Activity of oxiracetam in patients with organic brain syndrome: a neuropsychological study.

Oxiracetam is a new psychotropic drug that has been shown to positively affect processes both in animals and in patients with impaired brain function. The aim of this study was the evaluation of the effects of oxiracetam treatment on clinical symptoms in 43 patients with organic brain syndrome (OBS). After a 2-week washout period, patients were assigned to either oxiracetam or placebo, according to a randomized, double-blind, between-patients design. Both oxiracetam and placebo were orally given bid for 8 weeks; daily dose of oxiracetam was 2 X 800 mg. In OBS patients with a mild to moderate degree of cognitive impairment, oxiracetam showed to improve cognitive functions, logical performance, and attention. Other behavioral and functional parameters were also positively modified.

Acute i.v. vincamine teprosilate administration: quantified investigation in elderly subjects.

The EEG changes in elderly subjects with chronic cerebrovascular disorders (CCVD) are well known and have been described by many authors. Vincamine teprosilate (Teproside), a drug supposed to act on the electrical activity of the brain, has the properties of modifying and, to some extent, improving age-related changes. Ten subjects, whose age ranged from 60 to 70 years, underwent the trial. Each received 1 ampoule i.v. of the active drug and 1 ampoule of placebo (or vice versa) after a 48-hour wash-out period, according to a double-blind randomized schedule. EEG recordings were performed at time 0 and then 30 min, 1 h, 2 h, and 4 h after injection. A double effect of vincamine teprosilate could be observed at the quantified EEG: 1) an early effect, i.e., an improvement of the EEG pattern within the first hour following administration; and (2) a slow-occurring effect that prevented negative EEG modifications from taking place at the fourth hour following administration.

Ophthalmoplegia plus: CSF and CT features of seven cases.

The authors describe the clinical and laboratory results relative to 7 cases of Ophthalmoplegia Plus, with special reference to the histochemical, ultra-structural and CFS findings and to the data obtained by Computerized Tomography (CT) of the brain. The CSF was altered in all the examined patients: 3 out of 7 presented an abnormal albumin quotient, 1 patient had an increased IgG Index; the isoelectrofocusing of the CSF proteins, carried out in 6 out of 7 cases, showed an abnormal protein content referred to gamma trace in 3 cases, oligoclonal bands in the CSF in one case, a double Tau fraction in one case, a damaged blood-brain barrier in 5 cases. The CT-performed in 6 out of 7 cases-revealed in one patient a diffuse hypodensity of the periventricular white matter, without signs of atrophy; 2 cases showed a mild atrophy of the posterior fossa involving the cerebellar hemispheres and vermis in one case, the vermis and the brain stem in the other one; no patient revealed signs of cortical or ventricular atrophy. The boundaries of the Ophthalmoplegia Plus are here discussed on the ground of the results obtained by the employment of the various research methods.

Long-term therapy with carbamazepine: effects on nerve conduction velocity.

Peripheral nerve function was evaluated in 30 epileptic patients on chronic therapy with carbamazepine (CBZ) and 20 healthy controls. The electrophysiological data indicated a mild progressive reduction of both motor and sensory conduction velocity in the patients on long-term treatment with the drug. The impairment of nerve function paralleled the duration of therapy (i.e. 1 through 3 years). CBZ plasma levels were constantly within the therapeutic range. Folic acid levels were below normal in about 50% of the subjects. In the absence of other evidence, the hypothesis cannot be excluded that folate deficiency may have played some role in the development of the peripheral nerve dysfunction.

Electromyographic investigation in early diagnosis of carbon disulphide neuropathy: a study on 216 workers with different degrees of exposure.

An electromyographic (EMG) investigation was performed on 216 workers of a viscose rayon plant with different degrees of exposure to CS2. In the light of the data supplied by the survey the AA. maintain that meedle EMG is able to detect, probably better than other test, changes of PNS even if they are very slight, and to select subjects having a higher risk of suffering from neuropathies. Nevertheless, also because of its sensitivity, EMG seems to be less specific than other electrophysiological parameters such as maximal motor conductions velocity (MMCV), that is significantly altered in exposed people. Muscular changes may be connected to a specific effect of CS2 less surely than MMCV change. However, they might be considered as pointing to a predisposition to PNS illness and it seems advisable to eclude these subjects from CS2 exposure.

[Clinical and instrumental evaluation in occupational toxic neuropathies].

The AA conducted clinical electroneurophysiological study on 321 subjects occupationally exposed to neurotoxics substances, hospitalized (Istituto Medicina del Lavoro Università di Pavia) in the period 1977-1981. The AA tried to establish a possible relationship between EMG and clinical date. Results of this study provide further evidence of the diagnostic value of electroneurophysiological investigations and anamnestic data in professional neuropathy.

EEG findings in 118 cases of myasthenia gravis.

We examined 148 EEG recordings from 118 patients affected with myasthenia gravis (35 males and 83 females, aged 17-82, with illness durations between 1 month and 24 years). The cases were clinically evaluated according to Ossermann's classification at the time of EEG recording. EEG patterns were subdivided into normal, diffuse or focal slow abnormalities, or epileptiform patterns. Of the 118 patients, 104 had normal EEG recordings and 14 had at least one abnormal recording (8 diffuse slow abnormalities, 6 focal slow abnormalities). The EEG findings were not correlated with clinical parameters (thymic pathology, respiratory incidents, drug treatment, etc.). One patient (whose EEG recordings were normal) showed an isolated epileptic fit and another patient was affected by epilepsy. A possible relationship between the two diseases is discussed.

Chronic progressive external ophthalmoplegia. Clinical, electrophysiological, histochemical and ultrastructural studies of 14 cases.

Clinical, histochemical and ultrastructural findings concerning 14 cases with diagnosis of Chronic Progressive External Ophthalmoplegia are described. According to the clinical features the patients have been rated in two groups: the first including subjects with isolated ptosis or ptosis with external ophthalmoplegia and the second including subjects with a spreading of the muscular deficit and involvement of the neck and limbs. The most frequent histological and histochemical features are type I fibre atrophy, ragged-red fibres, DPNH-diaphorase reaction disorders and abnormal accumulation of lipids into the fibres. Electron microscopy reveal myofibrillar disorganization and clusters of polymorphous, abnormal mitochondria. In five cases mitochondria contain a variety of crystalline inclusions. Correlations between clinical data and histochemical and ultrastructural findings are discussed. Mitochondrial abnormalities are postulated to be a characteristic physiopathological pattern in CPEO.

Effects of lithium administration on central and peripheral nervous system in rats. Biochemical and morphological findings.

The effect of 4 weeks' treatment with lithium chloride on the central and peripheral nervous system of Wistar albino rats was studied. Normal activity values of some brain enzymes related to energy transduction (LDH, MDH, COX, NADH-ccRT) and neuro-transmission (ACHe), evaluated both in the homogenate in toto and in the crude mitochondrial fraction, were obtained. Fine changes in mitochondrial organelles and nerve processes of neurocytes were ultrastructurally observed. The peripheral nerve studies revealed in some treated rats a slight motor nerve conduction velocity impairment by electro-physiological methods, but no significant alterations in the sciatic nerve specimens examined by electron microscopy.

Immunological findings in epileptic and febrile convulsion patients before and under treatment.

Serum immunoglobulin levels of 86 epileptic patients have been evaluated in order to investigate the relationship between epilepsy, antiepileptic drugs and humoral immunity. The results confirm a high incidence of immunological disorders in the epileptic and febrile convulsion patients. These abnormalities were not related to clinical type of epilepsy nor to the therapy; the common feature seems the early onset of seizures and antiepileptic treatment.

Acute toluene poisoning. Electroneurophysiological and vestibular investigations.

Neurophysiological studies were carried out in six female workers of an electromechanical works after accidental exposure to toluene vapours. Electroencephalographic abnormalities and vestibular hyporeflexia were detected immediately after the accident. Six months later the EEG changes were still present, while vestibular findings were found to be markedly improved. Results of this study provide further evidence of the diagnostic value of vestibular investigations after toxic exposure to toluene.

Chronic treatment with phenytoin in rats: effects of peripheral nervous system.

60 albino Sprague-Dawley female rats, 250 g of body weight, were treated for 6 months with phenytoin 30 mg/100 g/die per os (mean plasma levels: 10-20 gamma/ml). Between 75 and 90 days of treatment motor and sensory nerve conduction velocities were examined along the tail (Miyoshi and Goto, 1973; Fiaschi et al., 1977) in 15 treated animals and in 10 untreated control animals. A slowing of sensory conduction velocity was shown in 6 treated animals (40%); in 2 of them (13%) the motor conduction velocity was also showed. 25 treated rats were controlled with the same neurophysiologic methods after 165-180 days of PHT treatment in comparison with 10 controls; a slowing of sensory conduction velocity was observed in 14 treated rats (56%) and a slowing of motor conduction in 5 of them (20%). The hystological and ultrastructural study carried out on the sciatic nerves of all animals revealed changes of myelinated fibres only in the animals with slowed motor conduction velocity. The non-myelinated fibres were apparently normal.
