A study of cutaneous tuberculosis in children.

Sixty-three children out of a total of 199 patients seen with cutaneous tuberculosis during a 7-year period were included in this study. Culture was positive in only four, and the diagnosis was based on clinical examination, tuberculin reaction, histopathology, and response to antitubercular therapy. Forty had lupus vulgaris (LV) and 23 scrofuloderma (SD). The lower half of the body was predominantly affected in those with LV, and keratotic and hypertrophic forms were frequently encountered. LV planus mainly affected the face. Ulcerative and atrophic types of LV were infrequent. Extensive lesions in three children led to disfiguring scars and contractures. Scrofuloderma often involved the cervical group of lymph nodes followed by the inguinal, submandibular, and axillary groups. As compared to skin tuberculosis in adults, regional lymph node involvement in LV was more common, and a combination of both LV and SD was less frequent in children. No difference in clinical presentation could be detected between the BCG vaccinated and unvaccinated children. Tuberculous infection either in the lungs or the bones was present in eight children. An HIV test done in five patients with widespread lesions was negative. Irregular therapy or late diagnosis leading to serious complications, inadequate parental or community support, and lack of awareness among practitioners are the problems to be remedied.

Aflatoxin B1 contamination in maize samples collected from different geographical regions of India--a multicentre study.

Under a multicentre study conducted by the Indian Council of Medical Research, 2,074 samples of maize were collected from rural and urban areas of 11 states representing different geographical regions of the country. These samples were analysed for aflatoxin B1 using the AOAC method. Analytical quality assurance between various participating laboratories was ensured through analysis of check-samples. About 26% of maize samples collected from 11 states exceeded the permissible Indian regulatory limit of 30 micrograms kg-1. No statistically significant difference in percentage of samples contaminated (> 30 micrograms kg-1) was observed between pooled rural (27.8%) and urban (23.7%) data. The maximum level of contamination of 666 micrograms kg-1 was observed in the state of Haryana. The median level of < 5 micrograms kg-1 was observed in the states of Gujarat, Haryana, West Bengal, Andhra Pradesh and Karnataka. In all other states studied, the median level was found to vary between 10 and 35 micrograms kg-1.

Induction of oxidative stress in antitubercular drug-induced hepatotoxicity.

BACKGROUND AND AIM: Oxidative stress could play a role in the pathogenesis of antitubercular drug (ATD)-induced hepatotoxicity. We therefore studied the plasma level of reduced glutathione (GSH) and malondialdehyde (MDA) in patients with ATD-induced hepatotoxicity (cases), ATD-treated controls (disease controls) and in healthy volunteers. METHODS: This study was carried out in a case-control design. Twenty-one cases, 21 age- and sex-matched disease controls, and 10 healthy volunteers were enrolled. Plasma levels of GSH and MDA were measured. RESULTS: Plasma levels of GSH (median [range] 11.5 [6.2-21.2] mmol/dL) and MDA (1390 [560-2310] nmol/dL) of cases were significantly different (p<0.01) from GSH (18.4 [10.5-24.4]) and MDA (290 [240-550]) of disease controls. Further, plasma GSH and MDA levels of both the ATD-treated groups were different from those in healthy controls. CONCLUSION: Lower levels of plasma GSH and higher levels of MDA may be due to oxidative stress resulting from ATD therapy.

Induction of lepromin positivity by a candidate anti-leprosy vaccine Mycobacterium w in lepromin negative healthy contacts of multibacillary leprosy patients.

In a hospital based study, 362 household contacts of multibacillary leprosy patients were screened for evidence of leprosy and 54 (14.9%) were found to be having leprosy. The remaining 308 apparently healthy contacts were lepromin tested and 109 (35.4%) were observed to be negative to Mitsuda lepromin. M.w vaccine was administered intradermally to 95 of these 109 lepromin negative contacts. Sixty eight of them could be retested for lepromin A reactivity. Fifty six (82.35%) manifested lepromin conversion. The twelve subjects who did not show lepromin conversion, received a second dose of the vaccine, and eleven subsequently became lepromin positive. The overall lepromin conversion rate was thus 98.5% (67 out of 68). Follow-up of these contacts upto a period of 30 months did not demonstrate reversion of lepromin positivity back to negativity status. No untoward effects of vaccination were observed except for local ulceration at the site of vaccine administration.

Evaluation of the prognostic indices in Legg-Calvé-Perthes disease: statistical analysis of 116 hips.

The records of 100 patients (116 hips) with Legg-Calvé-Perthes' (LCP) disease, with a minimum follow-up of 4 years and treated at the University Hospital, Pellenberg, were reviewed. Sex, Salter's, and Catterall's classifications and lateral subluxation were found to be important prognostic indices. Older children tended to do worse than younger children, but statistical significance could not be established. "Head at risk" factors other than lateral subluxation were found to be unimportant as prognostic indices. Salter's classification was found to be simple, accurate, and significant, and could be made quite early. In the future, this should be an important consideration when making decisions on the management of LCP.

Synovial swellings over wrist in leprosy.

Soft cystic swellings are noticed in leprosy patients during the course of disease and are seen all through the spectrum. The commonest site for these is the dorsum of wrist. At times these are seen over the dorsum and the lateral aspects of ankle as well. These contain straw colored sticky but clear fluid. On exploration these appear to arise from the synovial covering of the extensor tendons of wrist and fingers and have no communication with the wrist joint. Histology of the synovium suggests these swellings to be of inflammatory nature.

Vitiligo on tuberculoid patches--a case report.

A case of tuberculoid leprosy with complete depigmentation on its active margins, in a predisposed individual to vitiligo is presented. The postulated mechanism of hypopigmentation vis-a-vis leprosy lesions is discussed in brief in the context of the presenting unusual association.

Anticardiolipin antibodies in leprosy.

Eighty-four leprosy patients were studied clinically and for IgG and IgM anticardiolipin (aCL) antibodies. Following WHO criteria, 41 patients could be classified as multibacillary (MB) and 43 as paucibacillary (PB). Baseline levels of IgG and IgM antibodies were 27 +/- 4.8 GPL and 20 +/- 3.4 MPL per ml respectively. Comparing with these, 60.9% of MB and 39.5% of PB cases showed rise in IgG and IgM anticardiolipin antibodies; 19.5% of MB and 4.6% of PB sera showed rise in only IgG antibodies, while 4.8% of MB and 13.9% of PB cases showed rise only in IgM antibodies. Rise in aCL antibodies had no correlation with cardiovascular involvement, bacteriological index, reactive state and duration or regularity of treatment.

Evaluation of modified lepro-agglutination as screening test for leprosy.

One hundred thirty-three leprosy sera (83 multibacillary (MB) and 50 paucibacillary (PB) cases) were screened by lepro-agglutination (LA) and M.leprae particle agglutination (MLPA) tests. Larger number of MB sera were positive by LA (86.75%) than by MLPA (45.12%) test. Thirty-seven of the 45 MB sera negative by MLPA test were positive by LA test. The reverse was true in three out of 11 MB sera. PB sera showed positivity of 16% in LA test and 24% in MLPA test. All the 55 sera from normal healthy individuals and 18 VDRL positive sera from syphilis patients were found to be negative by LA test.

Supracondylar femoral extension osteotomy: its complications.

Between 1979 and 1989, 105 supracondylar extension osteotomies of the femur were performed to correct knee flexion contractures. Disturbed by serious neurovascular complications, we reviewed our experience. Besides other complications, nine patients developed serious neurovascular complications; seven had permanent residua. There was no correlation between neurovascular complications and degree of preoperative contracture, patients' ages, or scarring from previous operation. We conclude that this osteotomy, although it appears to be technically simple, is potentially dangerous. We propose that the osteotomy be internally fixed and that the knee be flexed to relax the posterior neurovascular structures.
