A prospective randomized study of 204 consecutive upper gastrointestinal endoscopies performed with a flexible esophago gastroduodenoscope was made to determine the efficacy of diazepam as compared to the lack of sedation as pre-medication. The mental attitude of the ambulatory patients was assessed prior to the endoscopy. It was found that calm patients were satisfactorily endoscoped with or without sedation. The anxious patient had an unsatisfactory endoscopy more frequently when no sedative was used. Those patients with complication conditions (hypovolemia, liver failure, anemia, cardiac failure, etc.) were adequately studied with or without sedation. Age or sex of patients did not affect results in either group. Results show that a majority of patients can be satisfactorily endoscoped without sedation.
The clinical cases of 391 men and 609 women, adolescent all of them, are included in this study. T.A. values were obtained by the Korotkoff method and the lower figures in both measurements analized. The mean value for the men's cases were 112/72/67 and for the women's cases 105/71/67 respectively for K1, K4 and K5. The difference between the sistolic pressures is significant. Taking the mean men and 126/89/86 for the women. Such values might represent the higher values plus two standard derivations it was possible to obtain 139/92/88 for the normal limit. At higher values we found K1 at 1% for men and 10% for women; K4 at 1.2% for men and 5.7% for women; for K5 at 1.1% for men and 2.2% for women. There is a direct relation between obesity and higher figures in T.A. for K1 and K4 in both sexes (p.0.01). The use of too much salt influenced the coming up of K4 to (p.0.01).

Complications and mortality of blood hypertension depend on the time blood pressure levels remain above normal as well as on the values they reach along the evolution. In order to detect blood hypertension before it is well established as a disease it is necessary to identify those individuals prone to it or those with predictive data that they will develop hypertension. Distribution of blood pressure levels during the pediatric age is one of the first studies that should be done to identify those children with high blood pressure or those who will develop the disease. After reviewing the literature we found that there is disagreement on the criteria of normal and upper normal limits of blood pressure during childhood and adolescence. This explains the variability of the reports on incidence or prevalence of hypertension during this age. There are other factors of variability as methodology on blood pressure registration, geographic and time changes. Most authors agree that blood pressure levels are higher as age increases and boys have higher blood pressure level than girls. Our findings are within the those reported by others.
To assess the presence of cytomegalovirus in various tissues and its relevance to the development of graft atherosclerosis, 54 postmortem paraffin-embedded tissue samples from 15 heart transplant recipients who survived more than 100 days were analyzed by the polymerase chain reaction for cytomegalovirus. Eight patients had known previous cytomegalovirus exposure; 7 patients did not. Of the eight with known previous exposure, three patients (38%) died of graft atherosclerosis versus four (57%) of the seven patients without previous exposure. Of the 54 specimens, 49 were positive for beta-globin (inclusive positive control), including 38 coronary artery, five lung, five gastrointestinal, and one kidney. Only two coronary artery specimens were cytomegalovirus positive in a single patient with known cytomegalovirus exposure who did not have evidence of graft atherosclerosis at autopsy. Other tissues tested showed positive lung and stomach specimens in the patient with cytomegalovirus-positive coronary artery specimens and positive kidney, lung, and gastrointestinal specimens in a second patient. No specimens were cytomegalovirus positive in the remaining patients, despite the presence of graft atherosclerosis or previous cytomegalovirus exposure. Our data do not support the hypothesis that graft atherosclerosis is associated with latent cytomegalovirus infection of the coronary arteries. The role of cytomegalovirus in the pathogenesis of graft atherosclerosis is unknown, but possibly it represents a modulation of the immune system by remote infection.
