Determination of 3,4-dihydroxyphenylethyleneglycol in urine by gas chromatography with a flame ionization detector: a new rapid method.

3,4-Dihydroxyphenylethyleneglycol, a major metabolite of noradrenaline in rat brain, is estimated alone or with 4-hydroxy-3-methoxyphenylethyleneglycol in rat and human urine by gas chromatography with a flame ionization detector. The samples are hydrolyzed and extracted at pH 2 with ethyl acetate. Then, to analyze only 3,4-dihydroxyphenylethyleneglycol the reaction with n-butaneboronic acid is carried out directly; if 4-hydroxy-3-methoxyphenylethyleneglycol also has to be estimated, preliminary acetylation in alkaline aqueous solution is performed. The advantages of the specificity due to the reagents used is discussed.

Resection of the median nerve without sequelae (resection of 15 cm due to neurofibroma).

A case of neurofibroma of the median nerve at the wrist is reported. An operation to resect 15 cm of the nerve was performed; no transplant was substituted. No motor or sensory paralysis was noted, either directly after the operation or at follow up after nine years. Injection of procaine into the ulnar nerve at the epitrochlear groove demonstrated complete compensation, by means of anastomoses between the two nerves at the upper third of the forearm. It is assumed that the type of lesion (circumscribed neurofibromatosis) beginning in infancy or childhood, had some influence in giving rise to this substitution.

Arthroprostheses without cement in osteoarthrosis of the hip (preliminary report) (16 cases of Sbarbaro arthroprosthesis).

The authors report the results obtained with arthroprostheses without cement. In this preliminary report they examine sixteen cases treated with Sbarbaro's arthroprosthesis from 1972 to 1976, at the Orthopaedic Division of the Specialized Regional Hospital, Udine. They stress the efficacy of this kind of arthroprosthesis, which can be used in subjects under the age of sixty years who have severe bilateral lesions, especially of the femoral head. The operation is contraindicated when there is severe dysplasia of osteoporosis of the acetabulum. Weight bearing must be avoided for six months, the time required for the noncemented arthroprosthesis to become stabilized. The two poor results were attributable to local metabolic changes in one case, and general changes in another, rather than to the arthroprosthesis itself.

Paralysis of the trapezius associated with myogenic torticollis. A report of 6 cases.

The authors report the follow-up results of forty-three patients operated on for myogenic torticollis between 1965 and 1975. The surgical treatment consisted of distal tenotomy in eight cases, distal and proximal tenotomy in twenty-three cases, tenomyomectomy in eight cases. The results were subdivided into three groups: excellent twenty-two, (57 per cent); good , fourteen (38 per cent); poor, two (five per cent). The most interesting finding that emerged was the association of paralysis of the trapezius muscle with myogenic torticollis: this was observed in six cases, three after and three before operation. Preoperative investigations, including an EMG test and confirmation at operation, appeared to show that the paralysis was secondary to compression of the accessory nerve in its passage through the sternocleidomastoid muscle. Neurolysis, performed in three cases, led to recovery of the paralysis. Compression of the accessory nerve is slowly progressive, and paralysis or paresis of the trapezius muscle therefore occurred late. In fact, the patients who presented with both lesions were well beyond infancy. The authors emphasize the importance of being aware of the association of this lesion with myogenic torticollis so that operation for the latter may complemented by neurolysis of the accessory nerve.

[Use of mannitol and propranolol in the treatment of experimental acute renal failure (histomorphological study)].

Experimental research was carried out in order to ascertain whether administration of drugs capable of acting on the renal circulation, such as mannitol and propanol, in addition to improving the renal function, also have a protective effect on the histomorphological alterations induced by the acute renal ischemia. On the basis of the results of the research the Authors conclude by asserting that the combined use of mannitol and propanol has a real protective effect in preventing or attenuating lesions of the kidney caused by serious acute renal failure.

Gluteal fibrosis.

After reviewing the literature the authors discuss the aetiology and pathogenesis of gluteal fibrosis. We believe that the condition occurs more frequently than is thought. If this is borne in mind when dealing with children from four to six years old, presenting with abnormal gaits, an early diagnosis can be made. As regards treatment, we agree with those authors who maintain that if diagnosed early the lesion can be treated successfully by physiotherapy alone. In more advanced cases, Z lengthening of the tendon is indicated.

Removal of total prostheses of the hip. Review of twenty six cases.

The "painful prosthesis" is a difficult problem. Removal is often the best solution. Out of 566 McKee-Farrar prostheses, twenty six (4.2 per cent) were removed. The causes that made removal advisable are analysed. The methods used and the results obtained are described.
