Nonencephalitogenic CD4-CD8- V alpha 2V beta 8.2+ anti-myelin basic protein rat T lymphocytes inhibit disease induction.

Recently there has been a number of reports suggesting that CD4-CD8- T cells participate in the processes of inflammatory reaction. In an attempt to delineate the distinctive functions of double negative (DN) T lymphocytes in an autoimmune-induced disease, we isolated and cloned such T cells, along with control CD4+ cells, from Lewis rats immunized with guinea-pig myelin basic protein in CFA. Both clones proliferated in response to the guinea-pig myelin basic protein and its synthetic encephalitogenic peptide, and expressed the same TCR V genes homologous to the mouse V alpha 2 and V beta 8.2 families that appear to be the defining entity of experimental autoimmune encephalomyeltis (EAE). Moreover, the TCR D and J region gene products of the DN cell were found to be similar to another encephalitogenic rat T cell clone. The two T clones did not differ markedly in their ability to produce TNF and IL-2 and to adhere to vascular wall-derived extracellular matrix- and laminin-coated plates. Surprising, therefore, was the finding that, although the CD4+ T lymphocytes were capable of inducing EAE, the DN cells did not elicit disease but rather inhibited subsequent EAE induction. Thus, TCR V alpha 2V beta 8.2 and its junctional region gene products are not the only prerequisite segment for a T cell to become encephalitogenic. We suggest that the important determinants of the T cell ability to induce disease are features of the T cell, other than or in addition to, the T cell receptor.

Models of human platelet thrombospondin in solution. A dynamic light-scattering study.

The translational diffusion coefficient (D20,w) of human platelet thrombospondin was measured by dynamic light-scattering. D20,w, measured in 20 mM-Hepes buffer, pH 7.4, containing 350 mM-NaCl and 2 mM-CaCl2, was 1.73(+/- 0.02) x 10(-7) cm2.s-1. After removal of bound Ca2+ by addition of EDTA, D20,w decreased to 1.56(+/- 0.04) x 10(-7) cm2.s-1; this was not a consequence of aggregation. D20,w showed little sensitivity to NaCl concentration between 130 and 550 mM. Through hydrodynamic analysis combining D20,w and other parameters taken from the literature, two major types of models for thrombospondin can be proposed: either classic compact models (i.e. low degree of hydration) such as prolate or oblate ellipsoids with a high axial ratio (greater than 20) or models of low axial ratio made of multiple subunits with significant cavities (i.e. high degree of hydration).

The association between alcohol and breast cancer risk: evidence from the combined analysis of six dietary case-control studies.

Data from 1,575 cases and 1,974 controls enrolled in 6 previously conducted case-control studies of diet and breast cancer have been analysed with respect to alcohol intake. There appears to be an absence of any association between consumption of up to 40 g of alcohol per day and risk of breast cancer, and a highly statistically significant and consistent elevated risk of breast cancer for drinkers of 40 g or more of alcohol per day, for whom the relative risk, as compared with that of non-drinkers, is 1.69 (95% confidence interval 1.19 to 2.40). This association is not due to confounding by a number of diet-related factors, including total calories, fat, fibre and vitamin C.

Analysis of structural design features in collagen.

The principles governing the molecular design of collagen have been investigated using molecular graphics to link the available sequence and X-ray diffraction data. The primary structure is classified into three regions of relevance to the intermolecular packing. Functionally important variations are revealed in the distribution of the amino acid residues. Solvent accessibility studies show that the formation of the triple helix is comparable to the stage of secondary structure formation in globular proteins. The remaining hydrophobic interaction potential serves to determine the three-dimensional packing of the molecules within a fibril. A model for the fibril compatible with the X-ray data is suggested. Particular attention is directed onto the boundaries of the overlap region designated the T zones. Azimuthal orientations of the helical portions at these levels appear to be locked by means of a network of aromatic interactions.

A flexible approach to infrapopliteal vein grafts in patients with diabetes mellitus.

From October 1985 to August 1989, we performed 156 dorsalis pedis artery bypasses in 146 patients, 139 (95%) of whom had diabetes mellitus. A variety of surgical techniques were used to shorten surgery, limit incision length, and optimize size-matching between vein grafts and arteries whenever possible. Arterial inflow was from the common femoral artery in 58 cases, from the distal superficial femoral or popliteal artery in 88 cases, from a tibial artery in three cases, and from a preexisting bypass in seven cases. In situ (75 cases), ex situ reversed- or nonreversed-vein (62), composite vein (nine), and polytetrafluoroethylene (one) bypasses were constructed. Four patients (2.7%) died, and seven grafts (4.5%) failed within 30 days. Actuarial patency and limb salvage 6 to 52 months after surgery were 87.1% and 91.6%, respectively. There were no significant differences in patency between in situ and ex situ vein grafts (93.2% vs 89.7%) or between common femoral artery inflow site and distal superficial femoral/popliteal artery inflow site (89.3% vs 88%).

Prevalence of spirometric abnormalities in a representative sample of the population of Michigan.

A random sample of a large industrial state in the United States (Michigan) was studied via respiratory questionnaire, medical history, physical examination, and spirometry: 976 white adults performed a battery of spirometric tests (FVC, FEV1, FEV1/FVC and FEF25-75%). Using predictive equations derived from normal subjects in the survey and defining spirometric abnormality by the lower 95% confidence interval, prevalences of abnormality were greater in those who had a smoking history. When equations derived from normal non-smokers were used, as has been the general practice, 21.3% of men and 18.5% of women with a positive smoking history had an abnormal FEV1 and 26.7% and 17.8% an abnormal FEF25-75%, respectively. Although FVC is thought to be affected little by smoking, mean values were lower and prevalences of abnormal values higher in smokers. Prevalences are also shown using an adjustment for the effect of smoking and using conventional (percent of predicted value) definitions of spirometric abnormality. The prevalences of spirometric impairments in this cross section of a large state should be useful for comparison with other populations under study in North America.

Radiographic abnormalities in asbestos insulators: effects of duration from onset of exposure and smoking. Relationships of dyspnea with parenchymal and pleural fibrosis.

Chest radiographs and spirometry were evaluated in 2,907 active and retired asbestos insulators; most (86.8%) had greater than or equal to 30 years from onset of asbestos exposure. Testing was performed in 19 cities in the United States during 1981-1983. Complete demographic, smoking, clinical, and radiologic data were obtained for 2,790 workers. This is the largest single group of insulators that has been studied. Five hundred forty-eight (19.7%) had never smoked cigarettes, 942 (33.9%) were current cigarette smokers, and 1,300 (46.6%) were ex-smokers. Only 439 (15.7%) workers had no radiographic evidence of asbestos-related disease (normal chest X-ray); 668 (23.9%) had pleural fibrosis only, 325 (11.6%) had parenchymal fibrosis alone, and 1,358 (48.7%) had both parenchymal and pleural fibrosis. The prevalence of radiographic parenchymal changes increased significantly (p less than .001) from 38.6% (DURONSET less than 30 years) to 70% (greater than or equal to 40 years). For pleural changes the comparative prevalences were 55% and 82%. Those with no history of cigarette smoking were more likely to have normal films than those with a history of smoking (19.2% versus 14.4% for current smokers and 15.2% among ex-smokers), and were less likely to have parenchymal fibrosis (44.5% versus 69.7% for current smokers and 60.2% of ex-smokers). Dyspnea, MRC grade 3 and higher, was more prevalent when pleural fibrosis was associated with interstitial pulmonary fibrosis (at all profusion levels of small opacities) than when pleural fibrosis was absent. Logistic regression analysis of factors contributing to such dyspnea showed that the presence of combined parenchymal and pleural abnormalities was a significant explanatory variable, in addition to age, smoking, and body mass (Quetelet index); the presence of parenchymal changes only or of pleural changes only, as factors contributing to dyspnea, did not reach the level of statistical significance in the regression analysis. The results of these examinations show that pleural fibrosis is a frequent finding in asbestos-exposed groups with long-term follow-up and that its functional significance is not negligible. The contribution of cigarette smoking to prevalence and severity of interstitial fibrosis is an additional reason for smoking cessation among asbestos-exposed individuals.

Pulmonary function and pleural fibrosis: quantitative relationships with an integrative index of pleural abnormalities.

Pleural fibrosis due to asbestos exposure was fully appreciated considerably later than pulmonary interstitial fibrosis due to similar exposure. This is well exemplified by the fact that pleura face on was included in the International Labour Office's International Classification of Radiographs of Pneumoconioses only in the last revision of the Classification. The functional relevance of pleural fibrosis, in particular circumscribed pleural fibrosis, has remained controversial. Since pleural fibrosis can occur at various sites (diaphragmatic plaques, chest wall in profile and face on, mediastinal) and can be of different thickness and extent, a comprehensive integrative assessment of pleural fibrosis was undertaken in order to permit a quantitative study of relationships between pleural fibrosis and pulmonary function. This approach was used for chest X-ray films of 1,584 asbestos insulation workers examined (1981-1983); 1,185 (75%) had pleural fibrosis. The distribution pattern of the integrative pleural index was found to be different in the subgroup with circumscribed (n = 975) from that with diffuse (n = 197) pleural fibrosis, with a higher profusion of high INDEX values in the latter. Stepwise regression analysis indicated that there was a significant inverse relationship between forced vital capacity (FVC) and the integrative index of pleural fibrosis in the subgroup with circumscribed pleural fibrosis. In the subgroup with diffuse pleural fibrosis, the obliteration of costophrenic angle(s), even with pleural fibrosis of limited extent, resulted in marked decrement in FVC% predicted; higher values of INDEX did not result in additional significant reductions of FVC. In those with both parenchymal and pleural abnormalities (n = 862) the pleural index was found to make a significant contribution, independent of that of parenchymal abnormalities, to decrements of FVC. Since pleural fibrosis has gradually become the predominant radiologically detectable abnormality in asbestos exposed workers, establishing its quantifiable functional relevance is useful.

Evidence for idiotypic sharing between conventional and naturally activated B cells.

In the primary immune response to hen egg-white lysozyme (HEL), approximately 50% of the primary anti-HEL antibody-forming cells (AFC) express IdXE, an idiotype absent from the secondary response. Spontaneous IgM enzyme-linked immunosorbent assay (ELISA)-AFC in spleen cells from naive A/J mice were analyzed for the occurrence of IdXE by use of two affinity-purified rabbit antisera, R213 and R8, each raised against a different primary IdXE+, anti-HEL mAb. Two ELISA-AFC assay methods were used: direct coating of immunoplates with R213 or development of IgM-producing ELISA-AFC with biotinylated R8. From ages 7 days until 6 months, 10-20% of spontaneous IgM AFC were found to be IdXE+. IdXE+ anti-HEL IgG1 monoclonal antibodies, 2F4 or 3C11 (100 micrograms/ml), completely inhibited binding of biotinylated R8 (0.5 micrograms/ml) to spontaneous IgM-AFC while IdXE-, anti-HEL IgG1, 5E11 and 2C7, showed no significant inhibition. Greater than 90% of IdXE+ spontaneous IgM-AFC were not HEL specific. We conclude that a dominant set of idiotopes found in a conventional antigen-driven immune response can also play a major role in the spontaneously activated B cell repertoire. Our data argue against a bifurcation of the immune system into a compartment of idiotypic network-related cells and an independent set of non-network cells subject to antigenic stimulation.

Angioscopy of arm vein infrainguinal bypass grafts.

Between January 1988 and December 1990, 56 patients underwent 66 arm vein infrainguinal bypass grafts for limb salvage. Thirty-nine grafts were intraoperatively monitored by the standard methods of continuous wave Doppler alone (30) and arteriography (9). Twenty-seven grafts were prepared and monitored by intraoperative angioscopy. No significant findings requiring intraoperative revision or correction were noted in the grafts monitored by the standard methods. However, in those grafts prepared and monitored by angioscopy, intraluminal abnormalities of the arm veins were detected and corrected in 20/27 (74%). None of the grafts prepared or monitored by angioscopy occluded within 30 days, whereas, in those grafts monitored by continuous wave Doppler and arteriography, 7/39 failed within 30 days, a primary patency rate of 32/39 (82%) (x2 with yates correction, p = 0.055). This study shows that angioscopic preparation and monitoring of arm vein bypass grafts allows the detection and correction of unsuspected intraluminal abnormalities, which appears to improve the early primary patency of arm vein infrainguinal bypass grafts.

Characterization of in vivo-activated T cell clones from peripheral blood of multiple sclerosis patients.

In vivo-activated interleukin-2 responsive T cell clones were generated from peripheral blood (PBL) of multiple sclerosis patients (MS) and normal subjects (N) by limiting dilution analysis. The frequency with which interleukin-2 responsive cells were cloned from PBL was higher in MS than N. CD8 was the predominant phenotype expressed by both MS (85%) and N (89%) clones. Seven clones from four MS patients but none from five N subjects specifically proliferated against myelin basic protein. These studies demonstrate the existence of MBP-reactive T cells in PBL of MS patients.

Popliteal-to-distal bypass grafts for limb salvage in diabetics.

Between January 1984 and August 1989, 117 diabetic patients with a palpable popliteal pulse but distal limb threatening ischaemia underwent 124 popliteal artery (or below) to distal bypass grafts. All grafts were intra-operatively monitored. The operative mortality was 0.8% and the 30 day primary patency 93%. Primary patencies at 1 and 3 years were 88.6 and 85.2%, respectively. The results of using the popliteal artery as the proximal graft inflow site in diabetes are comparable to other patient groups and to alternative more proximal inflow sites, but require a shorter length of vein graft with a shorter vein harvesting incision, avoid groin disection and result in a more peripheral operation.

Immunotherapy in autoimmune diseases.

Over the past decade, much progress has been made in our understanding of immunological tolerance of self-antigens and the genetic and molecular basis of T-cell recognition and activation that involves a ternary interaction of foreign or self-antigens with major histocompatibility complex molecules and T-cell antigen receptor gene products. Using this trimolecular complex as a target for immune intervention, animal models of spontaneously occurring and experimentally induced autoimmune diseases have provided opportunities for new immunomodulatory therapeutic approaches. Some of these have encouraging potential and are currently being applied in the immunotherapy of human autoimmune diseases.

Prevalence of antibodies reactive with a 64 kDa eye muscle membrane antigen in thyroid-associated ophthalmopathy.

We have determined the prevalences of antibodies reactive with certain pig eye muscle membrane antigens, as determined from SDS-polyacrylamide gel electrophoresis (PAGE) and western blotting, in patients with autoimmune thyroid disorders with or without associated ophthalmopathy. The most frequently detected antibody was that directed against a protein of 64 kDa. Antibodies against this antigen were detected in 33% of unselected patients with ophthalmopathy and in 75% of those with severe, active, orbital inflammation of less than 12 months duration. Such antibodies were detected also in 33% of patients with Graves' hyperthyroidism with no apparent eye disease and in 17% of patients with Hashimoto's thyroiditis with no eye disease but were not detected in patients with nonautoimmune thyroid disorders in 25 normal subjects tested. In the majority of sera showing reactivity with this protein, reactivity with membrane antigens of 55 and 95 kDa also were observed, suggesting that the three proteins were conformationally associated. Cytotoxic antibodies against human eye muscle cells, measured in antibody-dependent cell-mediated cytotoxicity (ADCC), were demonstrated in 37% of patients with ophthalmopathy. There was no significant correlation, in patients with eye disease, between positive ADCC tests and antibodies to the 64 kDa protein, the 64 kDa reactive antibody being found in 50% of patients with positive ADCC and in 35% of those with negative tests. In this study, a clear association has been shown between thyroid-associated ophthalmopathy and the detection, in immunoblotting, of antibodies reactive with a 64 kDa eye muscle membrane antigen.(ABSTRACT TRUNCATED AT 250 WORDS)

Tolerance and suppressor mechanisms in experimental autoimmune encephalomyelitis: implications for immunotherapy of human autoimmune diseases.

Recent advances in understanding antigen recognition at the level of the trimolecular complex have provided new approaches for selective immunotherapy. Many of these approaches have been applied successfully to the animal model experimental autoimmune encephalomyelitis, and some are being tested in the human disease multiple sclerosis. In addition, new approaches utilizing nonspecific modulation of immune function are being explored in animals and humans. Immunospecific therapy in autoimmune diseases will ultimately be based on understanding how the normal immune system maintains unresponsiveness to self and how this state of self-tolerance is broken. Strategies for specific immune intervention in human diseases based on components of the trimolecular complex will have to take into account the polymorphism of the major histocompatibility complex in humans and the degree of heterogeneity among autoimmune T cells that react with an autoantigen.
