An adult patient had a cranlopharyngloma that involved and displaced the midportion of the midbrain, producing Weber's syndrome. Oculomotor paresis in cranlopharyngloma is not always due to parasellar involvement but may be due to midbrain involvement.

A patient is reported with a purely pial arteriovenous malformation (AVM) supplied from the posterior parietal artery. The prominent middle meningeal artery contributed to opacification of the angular branches distal to the AVM, but did not contribute to the AVM. After total removal of the AVM, the angular branches became opacified from the middle cerebral artery. Review of the literature suggests that hypertrophied dural arteries which do not contribute to the AVM's but which do opacify the cortical branches distal to the AVM's are rare.
A patient had a subarachnoid hemorrhage in 1969. Four-vessel angiography demonstrated an infundibular widening of the posterior communicating artery alone. In 1978, nine years later, angiography demonstrated that the infundibular widening had enlarged and developed into an aneurysm, though unruptured. Another aneurysm of the anterior communicating artery was also found. Both were confirmed at operation and clipped. Review of the pertinent literature emphasizes that the infundibular widening may be preaneurysmal.
A patient with trigeminal neuralgia caused by direct compression by an elongated basilar artery of the 5th nerve at the zone of entry of its sensory root into the pons is presented. Rhizotomy of the portio major is a treatment of choice in such a case instead of simple microsurgical decompression. The elongated basilar artery is very firm and not readily movable with manipulation.
We report a patient who suffered two traumatic lesions in the same internal carotid artery-a carotid-cavernous fistula and a traumatic aneurysm of the cavernous portion of the internal carotid artery. Both lesions were treated by trapping procedure combined with controlled muscle embolization. The value of intraoperative angiography is emphasized.
The immediate skin reaction (IR) with streptokinase-streptodornase (SK-SD) was found with a high incidence in children with renal disease and was confirmed to be mediated by a specific IgE antibody against SK-SD by means of the radioallergosorbent test (RAST). This test revealed high radiocounts in children with renal disease who showed the IR and there was a close correlation between the RAST counts and the intensity of the IR. Furthermore, the RAST counts in the severe nephrotic syndrome belonging to corticosteroid dependent and resistant group were far higher than those in the corticosteroid sensitive group. From these results, a determination of the RAST counts against SK-SD seemed to help evaluate the severity of some nephrotic syndromes and the specific IgE antibody may be involved in the pathogenesis of the disease.
A case of dural arteriovenous malformation in the anterior fossa is reported. It first caused a subarachnoid hemorrhage and subsequently an intracerebral hematoma. Pertinent literature is briefly reviewed. Such malformations may be divided into three groups; those involving primarily the basal dura mater of the anterior fossa, the anterior portion of the falx, or the dura mater of the frontal convexity. Principal blood supply comes from the ophthalmic artery with an additional supply from the external carotid system. The treatment of choice is surgical excision with or without artificial embolization of the feeding arteries from the external carotid artery.
A clinicopathological study of 13 cases of cranial histiocytosis X was discussed. The patients were mostly infants, and those under 15 years of age numbered eight. Hematological, blood chemistrical, and other laboratorical prodedures have produced no direct diagnostic value. Radiographically, this lesion in skull appeared to be a lytic defect in all cases. Histologically, they were unified by a localized or generalized histiocytic proliferation. Eosinophilic cells were also found throughout the lesional tissue. There were some slight histological variations from case to case, for instance, in the degree of infiltration by eosinophilic cells, and the number of the histiocytic cells. However, the basic histological pattern remained the same in all cases. The lesion may already have extended into the neighboring osseous tissues. Sections of the neighboring osseous tissues showed the intertrabecular spaces to be infiltrated by the same type of lesion. The patient related trauma to the onset of symptoms in 6 cases, but it is true that we know nothing of the etiology of this lesion. The treatment of choice is the adequate surgical extirpation. It seems fair to conclude that wide surgical excision with postoperative irradiation or with chemotherapy may be expected to affect a clinical cure.
A novel model is presented for bacterial active transport reactions which show a curvilinear Eadie-Hofstee plot and negative homotropic cooperativity in the kinetics of substrate uptake. Various models of a single carrier with multi-binding sites for substrate were constructed and examined theoretically. The fit of these models with experimental data on the kinetics of branched chain amino acid transport reactions were tested by iterative computation using the non-linear least square method. The transport model which fitted the experimental data best consisted of a single carrier with three binding sites for substrate in which one of the substrate-carrier complexes, CSS, is not active in translocating substrate across the cytoplasmic membrane. The mechanism of homeostatic regulation of the intracellular concentration of amino acids by active transport systems is discussed on the basis of this transport model.
