Parabiotic reinnervation: surgical technic and animal care.

A technic called parabiotic reinnervation was developed to test the hypothesis that a derangement of the trophic function normally exerted by a nerve on a muscle is an element in the pathogenesis of muscular dystrophy in the 129/ReJ dydy mouse. The technic involves the reinnervation of the muscles of the anterior compartment of the hind limb of 1 animal (the recipient) by the peroneal nerve of another animal (the donor). Survival of the parabiosed animals depends on attention to nutrition and cage environment.

Association of the A1 allele of the D2 dopamine receptor gene with severe alcoholism.

In a blinded study, 159 subjects composed of nonalcoholics (N = 43), less severe alcoholics (N = 44), severe alcoholics (N = 52) and young children of alcoholics (CoAs, N = 20) were studied for their allelic association with the D2 dopamine receptor (D2DR) gene utilizing peripheral lymphocytes as the DNA source. The combined alcoholic group compared to the nonalcoholic group showed a significantly greater association with the A1 allele of the D2DR gene. Furthermore, an even more robust association was found when severe alcoholics were compared to nonalcoholics. CoAs also showed a significantly greater association with the A1 allele than nonalcoholics but not when compared to alcoholics. Analysis of risk of alcoholism severity suggests that it comprises of two independent components: family history of alcoholism and presence of the A1 allele. Genotype and allelic frequency of the D2DR gene were also analyzed with respect to race. A higher percentage of blacks compared to whites had the A1/A1 genotype, and A1 allelic frequency in the total sample of blacks was significantly greater than in the total sample of whites. Moreover, frequency of the A1 allele was significantly greater in severe alcoholics compared to nonalcoholics in both whites and blacks. However, due to the small sample size of blacks, these racial differences need to be further studied. This study, of the largest sample of alcoholics to date, strongly affirms association of severe alcoholism with the A1 allele of the D2DR gene.

Genetic predisposition in alcoholism: association of the D2 dopamine receptor TaqI B1 RFLP with severe alcoholics.

Previous studies have shown an association of the 3' Taq1 A1 allele of the D2 dopamine receptor (DRD2) gene with severe alcoholism. The recent demonstration of a new polymorphism located closer to the regulatory regions of this gene, permits an associational analysis of these 5' Taq1 B alleles with alcoholism and a comparison with the 3' Taq1 A alleles. Restriction fragment length polymorphism methodology was used to analyze a total of 133 blood samples of nonalcoholics, less severe alcoholics, and severe alcoholics. In white subjects (n = 115), no significant difference in the prevalence of the B1 allele is found between nonalcoholics (n = 30) and less severe alcoholics (n = 36). However, the prevalence of this allele is significantly higher in severe alcoholics (n = 49) when compared to either nonalcoholics (p = 0.008) or less severe alcoholics (p = 0.005). When Taq1 B and Taq1 A alleles of the DRD2 gene are compared in whites, the prevalence of the A1 allele is significantly higher than the B1 allele only in the severe alcoholic group. In conclusion, alleles in both the 5' and 3' region of the DRD2 gene associate with severe alcoholism. This suggests that the DRD2 gene may have an etiological role in some severe alcoholics.

The Early Reading Screening Instrument: a method for identifying kindergarteners at risk for learning to read.

This study was designed to provide speech-language pathologists and educators with a method for identifying children at risk for reading failure. The Early Reading Screening Instrument (ERSI) was given to 149 end-of-the-year kindergarten children. Half of the sample was tested 1 year later with standardized reading measures. Total ERSI scores from the kindergarten children strongly correlated with reading skills in first grade. Reading comprehension in first grade was the skill most strongly predicted by the subjects' total ERSI scores. The word recognition and invented spelling subtests of the ERSI were the best variables to be selected as predictors of first grade word analysis, word identification, and passage comprehension skills. A split-half reliability study of the ERSI showed that a shortened form could be used with the same degree of predictive validity. The ERSI or similar tools can be used to assess reading readiness in kindergarten children so that we can promptly begin an intervention to prevent or, at the very least, to diminish risk for reading failure. For screening reading readiness across different languages or cultures, the content of the four ERSI subtests can easily be modified.

Women's physical activity patterns: nursing implications.

OBJECTIVE: To develop a method of categorizing patterns of physical activity by describing the frequency, intensity, and duration of women's activities. DESIGN: A 24-cell quota sample stratified by four occupations, two races, and three age groups. SETTING: Ten employment sites. PARTICIPANTS: One hundred seventy-six women, ages 35-65 years, who worked 20 or more hours per week at their job, were not currently using hormone replacement therapy, not pregnant, and did not have a hysterectomy before the age of 53. MAIN OUTCOME MEASURES: An interviewer and self-administered, retrospective occupational, household, and leisure physical activity questionnaire covering the previous 12 months and lifelong activity. RESULTS: Five patterns of household and leisure physical activity were identified: vigorous, continuous, cumulative, occasional, and inactive. Participation in the vigorous pattern was low, but 34% followed a continuous pattern of leisure activity and 75% followed a continuous pattern of household activity. The number of weekly work hours did not affect the household or leisure pattern. CONCLUSION: Women may be able to obtain the recommended levels of physical activity from a combination of occupational, household, and leisure activities. Nursing recommendations should guide women to increase their regular leisure physical activity and/or accumulate sessions of moderate-intensity activity by aerobically enhancing daily activities in which they already participate.

Does mesh implantation affect the spermatic cord structures after inguinal hernia surgery? An experimental study in rats.

BACKGROUND: Inguinal hernia repair is the most common operation in general surgery. Prosthetic reinforcement of the inguinal area with polypropylene mesh has increased dramatically in the last decade. The aim of this study was to evaluate how different types of mesh affect the spermatic cord structures. METHODS: Thirty rats were divided into three groups. The spermatic cord was dissected free and a conventional suture repair was performed in group I, an operation mimicking the Lichtenstein operation with a heavyweight polypropylene mesh in group II and the same operation using large pore, lightweighted polypropylene/polyglactin composite mesh in group III. A vasography was performed after 90 days. The cross-sectional area of the vas deferens and s-testosterone from the spermatic vein were measured using the contralateral side as control. Light microscopy of the inguinal canal was performed and inflammation and fibrosis were graded. RESULTS: Vasography revealed patent vas deferens in all animals. In group III, there was a lower s-testosterone in the spermatic vein and a reduced cross-sectional area of the vas deferens on the operated compared to the control side. However, there was no difference in the other groups and there was no significant difference in s-testosterone levels between the groups. There was significantly more inflammation and fibrosis after mesh repair compared to suture repair, but there was no difference between the two mesh groups. Unexpectedly, polyglactin fibres were still seen in specimens in group III after 90 days. CONCLUSION: In conclusion, the only effect on the spermatic cord structures in a rat model is seen as an impaired s-testosterone production and a reduced cross-sectional area of the vas deferens after use of a low-weight composite mesh compared to the control side. No difference in inflammation or fibrosis was found between heavyweight polypropylene mesh and low-weight composite mesh.

Measurement Properties of the Symptom Impact Inventory.

The purpose of this study is to evaluate the measurement properties of the Symptom Impact Inventory using both psychometric and Rasch analyses. This inventory is designed for generally healthy midlife women. The sample included 340 midlife women aged 45-65 representing two studies. The first study involved Black and White employed sedentary women (n = 161) who volunteered for a walking intervention. The second study of migration and health included women who were recent immigrants from the former Soviet Union (n = 179). The women reported experiencing an average of 13.44 symptoms (S.D.=7.88) with a range of 1 to 32. Principal components analysis identified 5 components in this sample. Rasch measurement analysis found excellent model fit for the Symptom Impact Inventory with only 2 symptoms, Decreased appetite and Decreased sexual desire or interest, unstable in scale dimensionality analyses. Person and item parameters were reliable, and comparisons with groups known to differ on symptom reporting provided substantial validity. Although the two sample groups differed significantly on most demographic characteristics, a cross-cultural comparison found the scale structure remarkably robust.

Surveillance for fetal alcohol syndrome in Colorado.

The authors performed surveillance for fetal alcohol syndrome with an existing birth defects registry. Fetal alcohol syndrome cases were identified from multiple sources using passive surveillance and from two selected medical sites using enhanced surveillance. Between May 1992 and March 1994, a total of 173 cases were identified, and the medical records of the cases were reviewed to determine whether the cases met a surveillance case definition for fetal alcohol syndrome. Of these cases, 37 (21 percent) met either definite (28) or probable (9) criteria for fetal alcohol syndrome, 76 met possible criteria (44 percent), and 60 (35 percent) were defined as not fetal alcohol syndrome. Enhanced surveillance had the highest sensitivity for definite or probable cases, 31 of 37 (84 percent), followed by hospital discharge data, 14 of 37 (38 percent). The authors also compared birth certificate information for 22 definite or probable cases in children born between 1989 and 1992 to birth certificate information for all Colorado births for that period. The proportion of mothers of children with fetal alcohol syndrome was statistically significantly greater (as determined by exact binomial 95 percent confidence limits) than the proportion of all mothers for the following characteristics: black race (0.23 versus 0.05), unmarried (0.55 versus 0.22), not employed during pregnancy (0.86 versus 0.43), and started prenatal care in the third trimester (0.18 versus 0.04). Surveillance for fetal alcohol syndrome can be accomplished with an existing registry system in combination with additional case finding and verification activities. Through followup investigation of reported cases, data can be gathered on the mothers of children with fetal alcohol syndrome. These data could be used to target fetal alcohol syndrome prevention programs.
