Influence of cigarette smoking on recurrence of duodenal ulcer.

The aim of this study of 225 duodenal ulcer patients was to determine the association of cigarette smoking to ulcer recurrence. In this study, smokers were mostly men whose smoking habit was positively associated with a past history of duodenal ulcer, stress and alcohol consumption. These characteristics are regarded to favor ulcer recurrence. In 161 patients not on maintenance therapy, smokers had a higher recurrence rate than nonsmokers, even when foregoing patients characteristics were taken into consideration. No difference in recurrence rates could be observed between nonsmokers and smokers in 64 patients under maintenance therapy. In conclusion, our findings suggest that smoking plays a major role on recurrence of duodenal ulcer and that maintenance therapy can overcome the adverse effect of cigarette smoking on ulcer recurrence.

Antitumor effects of fotemustine and busulfan against a human neuroblastoma xenograft.

We examined whether or not fotemustine, a new nitrosourea derivative, and busulfan, an agent already clinically used, are effective against human neuroblastoma, using a human neuroblastoma xenograft model designated TNB9. The maximum inhibition rate (MIR) of fotemustine against the TNB9 model was 44.6% with a total dose of fotemustine of 75 mg/kg, indicating that fotemustine is not effective against TNB9. The MIR of busulfan against the same model was 26.7% when a total dose of 135 mg/kg was administered orally to nude mice. Busulfan was also suspended in carboxy-methylcellulose, and was administered intraperitoneally. The MIRs were 19.4% and 36. 4% when busulfan was administered intraperitoneally at a total dose of 40 mg/kg and 60 mg/kg, respectively. The total doses of 40 mg/kg and 60 mg/kg did not show any adverse effects on mice, but were found to be ineffective against TNB9, indicating that busulfan might not be an effective chemotherapeutic agent against human neuroblastoma.

Successful interferon therapy in a neonate with life-threatening Kasabach-Merritt syndrome.

We report a neonatal case of severe, life-threatening Kasabach-Merritt syndrome that was successfully treated with interferon-alpha: This patient had a huge hemangioma of the right leg and a general bleeding tendency. Although the condition initially responded to steroid and radiation therapy, after a relapse, no therapy including steroids, radiation, aspirin, and dipyridamole was effective. Because of severe thrombocytopenia and extension of the hemangioma to the pelvic region, surgical intervention was not indicated. Interferon-alpha therapy was started on day 61 of life. During the therapy the platelet counts increased by more than tenfold and reached the normal level in a month. The size of the hemangioma dramatically decreased. The administration of interferon-alpha might be indicated as a therapy for severe, life-threatening Kasabach-Merritt syndrome, especially when there is resistance to steroid or radiation therapy.

Ultrafast CT scoring system for assessing bronchopulmonary dysplasia: reproducibility and clinical correlation.

PURPOSE: To evaluate the reproducibility of the Ultrafast CT (UFCT) scoring system and assess its usefulness in monitoring clinical severity in infants with bronchopulmonary dysplasia (BPD). MATERIALS AND METHODS: UFCT scoring was done in 22 infants (15 boys and 7 girls aged 1 to 37 months) with BPD. A total of 258 lung fields were evaluated for the presence of hyperaeration, linear opacities, triangular subpleural opacities, and bronchovascular bundle distortion or thickening, and UFCT scores were given. Intraobserver and interobserver agreement and reproducibility of UFCT scores were statistically analyzed. In 12 patients, UFCT scores were linearly correlated with clinical severity scores based on respiratory dysfunction and complexity of care. RESULTS: "Hyperaeration," which was the most frequent (18 of 22, 81.8%) finding, showed high concordance (kappa = 0.73, p < 0.001, kappa = 0.59, p < 0.001), and its UFCT scores significantly correlated with intraobserver and interobserver analyses (r = 0.94, p < 0.001, r = 0.82, p < 0.001, respectively). UFCT scores for hyperaeration significantly correlated with clinical scores (r = 0.75, p < 0.01), whereas those for the others did not. CONCLUSION: UFCT is useful for assessing BPD. Hyperaeration was the most common and reproducible finding, and its extent significantly correlated with clinical severity.
