Local venous lactate changes and spectral analysis of surface EMG during fatiguing isometric efforts in intrinsic hand muscles.

In this study relationships between local venous lactate concentrations and power spectrum changes in surface EMG due to fatiguing isometric contractions were investigated. Two different isometric efforts of the opponents pollicis muscle, 25% and 50% of maximum voluntary contraction, were examined. In 25% of maximum voluntary contraction no changes in local venous lactate were observed, whereas mean power frequency values of surface EMG shifted significantly towards lower frequencies. On the contrary, in 50% of maximum voluntary contraction lactate significantly increased and was related to the mean power frequency shifts of the surface EMG. These data suggest that lactate plays a role in muscle fatigue only in a limited intensity range of isometric muscle contractions.

Pattern of plasma opioids in menstrually-related migraine and epilepsy.

Plasma estradiol (E2), progesterone (P), beta-endorphin (beta-EP) and beta-lipotropin (beta-LPH) were measured in the luteal phase of 8 patients affected by menstrually-related migraine (M) and in 3 cases of catamenial epilepsy (CE). Plasma P and E2 of the M patients were lower than in the CE group. Both beta-LPH and beta-EP showed a reduction in M patients near menses, while the opposite pattern was found in CE. These data demonstrate that premenstrual syndrome is sustained by different, neurobiological dysfunction even if endogenous opioids could be involved in both migraine and epilepsy.

Flunarizine as add-on therapy in epilepsy. Crossover study vs placebo.

The anticonvulsive effect of flunarizine was studied in a multicenter trial, by means of a randomized, double-blind, single crossover design. The subjects who entered the study were 51 males and 39 females, aged 15 to 73 years. They were epileptic patients who suffered from at least two generalized seizures per month or more than 4 partial seizures per month. The patients were already being treated with major antiepileptic drugs. Flunarizine was administered in a single evening dose of 10 mg/die in patients who weighed less than 70 kg and of 15 mg/die in patients who weighed more 70 kg. Our results show that flunarizine, given as add-on therapy, produced a slight but significant decrease in the number of monthly seizures at the end of a 3-month period, while placebo did not significantly change the seizures frequency.

Surface EMG evaluation of quadriceps femoris muscle in hemiplegic patients.

The method applied consists of evaluating the power spectra of the electromyographic (EMG) surface signal obtained during maximal isometric effort of the quadriceps femoris muscle. In hemiplegic patients it is possible to observe a particular frequency distribution towards the lowest values of the range considered. This trend might be accounted for by the morphological and enzyme- histochemical studies on the quadriceps femoris showing a selective atrophy of type 2 fibers. During rehabilitation treatment, however, it is imperative to consider parameters such as muscle strength changes and frequency values preferentially activated during the isometric effort (time-course). Observation of the pattern of the power spectra time-course therefore seems to contribute to a better view of the rehabilitation strategies suitable for exploiting the residual motor abilities.

[Acute poisoning by Vantal (combination of dimethoate and DDT) in a group of longshoremen].

In this work the authors describe a case of acute intoxication by "VANTAL" (DDT and Dimethoate) occurred in a group of six longshoremen. The authors particularly consider the reports about the gastroenteric apparatus and the central and peripheric nervous system. It has in fact been possible to document all kinds of pathologic condition from gastritis to duodenal ulcer in all subjects; whereas only three subjects out of six showed alterations in E.E.G. and E.M.G.

[Experimental EMG-anatomopathological study of rabbits treated with Cialit grafts for the repair of sciatic nerve loss of substance].

The sciatic nerves of 10 rabbits of the New Zealand strain were cut and isolated in the latero-posterior region of the thigh under general anesthesia. Immediate homograft technique was used in animals. The maximal motor conduction velocity of the sciatic nerve was measured in all the animals before the cutting of the nerve. Long-term EMG and anatomo-pathological controls were carried out. The results obtained are discussed. Further research will be continued by applying new techniques and new EMG parameters as well as more sophisticated anatomo-pathological techniques.

Clinical and neurophysiological study in diabetic children and adolescents.

We investigated 82 unselected insulin dependent diabetes mellitus children and adolescents by clinical and electrophysiological evaluation to assess the frequency of diabetic neuropathy and to relate the results to age and height of patients, duration of illness and degree of metabolic control. Clinical abnormalities were found in about 1/3 of patients: these signs of diabetic neuropathy were related to the age and the duration of diabetes but not to the degree of metabolic control. Neurophysiological study showed alterations, especially of sensory nerve conduction. These results were seen to be related to height and age of patients and duration of illness but not to the degree of metabolic control, probably due to the young age of the patients. Our study confirms that diabetic neuropathy is a heterogeneous disorder that may be caused by the interaction of host susceptibility and vascular, metabolic and perhaps environmental components also in diabetic children.
