Friedreich's ataxia and oral glucose tolerance: II. The effect of ingested glucose on serum growth hormone in homozygotes, obligate heterozygotes and potential carriers of the Friedreich's ataxia gene.

Homozygotes, obligatory heterozygotes and potential carriers of the Friedreich's Ataxia gene were tested with an oral glucose tolerance in order to assess a. the prevalence of abnormalities in glucose handling, b. the secretory pattern of two "diabetogenic" hormones, growth hormones and prolactin and c. to evaluate the possibility for detection of an abnormal trend in these hormones to be used as a genetic marker. Despite the high prevalence of glucose abnormalities horizontally and vertically in these families, the basal output and responses of these metabolic hormones to a glucose challenge were appropriate and thus not characteristic of any of the above groups.

Isoelectric focusing of cells using zwitterionic buffers.

A simple method of isoelectric focusing of cells is described. The pH gradient, superimposed on a density gradient, is developed by generating opposing concentration gradients of two zwitterionic buffers. The method can be used as a cell separation technique or as a means of characterizing the cell type on the basis of the focusing pH. Focusing is rapid and thus the method is of special advantage in its application to cells.

Ball milling as a measure of crushing stength of granules.

When granules are milled in a ball mill, the size decrease follows a modification of Kick's law. The Briggsian decay constant, here denoted as the attritional crushing strength, shows correlation with the Harwood-Pilpel crushing strength. Both crushing strengths show a correlation with the amount of granulating agent (povidone) added to the granulation, i.e., the more povidone added, the harder the granule.

Influence of granulation liquid flow rate on particle-size distribution in spray-granulate products.

Granulations were made at various air speeds and various rates of addition of granulating liquid in a spray-granulatin assembly. The ensuring particle sizes were log-normal, and the data substantiate that the process is of the nature previously described. The mean diameter obtained was approximately proportional to the liquid flow rate squared but was independent of air rate. The standard deviation was independent of both parameters.

Subcapital fracture of the hip: a rare complication of intertrochanteric fracture of the femur.

Three cases of subcapital fracture of the hip that occurred after an intertochanteric fracture of the femur had been nailed have been described in the literature. The tip of the nail failed to penetrate the femoral head and a stress fracture occurred in two at the tip of the nail and in the other at the tip of a blade plate. This paper reports three further cases of this recently recognized and rare complication of intetrochanteric fractures. It is emphasized that the condition must be recognized and treated without delay.

Surgical bifascicular block.

Electrophysiological studies were performed in 18 patients who developed bifascicular block after repair of ventricular septal defect (VSD) or tetralogy of Fallot (TF). Two had transient complete heart block (CHB) in the immediate postoperative period. The P-A intervals were normal in all. A-H and H-V intervals were prolonged in three and four cases, respectively. Atrial pacing at progressively increasing heart rates was performed in 15 patients; two developed type II block distal to the His bundle (H). The effective and functional refractory periods (ERP and FRP) of the atrium (11 cases) were normal in all. The ERP of the A-V node (seven cases) was prolonged in found and the FRP was increased in three. The ERP of the ventricular specialized conduction system was measured in two cases and was prolonged in one. In all, seven cases had abnormalities indicating disease of the A-V node and/or His-Purkinje system. Recording of intervals, atrial pacing, and determination of refractory periods (RPs) was necessary to reveal all conduction abnormalities. One patient died of unrelated causes. The others are alive and in sinus rhythm with intact conduction 3 to 16.5 years following surgery (mean follow-up of 8.3 plus or minus 0.95 years). The clinical course in patients with normal and abnormal findings was equally benign. Prophylactic insertion of demand pacemakers does not appear indicated in these patients.

Simplified method for the purification of group A streptococcal M-proteins: solution of the multiple banding problem.

A simple and rapid procedure for the isolation in high yield (about a 30% recovery based on the total 30 to 60% ammonium sulfate recovery) of homogeneous purified group A streptococcal M-protein is described. M-proteins extracted from whole cells of group A streptococci by treatment with hot HCl were neutralized, fractionated with ammonium sulfate, dialyzed, lyophilized, and then subjected to treatment with hot 60% trichloroacetic acid. This was shown to produce an M-protein preparation, free of group A carbohydrate activity and extraneous antigens, in yields up to 10-fold higher than previous methods in about one-fifth the time. These M-protein preparations were shown to: (i) have similar amino acid compositions to their respective type-specific proteins purified by diethylaminoethyl and O-(carboxymethyl) cellulose chromatography, (ii) react with their respective type-specific antisera in Ouchterlony diffusion, (iii) produce antisera in rabbits capable of promoting streptococcal long-chain formation in vitro, and (iv) give only one major band on polyacrylamide gel disk electrophoresis. The data allow for an explanation of the hitherto described multiple banding M-proteins seen on acrylamide electrophoresis.
