Sugiura procedure outside Japan. The Mexican experience.

In the last 10 years, we operated on 231 patients with hemorrhagic portal hypertension. Most of these patients had some form of liver disease. We performed various surgical procedures: 47 conventional shunts with H grafts and terminolateral portacaval shunts with arterialization of the portal stump, 139 selective Warren shunts, and in those patients in whom a selective portasystemic shunt could not be performed for technical reasons, esophagogastric devascularization in the form of the Sugiura procedure. Forty-five patients were treated with the Sugiura procedure as a one stage or two stage procedure. A total of 68 emergency and elective operations were performed. The operative mortality rate for the emergency thoracic operation was 41 percent and for the abdominal operation, 42 percent. The overall operative mortality rate in the emergency group was 41 percent. The incidence of recurrent variceal bleeding and encephalopathy was 0 in the surviving patients. The survival rate at 3 year follow-up was 40 percent. The elective group was made up of 24 patients. Eighteen patients had a two stage procedure and 6 patients had a one stage procedure. The operative mortality rate for the abdominal operation was 11 percent, whereas that for the thoracic operation was 7 percent. The operative mortality rate for the one stage procedure was 16 percent. The overall operative mortality rate in the elective group was 10.8 percent. None of these patients had recurrent variceal bleeding and encephalopathy developed in only one (5 percent). The encephalopathy was easily controlled with medical treatment. The 3 year survival rate was 83 percent. We conclude that the Sugiura procedure is an effective procedure to treat hemorrhagic portal hypertension when a selective shunt cannot be performed.

Prenatal ultrasonographic diagnosis of congenital adenomatoid malformation of the lung. Correlation with pathology and implications for pregnancy management.

Congenital cystic adenomatoid malformation of the lung is an uncommon malformation. The solid type III variety is the rarest and carries the poorest prognosis. This report describes the prenatal diagnosis of a case of congenital cystic adenomatoid malformation type III at 24 weeks' gestation. The pathologic features of the malformation and the etiology and prognostic significance of hydramnios and anasarca are discussed. This case indicates that the malformation can be diagnosed early enough to allow for therapeutic intervention.

Supradiaphragmatic liver in the lung.

Heterotopic liver tissue has only been rarely described, and then usually in the immediate vicinity of and in communication with the liver or extrahepatic biliary tree. We studied a case of an isolated heterotopic nodule of liver in the lung of an infant in which no connection to the hepatobiliary system was present, and in whom the diaphragm was intact.

[Goniotomy in congenital glaucoma].

Sixty eyes presenting with congenital glaucoma and operated on by goniotomy, have been followed-up for 1 to 30 years (mean 10.4 years). Intraocular pressure was normalized in 91 p. cent of cases, either by goniotomy alone with or without medical treatment, or by goniotomy with an associated fistulizing operation, if necessary. When preoperative intraocular pressure is less than 40 mmHg, 2 cases out of 3 are normalized, but when it is more than 40 mmHg, only 1 case out of 3 is normalized.

Fine structure of the receptor-free epithelium in the vomeronasal organ of the rat.

The topography and distribution of the epithelia of the vomeronasal organ were investigated in the adult male rat. Special attention was given to the light and transmission electron microscopic structure of the area that is known to be free of receptor cells. In contrast to several brief descriptions in which many cell types have been reported, only one cell type is presently described in different functional phases. In the rat, this epithelium does not show the typical features of a respiratory or, in more general terms, non-sensory epithelium. The authors propose to call this epithelium as "receptor-free" rather than respiratory, non-sensory or ciliated. The importance of the vomeronasal organ in the maintenance of olfacto-endocrine mechanisms and reproductive behavior is discussed.

Stereologic evaluation of the vasculature in a MX1 xenotransplanted tumour model after combinations of treatment with ifosfamide, hyperthermia and irradiation.

The vascularization of tumours is a critical parameter of their growing and metastatic behaviour. However, little is known about the morphologic reactions of the microvasculature, especially the capillary bed of tumours and the adjacent tissue. In this study, the vessels in MX1 xenotransplants in athymic nu/nu nude mice were quantified and the angioarchitecture was visualized with the aim of presenting stereologic parameters of vessels based on a morphometric analysis of post mortem tissue blocks which were processed by standard histological procedures. In order to study changes of the microvasculature of MX1 tumours, the xenotransplanted nude mice were treated by different therapeutic regimens. Standardized hyperthermia, ifosfamide and irradiation therapies were applied. Special interest was focused on early changes of capillaries and of the pre- as well as post-terminal vascular bed. The stereologic evaluation of capillaries and larger vessels immediately after the therapy with ifosfamide and hyperthermia shows an increase of the mean capillary sizes. Furthermore, tumour samples after the 5th day of irradiation (5 x 2 Gy) and combinations of irradiation and chemotherapy treatment have been investigated. After 5 days of irradiation, a significant decrease of the vascular density was found. The results presented here clearly show that the timing and the mode of therapy influence the capillary morphology and periterminal vasculature of xenotransplanted MX1 tumours.
