The RV volume and ejection fraction were determined by two-dimensional echocardiography in 25 patients and the results obtained were compared with contrast ventriculography findings. Taking into account the complexity of geometrical shape of the right ventricle its volume based on the results of echocardiography, equaled the sum of the volumes of 3 figures: 2 frusta and one cone. Comparing the results obtained by two methods, significant correlation of all indices characterizing RV function (r-0.85-0.89) was revealed. The prospects of future development of the calculation of RV volumetric indices by the results of two-dimensional echocardiography were discussed. Limitations of the use of this method were described.

[Total and segmental contractile function of the left ventricle in acute myocardial infarction studied by angiography].

An angiographic study of 50 patients with acute myocardial infarction was carried out in order to assess total and segmental left-ventricular contractility. The comparison of left-ventricular activity in patients with transmural vs. nontransmural anterior myocardial infarction demonstrated no differences, related to the type of lesion. The type of affection of the infarction-linked artery largely influences left-ventricular activity. Nontransmural diaphragmatic myocardial infarction was associated with the least left-ventricular dysfunction. Various functional left-ventricular disorders corresponded to definite central hemodynamic patterns, associated with myocardial infarction.

[Normal indices of left ventricular function assessed by using computer technology].

While analysing ventriculograms of 18 persons without any cardiovascular pathology 3 "independent" research workers (to exclude a subjective factor in the appraisal of the left ventricle contours) have developed normal indicators of the total and regional contractility of the left ventricle. The data obtained with the use of angiographic and computer-aided techniques of the recent generation and machine analysis employing unpaired statistics can be recommended for application as normal indicators during the performance of different hemodynamic studies.

[An immunoenzyme method of determining apoprotein B and its diagnostic value in ischemic heart disease].

Enzyme immunoassays were used to measure blood apoprotein B (apo-B), a major protein component of low-density lipoproteins, in 96 patients with coronary disease due to coronary atherosclerosis, 21 patients with angiographically intact coronary arteries, and 130 apparently normal individuals (donors). The results of serum apo-B measurements were correlated to the presence or absence of atherosclerotic coronary arterial lesions, the spread of coronary atherosclerosis, as evidenced by selective coronarography, and patients' sex and age. The measurement of serum apo-B may be helpful in early diagnosis of coronary disease, being particularly valuable in males below 50 years of age.

[Embolism of the anterior descending artery as a cause of myocardial infarct in a patient with a prosthetic aortic valve].

A patient aged 42 with the aortic valve prosthesis was examined with the help of coronography and ultrasonic scanning of the heart in the period of ischemic injury of the anterior myocardium: occlusion of the unchanged anterior descending artery by an embolus formed on the artificial cardiac valve was revealed. Despite rapid recovery of arterial permeability (3 h 47 min, after the onset of disease) by intracoronary administration of streptase with subsequent anticoagulant and antiplatelet drug therapy the patient developed anterior myocardial infarction with left ventricular aneurysm and intracardiac thrombosis.

[Clinical characteristics of the initial stage of alcoholic lesion of the heart in periodic and systematic forms of alcohol consumption].

A combined study of patients with second- or third-stage chronic alcoholism, prone to drinking bouts or regular alcohol consumption, was carried out to evaluate the clinical course of an early stage of alcoholic heart damage (AHD). The AHD patients with second- or third-stage chronic alcoholism and periodic alcohol consumption typically exhibited recurrent chest pains, heart rhythm disorders, signs of heart failure after an alcoholic bout, and those given to regular drinking had somato-vegetative complaints.

[Angiographic study of left and right ventricular function in patients with alcoholic lesions of the heart and dilatational cardiomyopathy].

An angiographic study of left- and right-ventricular function in 57 patients with alcoholic heart and dilatation cardiomyopathy demonstrated preclinical disorders of left-ventricular myocardial contractility and more marked right-ventricular changes in patients with second-stage chronic alcoholism. In cases of dilatation cardiomyopathy, left-ventricular dysfunction was predominant, while right-ventricular changes were less pronounced.

[2-dimensional computed echocardiography. The potentials and limitations of the method in determining the global and local contractile function of the left ventricle].

To study the possibilities of two-dimensional echocardiography (2EchoCG) in evaluating total and regional left ventricular contractility using computerized quantification as compared to biplane contrast ventriculography (CVG) 50 subjects were examined. There was a good correlation between the values of the end-diastolic and end-systolic volumes and the ejection fraction obtained by the two techniques (r = 0.79-0.86) when the volume parameters were calculated by the disk method using Simpson's rule. When the "area-length" method of calculation was used the correlation factors were somewhat lower; 2EchoCG was shown to underestimate the volume parameters and the ejection fraction as compared to CVC. The analysis of regional contractility determined by the method of area changes in a floating axis system revealed high sensitivity and specificity of 2EchoCG as compared to CVG data, the sensitivity being highest when the lateral and septal left ventricular regional contractility was analyzed and lowest when decreased regional contractility was revealed in the inferior area.
