[Leydig cell tumor of the testis. Therapeutic and anatomopathologic clinical study of 2 new cases].

Tumors of the testis comprise 1% of male neoplasms. Interstitial cell tumors that originate in Leydig's cells account for 1 to 3% of these testicular tumors. A prognosis is difficult to make since these rare neoplasms manifest clinically in variable forms and it is extremely difficult to distinguish the benign from the malignant. Two new cases of this rare tumor type seen at the Department of Urology of the University Hospital of Florence are reported. The literature is reviewed, highlighting the clinical aspects, particularly the symptomatic, histopathologic, and the differential diagnostic features of the less and highly malignant forms. Furthermore, a protocol for selecting the therapeutic modality for this rare neoplasm is described.

Xanthogranulomatous pyelonephritis: clinical experience with 62 cases.

A review of 62 patients who underwent surgical treatment for xanthogranulomatous pyelonephritis is presented. Polar resection was performed in 3 cases with localized disease, while the remaining 59 patients were treated by nephrectomy. Preoperative diagnosis is still tentative, but the recent advances in imaging techniques and urinary cytology will provide greater accuracy in the future.

[Our experience in the surgical treatment of oncocytoma of the kidney].

Five patients, two men and three women age range 38 to 84 years, were treated for oxyphil cell adenoma of the kidney. Clinical signs were present in three cases and tumor size varied between 7 and 15 cm. Treatment was by nephrectomy in 4 cases and ablation of adenoma in the fifth patient, the tumor being located in the only functioning kidney in the latter. Close follow up surveillance for 32 months failed to detect any recurrence. Clinical and pathologic features of this neoplasm and therapeutic choice in these cases are discussed below. Although radical surgery has been employed in the past as principal therapy, more precise pre- and perioperative diagnosis should allow more frequent use of conservative surgery: partial nephrectomy or tumor excision.

Leiomyosarcoma of the prostate.

3 cases of prostatic leiomyosarcoma, observed over a 15-year period, are reported. 2 patients underwent radical surgical treatment only and succumbed to metastatic diffusion after 13 and 29 months, respectively. After palliative surgery, a 3rd man received subsequent radiation therapy for pelvic and pulmonary recurrences, and is alive with tumor after 6 years and 8 months. The diagnostic and therapeutic aspects of this uncommon and aggressive prostatic neoplasm are discussed.

Chylous ascites after retroperitoneal lymphadenectomy: successful management with peritoneovenous shunt.

A case of chylous ascites occurring after retroperitoneal lymphadenectomy for testicular carcinoma is described. After failure of conservative therapy, the patient was successfully treated with peritoneovenous shunt. The pathophysiological and therapeutic aspects of this rare complication are described; in the world literature only 7 cases of chylous ascites following retroperitoneal lymphadenectomy are reported.

Granulomatous orchitis: clinical and pathologic aspects.

3 cases of granulomatous orchitis are described. Treatment consisted of radical inguinal orchiectomy in 2 cases, and biopsy followed by steroid therapy in the 3. The clinical and pathologic aspects of this rare inflammatory disease of the testis are reviewed.

Inverted papilloma of the ureter.

The clinical and pathological findings of a case of inverted papilloma of the ureter are reported. Treatment consisted of partial ureteral resection, followed by end-to-end anastomosis. This uncommon lesion can be considered as a benign neoplasm, since recurrence and malignant degeneration are quite exceptional. Therefore a plea is made for conservative surgical treatment, although careful long term follow-up is advisable.

[Topical treatment of peri-implantitis with metronidazole dental gel 25%. Clinical analysis and microbiological control].

BACKGROUND: Aim of this research is to verify the efficacy of metronidazole dental gel 25%, used as a topical antibiotic for the treatment of peri-implantitis. The efficacy of the medicine in the starting phase of the disease (mucositis), as well as during peri-implantitis involving bone-bearing loss has been evaluated. METHODS: Twenty patients were chosen as a sample for the study. After careful evaluation of the several protocols concerning peri-implantitis treatment, some diagnostic clinical parameters have been recorded for each patient and metronidazole dental gel 25% (Elyzol Cabon) has been administered in two applications at perimplants pocket (one after a week). For each patient 3 microbiological drawings were made for the evaluation of the bacterial population around the implant site. RESULTS: From the microbiological examination a decrease of Gram- and an increase of Gram+ which returned in almost all the patients to normal levels have been observed; moreover, in all the patients a gradual decrease of PMN, resolution index of the inflammatory process was obtained, confirmed also by a remarkable improvement of all the observed diagnostic parameters, except for the peri-implants bone radiotransparency which was unchanged. CONCLUSIONS: The research showed that the peri-implant diseases can be positively resolved by using the metronidazole dental gel 25% topical antibiotic. This drug led to a 60-70% decrease of Gram-, and 40-50% increase for Gram+, bringing these back to normal values in almost all the patients. Moreover, a good recovery of the peri-implants soft tissues has been observed.

Gemcitabine plus Epi-doxorubicin as first-line chemotherapy for bladder cancer in advanced or metastatic stage: a phase II.

Combination chemotherapy with newer, more active drugs in patients with advanced and/or metastatic bladder cancer might show improved response rate and survival. Gemcitabine (GEM) and Epidoxorubicin (EPI) have demonstrated activity in this disease. In addition, experimental studies in vitro have shown that the two agents have additive-synergistic effects when used in combination. Our prior phase I dose-finding study in previously untreated patients with advanced or metastatic bladder cancer defined recommended doses for further trials of GEM 1000 mg/m2 and EPI 25 mg/m2 on days 1, 8 and 15 every 28 days. A phase II trial at this dose level was initiated in previously untreated patients to assess efficacy and toxicity. Eligible patients had measurable disease; Karnofsky performance status (PS) of > 40; no prior chemotherapy; and adequate bone marrow reserve, cardiac, hepatic and renal function. Thirty- one patients (22 males, 9 females) with median age of 64 (range 44-75) and median PS of 80 were accrued, and all were eligible. Twelve patients had T4N1-2 M0, 8 had lymph node only metastases, while 11 had visceral metastases (liver, bone, lung). A total of 181 cycles was administered (range 3-7 per patient). Major toxicities (WHO grade > or = 3) were: neutropenia in 5 patients, thrombocytopenia in 2 patients, and anemia in 2 patients. Three patients had febrile neutropenic episodes and only 3 patients required dose reduction. Grade 1-2 non-hematological toxicities included nausea/vomiting, stomatitis and alopecia. No cardiac toxicity was observed. Of the 30 response evaluable patients, 17 (57%) demonstrated a major response (3 complete and 14 partial) (95% CI: 39%-75%), 7 had stable disease (23%) and 6 progressed (20%). These preliminary results confirm the phase I observation that the combination of GEM--EPI is highly active in the treatment of advanced and metastatic bladder cancer with a favourable toxicity profile.

[Oncocytoma in a horseshoe kidney].

Oncocytoma is a neoplasm that constitutes 4-5% of tumors located in the renal parenchyma. It was studied for the first time by Zippel in 1942 and since than nearly 150 cases have been described. The association between neoplasm and horseshoe kidney is extremely rare. The horseshoe kidney is a malformation which often becomes complicated by infection, hydronephrosis and calculosis, but very rarely by a neoplasm (only 113 cases). The Authors report a case of oncocytoma in the horseshoe kidney and discuss the diagnostic, clinical and therapeutical aspects.

[Diagnostic, clinical and therapeutic aspects of primary transitional cell carcinoma of the prostate].

Transitional cell carcinoma is a rare neoplasm with an reported incidence of 2%. It is biologically different from adenocarcinoma in that it is not hormonally responsive and it is not associated with an elevated serum acid phosphatase. It is marked by local invasiveness and distant metastases. Transitional cell carcinoma has its origin at the level of the periurethral ducts in the area of the junction of the cylindrical and transitional epithelium. Transitional cell carcinoma can be distinguished from adenocarcinoma and carcinoma endometriode exclusively for its histological aspect. Various therapies have been used to treat transitional cell carcinoma but with poor results. Surgical treatment is still today the most efficient with a surviving average of 2 years. The Authors report 3 new cases, discussing diagnostic, clinical and therapeutical aspects of this rare neoplasm.

[Posttraumatic functional exclusion of the kidney of nonvascular origin. Description of a case and review of the literature].

A review of published series about 56 cases with unilateral nonvisualization on excretory urography after external trauma have been described: the renal injury causing the nonvisualization was contusion in 8 patients, laceration in 13, rupture in 12 and renal pedicle in 19. The acquisition of the most important methods has allowed a total view as about minimal lesions (contusion, laceration), as rupture or fragmentation of the kidney and renal pedicle. We have evaluated 32 cases of renal traumatic injuries and we have studied one case with unilateral nonvisualization stressing the diagnostic, etiopathogenetic and therapeutic problems.

[Paraneoplastic syndromes associated with bladder carcinoma. Apropos of 2 cases of hypercalcemia].

Apart from presenting a specific symptomatology, cancer of the bladder in some rare cases may present with a set of symptoms that at first sight seem unrelated to the tumour and that constitute the paraneoplastic syndromes. The importance of these paraneoplastic symptoms is represented by the fact that they may constitute the first sign of malignity, or may be the expression of a recurrence. Two rare cases of paraneoplastic hypercalcemia are reported, the literature is reviewed and the aspects of this rare pathology are analysed.
