Chylothorax occurring 23 years post-irradiation: literature review and management strategies.

Chylothorax is an uncommon condition rarely seen outside the clinical scenario of trauma or malignancy, and management may be difficult. We report the case of a 52-year-old man with a right chylothorax occurring 23 years after mantle irradiation for Hodgkin's disease. In addition, a Medline literature search was performed. Chylothorax occurring post-irradiation is rare and the aetiology is uncertain. Recurrent malignancy must be excluded. There are a number of management options and our patient was successfully treated with a combination of thoracocentesis, medium chain triglyceride diet and thoracoscopy with talc pleurodesis. There was no evidence of recurrent chylothorax at 2 years follow up. Prompt assessment and treatment is required. Surgical measures need to be considered early if medical treatment is unsuccessful.

SAPPHIRE: scenarios, architecture, and process.

General Medical Practice (GMP) information systems within the UK are becoming more sophisticated and more complex and are widely available from numerous suppliers. Although such systems are viewed as being important, they are problematic in terms of interpreting and assessing their usefulness, and their impact upon work and the organisation (G. Walsham, Interpreting Information Systems in Organizations (Wiley, Chichester, 1993)). In particular, it is difficult for any who have an interest in these systems to apply existing technical specifications to a specific situation, and to match individual requirements with the supplier's products. The research project SAPPHIRE seeks to inform the decision making of stakeholders, e.g. GPs, facilitators and suppliers, with respect to procurement, update, design and supply of GMP systems by developing the means of evaluating such systems, and by facilitating an accreditation process through that evaluation. This extended paper introduces the multi-faceted approach, scenarios, architecture and process of SAPPHIRE.

Educational audit of postgraduate medical education.

This project examined the academic audit of postgraduate medical education, and went on to research, design and implement an educational audit system for use in Mersey Region, UK. Issues found to be significant included ownership of both standards and results, norming, and the levels of aggregation of data. The project has demonstrated that designing and implementing a system of feedback which is effective and sustainable is feasible, but requires the agreement and support of tutors as to its objectives and use. The resulting system has now been tested across a range of courses and programmes of meetings in Mersey Region, and has been accepted by the Postgraduate Office as the basis for future development.
