Choice of response scale for health measurement: comparison of 4, 5, and 7-point scales and visual analog scale.

To compare the feasibility of response scales with different numbers of steps for measuring health status, we assessed the ease of completing 4, 5, and 7-point scales and a visual analog scale. Four forms of the questionnaire, each of which consisted of the same ten questions about health status, but with a different type of scale among the four above mentioned, were given to 48 patients with a variety of conditions and diagnoses. The forms were attached together and the order in which they were presented was systematically varied considering permutations of forms. Respondents were asked to complete the forms following the order of the sheets. The 5-point scale form was most commonly chosen as the easiest to complete, and item omission was least frequent with this form. Similar high- reliability results in terms of Cronbach's alpha were achieved for each of the four forms. An additional survey among 153 patients confirmed the results of the first survey. The selection of the easiest scale to complete varied by sex (men preferred the 7-point scale), but was not affected by the conditions or diagnoses of the patients. The study suggests that the 5-point scale is most useful for measuring health status.

[Monthly changes in serum alpha 2-globulin and immunoglobulin levels in chronic hemodialysis patients--with special reference to the occurrence of cancer and blood transfusion].

Thirty-eight patients with chronic renal failure who were treated by hemodialysis more than 100 times each, including 13 cancer patients, were examined for serum alpha 2-globulin and immunoglobulin values monthly, the relationship between the possibility of cancer occurrence and the frequency of blood transfusion during hemodialysis was discussed. The alpha 2-globulin value of the patients treated with blood transfusion more than 20 times was significantly higher than that of the patients not receiving blood transfusion. Moreover, the IgA value was reduced and the IgM value was increased. The IgA values of cancer patients found before or during hemodialysis were significantly higher than those of patients without cancer. There was no significant difference between the group who had undergone hemodialysis more than 500 times those who had received it less than 500 times.

[4 Cases of malignant mesenteric tumor].

Four patients with malignant mesenteric tumors are presented. The first was an 81-year-old male who had non-Hodgkin's lymphoma, the 2nd a 66-year-old male with leiomyosarcoma, the 3rd a 35-year-old male with well differentiated fibrosarcoma. All 3 of these patients underwent radical excision. The 4th patient was a 63-year-old male who underwent exploratory laparotomy only. Postmortem study showed leiomyosarcoma. Based on those and published experiences, we suggest that as soon as a diagnosis of mesenteric tumor has been established with CT scan, GI series etc., surgical removal and Chemotherapy should be performed.

[Leiomyosarcoma of the rectum--a case report].

Tumors of smooth muscle origin are relatively uncommon in the rectum. Patients with leiomyosarcoma are very infrequently encountered. The patient was a 59-year-old Japanese male, who presented complaining of difficulty of defecation and melena. Rectal examination revealed an elastic hard and smooth-surfaced tumor of walnut size at 4 cm on the left antero-lateral wall. Needle biopsy was performed and the tumor was resected trans- sacrally. Histopathologically the tumor, was diagnosed to be leiomyosarcoma. Although the patient received no postoperative chemotherapy or irradiation, he is living without any symptoms or signs of recurrence 5 years and 6 months after the surgery.

[Antitumor immunity in the relatively early period after cryosurgery. Experimental study using rat's metastasizing mammary tumor].

Six-week-old female Sprague-Dawley rats were inoculated with a mammary tumor, MRMT-1. At 2 weeks after inoculation, one of the following 4 treatments was done: surgical tumor excision (SE), cryosurgery (CR), surgical excision plus re-inoculation of freezing-thawing-produced vaccine (FT), and surgical excision plus fasting for 72 hours (FA). In FT and FA groups, incidence of metastatic death were higher than in SE group, while that in CR group was similar as in SE group. Specific footpad reactivity at 2 and 3 weeks after treatment was lower in CR than in SE group. Winn's neutralization assay revealed that antitumor activity of spleen cells at 1 and 3 week(s) after treatment was lower in CR than in SE groups. In vivo observation on effect of inactivated serum at 1 week after treatment showed tumor enhancement in SE group and tumor inhibition in CR group. In conclusion, the observed mild reduction in antitumor immunity in the relatively early period after cryosurgery might not be due to blocking action of superfluous tumor antigens, but probably due to activation of suppressor cells consequent on cryosurgical stress or on slow absorption of tumor antigens.

[A case of metastasis of rectal cancer to the thyroid gland].

A 34-year-old male who showed a rectal mass upon irrigoradioscopy was admitted on October 6, 1979. Abdominoperineal amputation of the rectum was performed on October 11. Histopathological examination revealed that the tumor was adenocarcinoma of the well differentiated type, s, n0., stage II. The patient received chemotherapy and immunotherapy for 10 months postoperatively. In September 1981, enlargement of the thyroid gland associated with pain was noted and on January 12, 1982 total thyroidectomy was performed. Histologically, a diagnosis of well-differentiated adenocarcinoma, which was a metastasis from the rectal cancer, was made. On August 15, 1982, the patient died of generalized metastases.

[Evaluation of pm gastric cancer].

We studied 69 patients with pm gastric cancer seen during the last 10 years. Lymph node metastasis was present in 40.6%; the 5-year survival was 77.1%. The macroscopic form and tumor long diameter showed a good correlation with the rate of lymph node metastasis and the prognosis. Especially with regard to the early cancer-like type and the Borrmann type, remarkable differences were observed in the characteristics and the late results, even among cancers infiltrating to the same pm.
