[Amiodarone-induced hyperthyroidism].

Amiodarone is extensively used in cardiology practice because of its excellent antiarrhythmic properties. It produces alterations in thyroid functional because it contains 37% iodine and it is structurally similar to the thyroid hormones. Amiodarone inhibits 5'-deiodinase in the liver. The incidence of amiodarone-induced hyperthyroidism is between 6% and 12% of treated patients. The figures for pediatric patients are similar. Determination of tri-iodothyronine (T3), thyroxine (T4) and thyroid-stimulating hormone (TSH) plays an important role in the diagnosis and follow-up of thyroid alterations. Treatment options in amiodarone-induced hyperthyroidism in children include thionamide, potassium perchlorate, and prednisone. We present the case of hyperthyroidism secondary to amiodarone in a 10-year-old boy with Marfan's syndrome who was admitted several times for crises of paroxysmal supraventricular tachycardia and atrial fibrillation. After amiodarone treatment he presented a clinical and analytical picture of hyperthyroidism with very low TSH levels and increased free-T4 levels. Thyroid echography and scintigraphy were normal. Treatment with thiamazole did not alter the clinical picture, which returned to normal after prednisone administration. Currently, prednisone is being slowly withdrawn.Amiodarone. Hyperthyroidism. [Salmonellosis and ulcerative colitis. A case report and review of the literature].

A propos of a case of intestinal infection due to Salmonella non typhi in a patient with ulcerative colitis, we have reviewed the disease and, therefore ran a search through the scientific literature in which we have found 16 cases of ulcerative colitis diagnosed in a clinical setting which began as an enterocolitis due to Salmonella. Bacteriological studies, clinical evolution and response to treatment, normally allow to differentiate between the two clinical entities. Colitis due to Salmonella and ulcerative colitis can coexist in the same patient. Duration of the clinical manifestations oven four weeks, previous history of diarrhea with blood, the sudden worsening of a mild prolonged diarrhea, non-response to specific antibiotic treatment and fast response to therapy with corticoids of a diarrhea positive isolation of Salmonella in stools, should make us immediately think in the coexistence of a subjacent ulcerative colitis. Treatment with steroids always must be associated with systemic antibiotics if Salmonella has been isolated just before or during their administration.

[Experience in the anatomopathological diagnosis of cutaneous melanomas].

The morphologic findings that allow the classification of human cutaneous malignant melanomas are presented. Their frequency, intensity and value in our series are discussed. The diagnosis of any kind of melanoma depends on multiple morphologic findings, Few of them have an absolute value. Morphologic findings with prognostic significance and the difficulties in the diagnosis of malignancy of cutaneous lesions and lymph nodes are also reviewed. The assessment of dermal invasion depends on anatomical and technical problems. Errors in tissue processing are stressed.
