[Force systems for correction of lower molar inclination].

The authors analyzed the uprighting movement and suggested forces system developing wished movements considering M/F ratio generated on brackets. The effects of these system on anchorage segment were evaluated. Use of concurrent or non-concurrent was analyzed in the light of their "cohernce" with treatment goals.

[Cephalometric evaluation of lower incisor position].

The Authors examined some cephalometric values usually utilized to evaluate lower incisors inclination or anteroposterior position. Mean values and standard deviations of these parameters in a 194 subjects sample was calculated. Validity of these valued in different typology and skeletric classes was finally verified.

A phase II study with danazol in metastatic breast cancer.

Danazol is a synthetic steroid that inhibits the gonadotropin secretion. Its efficacy was tested in 27 patients with metastatic breast cancer at the dose of 200 mg three times daily. Characteristics of patients were as follows. The median age was 57 years (range, 44-85). All patients were postmenopausal, and all had previously received a median of two (range, 1-3) endocrine therapies. Estrogen receptor status was known in 12 (positive in five of 12; negative in seven of 12). Dominant sites of metastases were lung in seven, bone in ten, liver in three, and soft tissue in seven. Six of 27 patients were unevaluable for response (early death, four; lost to follow-up, two). Three of 21 patients showed an objective response, eight of 21 obtained stabilization of disease, and eight progressed. The therapy was well tolerated in the majority: gastric pain was observed in three and nausea in two. Danazol could have a role in the treatment of metastatic breast cancer as an alternative regimen when other treatment has failed.

[Rheumatoid arthritis. Considerations on a rational therapeutic strategy].

Rheumatoid Arthritis (RA) is a progressive disease of unknown aetiology which may be caused by faulty immune mechanisms. Early diagnosis and correct treatment can be extremely effective. Fast and lasting results can only be obtained by an appropriate combination of NSAID and DMARD drugs which both reduce subjective symptoms and halt the progression of the disease.

A dose-finding study of ifosfamide by three-day continuous infusion in pretreated, advanced breast cancer patients.

The purpose of the study was to establish the maximum tolerated dose of ifosfamide, administered over 72 hr, in metastatic breast cancer patients, pretreated with chemotherapy. Ifosfamide and mesna were given at the same dose, in the same solution, using a portable Pharmacia CADD-1 pump connected to a central venous access, at three dose levels: 7.5 g/m2 (6 patients), 9 g/m2 (8 patients), 10.5 g/m2 (3 patients); the courses were repeated every 3 weeks. Seventeen patients with a median age of 55 years (range, 34-68) and median performance status of 0 (range, 0-2) were treated. The patients were pretreated with a median of 2 (range, 1-3) prior regimens including anthracyclines in 14 patients and paclitaxel in 9. Dose-limiting toxicity was defined as the occurrence of any of the following events in > or = 2/6 patients: absolute neutrophil count < 500/ml for > 7 days or < 100/ml for > 3 days; febrile neutropenia; grade 4 thrombocytopenia; any grade > or = 3 nonhematologic toxicity. The dose-limiting toxicities were febrile neutropenia and grade 4 thrombocytopenia in 2/3 patients treated at 10.5 g/m2. Seven patients achieved an objective response (response rate 41%; 95% CI, 18% to 67%). We conclude that 72-hr infusion of ifosfamide is feasible in ambulatory patients. The recommended dose for phase II studies is 9 g/m2, with courses repeated every 21 days.

[Muscular physiotherapy in patients with temporomandibular disorders. Controlled clinical trial].

BACKGROUND: The purpose of the present study is to evaluate the short term effects of physiotherapy and counseling in the treatment of myofascial pain of the jaw muscles. METHODS: Forty consecutive myogenous TMD patients, were assigned to two treatment groups with a balanced-block randomization. The first group received only counseling, the second group received counseling supplemented by physiotherapy. The patients were followed for a period of three months. Data were collected on two different occasions: T0 = baseline and T1 = three months after the start of treatment. Treatment success was decided on the basis of an index called treatment contrast (Tc). It allowed a subjective, an objective and a total evaluation of the effects of the two therapeutic protocols. RESULTS: There was no significant difference between groups in the outcome measures except for the subjective evaluation of treatment success: physiotherapy seems to be slightly more effective than counseling. CONCLUSIONS: Elective therapy for the treatment of TMD patients remains the conservative one. Success of this therapy depend on a good patient--clinician relation, which improves patient's compliance.

[Epidemiologic study of patients with craniomandibular disorders. Report of data and clinical findings].

BACKGROUND: The purpose of this retrospective study was to identify the subjective and objective characteristics of a population referred to a clinic for diagnosis and treatment of orofacial pain and/or temporomandibular disorders. METHODS: Beginning in 1993, 825 patients, consecutively referred to the University of Naples Federico II , were examined and their records entered into a database. These data were collected by trained clinicians. Diagnostic subgroups were identified following the Research Diagnostic Criteria for temporomandibular disorders (RDC). RESULTS: Seventy-nine percent of patients were female, the mean age of the sample was 31.3+/-13 years (range: 5 to 74 years), most of patients were between 15 and 39 years of age. Based on diagnostic subgroups, patients were divided into: patients affected with myogenous pain (59%), arthrogenous pain (13%), arthrogenous and myogenous pain (16%) and fibromyalgia (4%). Sixty-three percent of the sample reported recent headaches, 53% reported parafunction, and 28% reported a previous trauma. Eighty-one percent reported spontaneous pain, which was chronic in 83% of them. The majority of patients (78%) presented a relatively high cultural BACKGROUND. CONCLUSIONS: These data appear to agree with other epidemiologic studies and depict the TMD treatment-seeking population as a predominantly female population during child-bearing years with multiple chronic pain complaints.

[Super-elastic wire used in orthodontics].

The Authors describe the unusual super-elastic property of the Japanese NiTi alloy wire. They examine the clinical applications in relation to the relatively constant force delivered during the deactivation and in relation to the new possibility to shape wire which are offered by the Archmate Wire Bender.
