Adrenocorticotropin (ACTH) induces phosphorylation of a cytoplasmic protein in intact isolated adrenocortical cells.

In 32P incorporation experiments with intact adrenocortical cells, adrenocorticotropin (ACTH) or adenosine 3',5'-cyclic monophosphate (cAMP) induced a rapid and transient increase of approximately 300-500% in the phosphorylation of a 32P-containing cytoplasmic protein of about 150,000 daltons (APS150). Half-maximal stimulation of APS150 phosphorylation was observed with about 3 pM ACTH. Receptor-bound cAMP, corticosterone production, and the appearance of phosphorylated APS150 increased in parallel with respect to both time and ACTH concentration. All three responses were dependent on extracellular calcium. Inhibition of protein synthesis with cycloheximide suggested a half-life of APS150 of about 10 min. The time course of 32P incorporation into ACTH-induced APS150 in the absence and presence of nonradioactive phosphate shows that the phosphorylation of APS150 is under simultaneous control of cAMP-dependent protein kinase and of phosphoatase activity. Thus a rapid ACTH-dependent and cAMP-dependent protein phosphorylation in intact adrenocortical cells within steroidogenic ACTH concentrations has now been demonstrated.

Steroidogenesis in isolated adrenal cells: excitation by calcium.

Calcium salts were found to replace ACTH in inducing steroidogenesis in isolated adrenocortical cells from rats. This Ca-specific stimulation occurred when the cation was presented to the cells in the presence of phosphate and carbonate as a counter-ions under conditions which favoured the formation of colloidal calcium. Colloid generation and stabilization was facilitated by the use of calcium buffers and gelatin. Stable soluble or sparingly soluble calcium complexes were inactive. The preparation of cells and metastable calcium solutions is described in detail. The Ca trigger was sensitive to Ca deprivation or inhibitors of Ca transport and could be replced by Sr. The relative role of Ca and cyclic AMP as second messengers is discussed.

Mode of action of peptide hormones.

The mode of action is discussed of the peptide hormones which trigger neosynthesis of a specific product in their target cells without being involved in any release step. Particular attention is paid to the early events elicited by ACTH in isolated adrenocortical cells. It is shown that extracellular calcium ions at physiological concentrations can serve as first messenger activating steroidogenesis if the isolated cells are pretreated in an appropriate ionic environment. Among other factors the extracellular calcium/phosphate ratio seems to be of importance. A model is proposed where calcium serves as direct messenger in the physiological activation by ACTH, cyclic AMP being a subserving factor maintaining full steroidogenesis.

Human granulocyte 6 phosphogluconate dehydrogenase. Purification by elective elution with NADP+, immunological and kinetic properties.

Human granulocyte 6 phosphogluconate dehydrogenase has been totally purified from a single patient with chronic granulocytic leukaemia. 48 mg of protein, of specific activity 20 IU per mg of protein, have been obtained in the course of three different steps only. The overall yield was 30 p. cent and the purification was 100 folds. Purified 6 phosphogluconate dehydrogenase was homogeneous when tested in acrylamide and acrylamide SDS gel electrophoresis or in immunodiffusion. The enzyme was immunologically identical in red blood cells, blood platelets and normal leukocytes. The fixation of both substrates, NADP-+ and 6 phosphogluconate, seemed to proceed through a non ordered mechanism. NADPH was an inhibitor strictly competitive with respect to NADP-+ and non competitive with respect to 6 phosphogluconate. 2-3 Diphosphoglycerate seemed to be able to bind on both the fixation sites of NADP-+ and 6 phosphogluconate. The inhibition by ATP was competitive with 6 phosphogluconate and non competitive with NADP-+. 6 phosphogluconate dehydrogenase was inactivated by SH reagents and was partially protected against this inactivation by both substrates. Both substrates protected the enzyme against thermal inactivation. The influence of ionic strength, pH and ions have been studied, and the results have been compared to those reported by other authors for erythrocyte enzyme.

Laboratory parameters in the natural history of liver cirrhosis.

The aim of the present study was to evaluate the changes of common laboratory parameters in the natural evolution of liver cirrhosis, as well as their relationships with the occurrence of ascites onset and death. Routine laboratory findings of 458 patients suffering from liver cirrhosis admitted to a 9-year follow-up period were retrospectively investigated, but only data of the 138 subjects (95 males and 43 females) who died within the follow-up period were considered. Data were grouped into different classes according to the months elapsed before ascites onset or death. The statistical differences among the various groups were evaluated employing analysis of variance and Turkey's test. No laboratory parameter showed statistically significant changes before or at the time of ascites onset, while significant modifications occurred in four liver indexes (albumin, total bilirubin, platelet count and gammaglobulin percentage) during the course of the illness, thus confirming the reliable prognostic value of these laboratory parameters in liver cirrhosis.

Effect of pre-treatment with inhaled furosemide on allergen nasal challenge.

The inhalation of furosemide has been reported to inhibit the bronchospasm induced by several agents. In the present study, we evaluated the effect of inhaled furosemide on the specific nasal challenge test in patients with allergic rhinitis. A total of 21 consecutive patients with allergic rhinitis (positive skin test and RAST) and a positive basal nasal provocation test (NPT) with the specific allergen were investigated. In each patient, we compared the changes in nasal air-flow (anterior rhinomanometry) during NPT after inhalation of placebo and 20 mg furosemide. The previously positive NPT response to the inhalation of the specific allergen became negative after the furosemide pretreatment in 16 patients (76.2%, p < 0.001, chi-square test). The nasal air flows during NPT were significantly increased after furosemide treatment with respect to placebo inhalation (F = 17.2, d.f. = 1 and 3; p < 0.03; covariance analysis). Our results suggest that the pretreatment with inhaled furosemide in atopic subjects is able to exert a protective effect on the nasal mucosa reactivity to the specific allergen. Therefore, the anti-reactive effect of the drug on the airways is not confined to the bronchial asthmatic response.

Nasal lavage cytometry in the diagnosis of nonallergic rhinitis with eosinophilia syndrome (NARES).

The nonallergic rhinitis with eosinophilia syndrome (NARES) is a type of chronic rhinitis, characterized by intense (> 20%) eosinophilia of the nasal secretions. In this study, screening for NARES in 81 patients with nonallergic chronic rhinitis was performed using a modified nasal lavage cytometric method. Twenty-one of the 81 investigated individuals (25.9%) showed nasal eosinophilia greater than 20%, therefore being diagnosed as having NARES. No statistical difference in total serum IgE concentration (Prist Pharmacia) was observed between the NARES subjects and those with other forms of chronic rhinitis, confirming the seemingly nonallergic nature of NARES. In 28.6% of these cases the presence of anosmia was detected: in 19.0% of cases with NARES (all subjects complaining of long-lasting symptoms and suffering from anosmia), nasal polyposis coexisted. Our results confirm that nasal lavage cytometry may represent a useful clinical tool in the diagnosis of chronic rhinitis.

[Huber needle in situ inpatients under continuous infusion chemotherapy: results of a study, Phase II].

PURPOSE: Chemotherapy administered as a continuous infusion is a widely used treatment in oncology. Huber needle deserves close attention during chemotherapy, but no data are reported on how long it can be left in situ without change. We therefore evaluated the feasibility of leaving in situ the needle for a prolonged time. METHODS: Patients candidated to continuous infusion chemotherapy were considered eligible for the study. The needle was changed at the end of the 21-day period when the patient started a new cycle of chemotherapy. On that occasion the site of injection was evaluated while replacing the needle. RESULTS: On 129 evaluable patients submitted to continuous infusion chemotherapy, 124 patients did not demonstrate any adverse cutaneous reaction. Five patients (3.8%) presented sores but we were able to continue the treatment leaving in situ the needle. CONCLUSION: Our results demonstrated that the needle can be left in situ for the entire time the patient is at home between cycles of chemotherapy. This procedure avoids patient stress and anxiety due to unjustified substitutions of the needle.

[Topical ampicilin and local infectious complications in oncological surgery].

420 consecutive patients having clean surgical operations were studied: 210 of them, selected at random, received 0.5-1 g of ampicillin into the wound before closure, the remainder being controls. Patients were examined for wound infection during the first postoperative week, in line with the opinion that topical antibiotic treatment can afford significant protection against wound infection caused by peroperative contamination. Wound infection has been defined as a local inflammation (redness and hyperthermia), usually with fever and/or discharge of pus. In some doubtful cases, a longer observation period was necessary to obtain a correct interpretation. Four out of 210 clean wounds (1.9%) became infected in the patients receiving ampicillin compared with 12 out of 210 (5.7%) in the control group. The difference could have arisen by chance less that one in a hundred times (p less than 0.01). It is our opinion therefore that after clean operations, the higher the risk of local infection, the more effective the topical use of antibiotics.

[Clinical significance of serum bile acid determination in liver diseases].

The recent availability of radioimmunological and enzymatic kits to measure serum bile acids (SBA) has allowed diffusion of this laboratory test. The aim of this paper is to provide an assessment of diagnostic and prognostic significance of SBA in liver cirrhosis, on the basis of the most recent data available to us. Measurement of these substances in serum does not appear to be very useful either in screening or in differential diagnosis of liver disorders, since it seems unable to provide further information with respect to commonly used parameters. On the contrary, SBA show a remarkable predictive value both in chronic active hepatitis and in liver cirrhosis. SBA measurement may therefore become a useful tool in planning the therapeutic approach to these hepatic diseases.

Nasal secretion specific IgE in rhinitis patients.

Forty-five subjects with rhinitis of suspected allergic origin underwent immune and allergy testing including skin-tests and immuno-enzymatic assay of serum and nasal secretion specific IgE (RAST). Nasal secretion was collected using a cotton tampon inserted in both middle meati. A statistical analysis comparing the results of skin-tests and serum RAST was performed: the results obtained with these two techniques were very homogeneous (86.7% accuracy). An overall concordance (82.2%) was also observed by comparing skin-tests and nasal secretion RAST. Finally, the results of serum and nasal secretion RAST were compared: in this case totally superimposed data were reported (overall accuracy: 93.3%). In all cases the comparison between RAST classes in serum and nasal secretion was carried out: the difference between the semiquantitative evaluation of reaginic concentration classes using these two methods is not statistically significant (chi-square test). This technique of nasal secretion collection is simple and with no risk involved.
