Six pediatric patients with lower-extremity excoriated autografts secondary to pruritus were treated randomly. Group 1 was treated with application of an Unna Boot that was changed every 7 days; Group 2 was treated with conventional dressing and antihistamines. Average time for complete wound closure was similar in the two groups: 32 days for group 1 versus 36 days for group 2. Treatment of group 1 cost $19.80/wk compared with $30.90/wk for treatment of group 2. The time for dressing change was 15 min/wk for group 1 and 3.5 hr/wk for group 2. The parents in group 1 gave their children better scores when asked about appetite, sleep, and play patterns.
A retrospective analysis of 29 patients who sustained perioral electrical burns was undertaken. Children were divided into three groups: (group 1) no surgery and no splint (n = 21), (group 2) nonsurgical management with splint appliance (n = 8), and (group 3) commissuroplasty (n = 9). Mean age was 3 years, and minimum follow-up was 1 year. Subjective evaluation of standard photographs was performed by six surgeons. Group 2 had a less noticeable scar and more normal lip parameters. Group 3 and group 1 had similar percent scar involvement and overall poor subjective scores. All evaluators felt that commissuroplasty (group 3) improved patients' appearance compared with no surgery and no splint (group 1). Therefore the application of our easily constructed splint alone yielded the best cosmetic results.
Endotracheal tube stabilization in children with facial burns can be difficult. Several methods rely on straps or complex devices that apply undesirable pressure to the face, potentially destroying skin grafts and making wound care difficult. Techniques that rely on a single wire or suture can be unreliable. Presented here is the arch bar method of endotracheal tube stabilization, which appears to be free of these problems. This method employs a standard dental arch bar secured to four maxillary teeth with 24-gauge stainless steel wire. The endotracheal tube is anchored to the arch bar with two pieces of wire or suture material. The arch bar method of endotracheal tube stabilization was used on three patients in the burn center at St. Christopher's Hospital for Children. Wound care and successful skin grafting were performed without difficulty. No complications related to the arch bars occurred.
PURPOSE: To assess stature in children with orofacial clefting to determine whether this population is at risk for short stature and if growth failure is caused by hypothalamic-pituitary dysfunction or if it is related to age, sex, or type of cleft. DESIGN: Cross-sectional. METHODS: All children 3 to 12 years old with nonsyndromic orofacial clefts who were evaluated as outpatients were measured; those who demonstrated growth failure were to have an evaluation of their hypothalamic-pituitary function. Data were also collected on sex, age, and type of cleft. RESULTS: Children with orofacial clefting had significantly more growth failure than the general population (p < 0.005). The parents of four of the five children with growth failure refused further evaluation because "no one else was ever concerned about my child's height." CLINICAL IMPLICATIONS: The high rate of growth failure in this population emphasizes the need for precise growth assessment of children with orofacial clefting. Growth failure should not be ignored or minimized in populations of children having other significant health care problems.
Free and ethidium bromide (EB) complexed alkali denatured T7 DNA molecules were renatured at 58 and 62 degrees C respectively for 1--3 h. The structures of the renaturation products of the free candidates were as usual showing native like, branched and unrenatured DNA whereas the structures of the ethidium bromide complexed one were somewhat different, showing non-renatured loop-like and entangled regions present inbetween the renatured segments. On the basis of the linear base sequence of T7 DNA, these non-renatured parts are indicative of the inhibition of renaturation by the complexed EB molecules. Mapping of the non-renatured regions showed that they were present at some specific sites, which inturn suggested that the EB-binding has some base sequence specificity.

Acute pulmonary embolus (less than 6 weeks old) has been considered an absolute contraindication to heart transplantation for fear of the potential problems of lung abscess, empyema, bronchopleural fistula, and systemic sepsis in an immunosuppressed patient. It is difficult to adhere to this principle because 30% to 50% of patients with dilated cardiomyopathy may have an acute pulmonary embolus and would be excluded from transplantation. Several centers have considered such patients for heart transplantation if they are young, on maximal medical therapy, and in extremis. The surgical management of the postoperative pulmonary problems can include bronchoscopy, antibiotics, surgical drainage, decortication, and pulmonary resection with or without muscle flaps. We describe our approach to two such patients who were managed successfully with lobectomies and latissimus dorsi muscle flaps to seal the bronchus and fill the pleural space.
